BUSPM Seniors Achieve 100 Percent Passage Rate on APMLE

For the second year in a row, seniors in Barry University’s Podiatric Medicine program have achieved a 100 percent passing rate on both the written and clinical APMLE Part II exams. The national average is 90 percent. In all, 43 Barry seniors took the exam.

Dr. Albert Armstrong

“In short, this unparalleled success is a combination of great students, great staff, and great faculty,” said Dr. Albert Armstrong, dean of Barry’s School of Podiatric Medicine. “It all starts with our admissions team. When we admit high quality students, which we have, and prepare them well, which we do, we can expect great outcomes.” Armstrong also attributed the results to board preparation courses and the ability for seniors to take time off to study.

NJ Podiatrist Discusses Advantages of Micro-Drilling

David Zuckerman, DPM presented new, patented technology at the Alabama Podiatry Association’s annual meeting earlier this month in Destin, FL. Impedance technology is now available as a controlled method for micro-drilling. Micro-drilling allows podiatrists to create micropores into patient toenails in an effort to more effectively deliver antifungal topical medications to the source of a fungal infection. Due to impedance technology, the drill knows when to auto-retract before it ever reaches the soft tissue of a patient’s nail bed. This also allows patients to keep their toenails intact.

Dr. David Zuckerman

One study from the technology’s manufacturing company, PathScientific, shows that nail trephination aids in medication delivery to the nail bed with 71 percent of patients showing improvement in clinical assessment in comparison to 28 percent in the placebo group. Another study concluded that treatment of subungual onychomycosis with topical terbinafine delivered to the nail bed via transungual microconduits had 64% of the subjects showing improvement compared to 28% in the placebo group.

IN THE COURTS
Jury Convicts Physician of HIPAA Violations

Among the many impermissible uses and disclosures of protected health information (PHI) is the sale and use by third parties. Specifically, 45 CFR § 164.502(5)(ii)(A) states, “[e]xcept pursuant to an in compliance with § 164.508(a)(4), a covered entity or business associated may not sell protected health information.” The exceptions are very narrow and require the express consent of the patient, as well as other required items. The exceptions are very narrow and require the express consent of the patient, as well as other required items.

After a five-day trial, a federal jury in Springfield, MA, convicted a physician of the following: disclosing patients’ PHI to a sales representative at pharma company Warner Chilcott without the knowledge or consent of the patient in violation of HIPAA; accepting remuneration in violation of the anti-kickback statute, which Warner Chilcott admitted to in 2015; and lying to federal agents during the investigation.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Financial Reports Your Practice Needs to Run - Part 7

Most physicians readily admit that classes on how to run a successful medical practice were not part of the curriculum in medical school. While "financial reporting," might not have been a class at your medical school, experts say you need to at least become familiar with understanding some of the basic reports. Here are seven to track.

7. Value-Based Reporting - "As practices move toward a value-based reimbursement environment, they need to capture clinical quality measures for submission to the [Merit-based Incentive Payment Systems] and others. With [clinical registries], clinicians can view practice and physician quality scores and benchmarks against peers. Quality measures also uncover potential areas for quality improvement and identify specific patients and gaps in care." - John Bender, Board of Directors, AAFP

CODINGLINE CORNER
Query: Wound Care LCD Update

I’ve recently been getting denials for medical necessity when billing CPT 97597 with the diagnosis L89.891. When the wound care LCD was updated 4/16/18 (Wound Care Coding Companion for Wound Care L37228 (A55909)), it appears that CPT 97597/97598 were grouped together with CPT 11042-11407, so L89.891/L89.892 are no longer considered medically necessary diagnoses.

Does anyone have a recommendation on what to do besides appeal every one of these claims?

Hannah Gross, CPC, Bingham Farms, MI

Response: The fact is you are using the wrong ICD-10 root codes. Do not use L89, but rather L97 codes instead. Then, resubmit and you will be paid in earnest.

RESPONSES/COMMENTS (CLINICAL)
From: Robert S. Schwartz, CPed

Memory foams are foams that have “memory” to rebound to their original thickness and density when non-weight-bearing. The higher durometer ones, such as midsoles and outsoles of footwear (40 durometer and above on the ‘Sure A’ scale) do not pack down, or bottom out. Most memory foams we use for insoles are made of 15 to 25 durometer polyurethane such as PPT and Poron.

These are highly successful in distributing cushioning over the entire foot in proportion to forces from above and below. Skechers markets their “memory foam’ shoes that do not fulfill the above criteria. They use a much softer material than described above.
