MI Podiatrist Recommends Wearing Shoes Inside Your Home
 
If you’ve ever stepped on a LEGO, then you’ve missed something very special. Even if you don’t have LEGOs littering your floor, there are a few serious reasons why you might want to keep shoes on in your house. Nicole Brouyette, DPM is a podiatrist at Henry Ford Health. She said that with more people working from home, she has seen an increase in foot pain and a condition called plantar fasciitis. She said a rigid shoe to protect the bottom of the foot and allow the arch to be supported has been beneficial to keep the alignment of your joints.

Also, older people can benefit from the added stability and traction a shoe provides. Slips and falls in the home are a major risk for seniors. Diabetics with peripheral neuropathy sometimes cannot feel the bottom of their foot, and the added protection of a shoe could be beneficial. While Dr. Brouyette is in favor of people wearing shoes indoors, she recommends having a dedicated pair of indoor shoes that you change into when you come home -- ideally a pair with good arch support and some traction.

VA Podiatrist Interviews Benno Nigg
 
Ben Pearl, DPM interviewed Benno Nigg, PhD for his upcoming participation in a Biomechanics Summit that will be taking place in June. Benno still continues his research at the Human Performance lab in Calgary. Nigg explained the principle behind small muscles with large levers in the foot, emphasizing the development of small muscles that cross the ankle. These muscles can contribute to stability with low energy expenditure and are often overlooked. Nigg gave an example of a professional middle distance runner he helped by having him train these muscles 2 hours a day. The athlete was back to normal training in 3 weeks! In the video below, he discussed the rationale for the preferred movement path. We reset to movement patterns that are comfortable. This theory is indirectly supported by lower muscle expenditure and greater comfort in the preferred movement path.

Pearl plans to continue the biomechanics summit series and will be looking for different speakers in his next summit to integrate biomechanics principles in foot snd ankle surgery as well. He feels that podiatry is falling behind other disciplines in the area of biomechanics and needs to be more proactive in staying current in concepts to remain relevant in this space.

OH Podiatrist, Lawmakers Introduce Prevent BLEEDing Act
 
Reps. Brad Wenstrup, DPM (R-OH) and Tom O’Halleran (D-AZ) and U.S. Sens. Bob Menendez (D-NJ) and John Boozman (R-AR) have introduced bipartisan legislation to expand access and training to life-saving bleeding control kits in communities and public gathering places.

Race, Ethnicity, and Rurality Associated With Amputation or Death with DFUs
 
Investigators from the University of Wisconsin School of Medicine and Public Health as well as the William S. Middleton Hospital, Department of Veterans Affairs, conducted a cohort study which concluded that rural patients with diabetic foot ulcers identified as Black had a greater than 10 absolute percentage increase in the rate of major amputation or death compared with the overall cohort. 
 
Of the 124,487 patients hospitalized with diabetic foot ulcers and who were Medicare beneficiaries, 17.6% underwent major (above-ankle) leg amputation or died within 30 days of hospital discharge; proportions increased to 18.3% for rural patients and 21.9% for those identifying as Black. The proportion further increased to 28.0% for patients who identified as both rural and Black, suggesting a role for intersectionality. (Meghan B. Brennan, et al., April 21, 2022.)

I practiced in a small town between Orlando and Tampa. We saw quite a few gout patients. I was always referred patients by FP/GPs, internists, and even rheumatologists. We took care of the acute attack, but always sent them back for them to handle the chronic condition. I suppose it’s not in my license/playbook, but I managed hyperuricemia in my practice. 
 
I fought Medicaid for a patient who was allergic to allopurinol to get him on Urolic. I received no help from his PCP. I even had help from the then governor. Years later, when I met the patient, he had tophi on his face, arms, feet, and chest. No one would treat him. Did I go out of my scope of practice? I think I did. But what else should I have done? Like many patients whose PCPs were not available and patients who ran out of insulin, I covered them for a week. Say what you want. Our job is clear, our license is not.

One of the things that I talk to my patients about more than anything else is expectations. I always talk with them about using a topical medication, and the one that I use exclusively is Tolcylen. Over the years, I have gotten good results. With co-pays and co-insurance as high as they are nowadays, patients may not be willing to pay high amounts for prescription topicals. I have found the efficacy of Tolcylen along with using a topical antifungal powder works well.
 
Additionally, from a business perspective, having something readily available for dispensing is something the patients want. I have now incorporated the use of Tolcylen powder/foot soaks into my regimen, also to treat the foot environment. When appropriate, I also talk with patients about treating from “the inside out and the outside in”, and if the patient is a candidate, or is willing, I will use oral terbinafine in conjunction with Tolcylen.

I have prescribed both Jublia and Kerydin hundreds of times over the years. Patient co-pays have ranged from zero to $1,100, based on insurance plans and pharmacy benefit. Sometimes manufacturers' coupons lowered the cost significantly. Prior to the availability of the aforementioned topical antifungals, ciclopirox was the most prescribed topical for onychomycosis.
 
Five years ago, I started dispensing Tolcylen and it has become the mainstay of topical antifungals in my practice. The combination of proven activity against a variety of common dermatophytes and several cosmetic ingredients has made this extremely popular with my patients. It has also become a constant profit center as patients love it and buy tube after tube.

In some populations, such as patients with diabetes, PAD, chronic edema, immunosuppression, onychomycosis is associated with increased incidence of repeated cellulitis, ulceration, and other significant poor outcomes. We have known this ever since the classic study by Jeffrey Robbins, DPM.
 
The use of ineffective therapies denies patients who should be treated, or who desire treatment, of effective treatment.
