Over-Prescribing Can Lead to Antibiotic Resistance In Diabetics: PA Podiatrist

Diabetic foot infections are an increasingly common problem, but proper care can save limbs and, ultimately, lives, suggest new guidelines released by the Infectious Diseases Society of America (IDSA).

"There is quite a bit of over-prescribing or inappropriate prescribing of antibiotics for diabetic foot wounds, which doesn't help the patient and can lead to antibiotic resistance," said Warren S. Joseph, DPM, co-author of the guidelines, and consultant for lower extremity infectious diseases at Roxborough Memorial Hospital, Philadelphia. "The guidelines note that when antibiotics are necessary, they should be discontinued when the infection is gone, even if the wound hasn't completely healed," says Joseph.

Pregnancy Can Lead to Overpronation: UK Podiatrist

During pregnancy, pain may often appear elsewhere in the body, says podiatrist Rina Bimbashi. In pregnancy, the impact of the weight gain is compounded by the effect of hormones. "During pregnancy, there is an increase in three hormones — relaxin, oestrogen and progesterone — which soften the muscles and ligaments in preparation for childbirth, making them more flexible," adds Mrs. Bimbashi.

The problem is that if you have mild to moderate degree of overpronation before pregnancy, the looseness of the ligaments means your feet can roll in even more. As a result, you can end up with moderate to severe overpronation (the effects of these hormones are evident in ligaments and muscles for about five months after giving birth). "Pregnancy has a huge impact upon your whole joint alignment, and your muscles and ligaments need to work much harder in order to maintain stability. I commonly see women who have good, normal foot arches which collapse during pregnancy leading to very excessive pronation."

MN, CA Podiatrists Report Increase in Barefoot Running Injuries

Podiatrist Dr. Paul Langer used to see one or two barefoot running injuries a month at his Twin Cities Orthopedics practice in Minneapolis. Now he treats between three and four a week. "Most just jumped in a little too enthusiastically," said Langer, an experienced runner and triathlete who trains in his barefoot running shoes part of the week.

Dr. Bob Baravarian, chief of podiatry at the UCLA Medical Center in Santa Monica, CA said he has seen "a fair number" of heel injuries and stress fractures among first-timers who are not used to the different forces of a forefoot strike. "All of a sudden, the strain going through your foot is multiplied manifold," and problems occur when people don't ease into it, he said.

FL, AZ Podiatrists Advocate a "Measure What You Manage" Revolution in Wound Healing

Robert Snyder, DPM, MSc, President of the Association for the Advancement of Wound Care and David G. Armstrong, DPM, MD, PhD, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA) today led discussion on next generation diagnostics in wound care in Vienna, Austria. "The fact of the matter is that we can't manage what we can't measure," noted Dr. Armstrong. "What is particularly exciting is we are seeing the first-ever point-of-care wound diagnostics to help direct therapy."

Dr. Snyder, who serves as Professor and Director of Clinical Research at Barry University College of Podiatric Medicine, went further "What we're talking about is nothing short of a fundamental change in how we assess wounds in our patients. Our current methods are based on thousands of years of experience by clinicians - but they are subjective. We need to make this objective. That's what's happening." This session preceded the European Wound Management Association (EWMA) symposium, which begins tomorrow.

CMS to Version 5010 Laggards: Get Ready to Borrow Money

Physicians not yet up to speed on new federal standards for electronic claims should be prepared to borrow money to keep their practices afloat come July. That dire counsel comes from the Centers for Medicare and Medicaid Services (CMS). Last week, as it has before, CMS advised physicians who are still trying to comply with the new standards to either take out a line of credit or expand an existing one at a bank because of payment delays that may occur when the agency begins to enforce the standards on July 1. At stake is the ability of physicians to meet payroll and write rent checks.

The new standards are called Health Insurance Portability and Accountability Act Version 5010 standards. The successor to Version 4010 standards, they are designed to streamline electronic claims and other digital transactions between providers and third-party payers, including Medicare. Physicians, insurers, makers of billing software, and claims clearinghouses are all under the gun to convert their systems to the new standards.

Query: Down's Syndrome Complication

I just saw a 5 year old female with Down's syndrome. She has the classic pes planus/HAV of Down's, but ambulates without pain or difficulty. Her mother relates that every night her feet become very flushed and apparently painful. She rubs the feet together until they are chaffed and nearly bleed. They have tried acetaminophen, antihistamines, and OTC topicals like Biofreeze without relief. She wears good supportive sneakers and custom orthotics made by a pediatric pedorthist.

I see no reason for these symptoms from my exam. She has no other contributory medical history. Has anyone else encountered a similar case, and if so, been able to offer some relief?

Query: Certifying Disability

I have a patient who is on permanent (disability) restrictions at his place of employment and limited to restricted sedentary work. This is never going to change in his case. His employer is good about the restrictions, but makes me fill out a new disability form quarterly.  Every two years, I have the patient come in so I can not be accused of any wrongdoing. Do I even legally need to see this patient (the paperwork always asks, which is why I see him)? He needs no current care, and has no problems other than the original one which limits him.

I checked Epocrates, which I use to check interactions on all my patients, and found this interaction listed there. I highly recommend using a system such as Epocrates (which is free) to check for interactions before prescribing any medication. You can never be too cautious.

Ms. McCormick apparently has taken a post provided by Dr. Osterman literally. I believe, after reading the initial post, it was the intent of Dr. Osterman to explain some aspects of predisposing nail disease in "layman's terms" so that the general public could comprehend the concerns and issues he raised. Ms. McCormack is absolutely correct in her citing that nails are "dead and do not breathe." She is also correct in that the nail bed is "living."

I disagree, however, that nail infections cannot occur as the result of multiple applications of nail polish which can potentially mask infections to a non-medically trained person. Increased duration of nail polish on nails can mask...
