Protection & Prevention Keys to Summer Foot Health: FL DPM

There are specific summer foot hazards, says Dr. Espeleta Marius, a Cape Coral podiatrist.

More foot injuries happen during the summer because people are more lax, go barefoot and don't wear the proper shoes, he says. "People take their feet for granted and don't know it's hurting until something goes wrong," Marius says. "It's one of the most take-it-
for-granted body parts."

Marius tells his patients to wear shoes at the beach. "People go barefoot, and that's actually dangerous," he says. "There are sharp shells, broken glass and foreign objects on the ground." He'd like to see people forgo flimsy flip-flops in favor of wearing sturdy brands such as Birkenstocks or Tevas, which offer more support. Marius reminds them to put sunscreen on their feet. Skin cancer can develop on the top of the foot as well. "The goal should
be protection and prevention," he said.

DPM’s on Forbes Best Paying Jobs List

Why do financially pushy parents want their children to marry doctors? Because, as Willie Sutton said of banks, that is where the money is.

The medical profession dominates the top end of our list of the 25 best and worst paying jobs in America. Surgeons are No. 1, with the next seven spots taken by various sorts of specialist practitioners. These numbers are drawn from the government's National, State and Metropolitan Area Occupational Employment and Wage Estimates. They, in turn, are based on a national survey of employers (the latest available one is from 2004) of every size and in all industry sectors. They cover 800 occupations

Here’s how podiatrists fared.

Mean Annual Salary: $111,130
Top Paying Industry: Offices of other health practitioners ($116,820)
Top Paying State: Kentucky ($170,740)
Top Paying Metro: Birmingham, AL ($185,230)

Podiatrists earned a bit more than the $110,590 mean income of lawyers.

RE: 3" Heels Create 7X More Stress Than 2" Heels: MO DPM

Dr. Holly Korges’ quote has greatly exaggerated the influence of 3” elevation over a 2” elevation. With no heel elevation the stress applied to the forefoot is 25% of the weight applied to the limb. As the heel is elevated to 2” there is a reversal of the forefoot to rear foot weight distribution, causing the stress to the forefoot to increase 3x (from 25% to 75%).

The increase at 3” is barely greater than anything over the 2” level. Even if the heel was totally off the ground, the forefoot would support 4x as much as at 0” elevation. Moreover, If the heel
elevation is greater than 3” the center of gravity would have to be displaced towards the heel (to prevent falling forward), thus decreasing the stress to the forefoot.

RE: Diabetic Golf Shoes (Dwayne Jacobus, DPM)

New Balance carries a golf shoe that comes in three widths and has a removable insert to accommodate the diabetic insert. This shoe does not have a SADMERC letter attached to it so I would be careful billing for it. Dunham makes steel toe shoes and boots that have removable inserts and come in 3 widths as well. None of these shoes/boots have SADMERC letters attached to them. Medicare does not require at this time a SADMERC letter for Diabetic shoes but they must meet certain requirements. They must at least come in three widths and be able to accommodate a diabetic Insert to name a few.

RE: Periodic Medical Exams of Diabetics By Podiatric Physicians (Kenneth Malkin, DPM)

Dr. Malkin said: "I am staking my professional reputation that diabetics are entitled to medical exams of the feet which have nothing to do with at-risk foot care." Kudos of praise to Dr. Malkin for standing up to insurers and government for diabetic care.

It is well known in the medical community that ophthalmologic exams are necessary for diabetics about twice/year. These examinations are not connected to the presence of symptoms.

It is time to stand up for diabetic patients and give them what they absolutely need. Some diabetic patients must be seen even more often than once yearly, as we know. This includes pressure testing and the latest modalities podiatry has available to treat this very difficult disease...and includes getting paid for these examinations.
