SD Podiatrist Provides Limb-Saving Care
 
Dale Pollema, 77, was planning to move some corn from the grain bin — something he has done countless times. But that day was about to become anything but ordinary. When he threw the switch, an auger flew off severely injuring his foot and leg. “I took one look at the injury film, and I was like, Definitely a pretty severe soft-tissue injury. But as long as the Achilles is intact, and his arterial flow is intact, we can definitely work with this,” said Peter Bellezza, DPM.

Dr. Bellezza first did surgery to repair the soft tissue in Dale’s ankle. To give him and his wound a better chance at recovery, he also did a skin graft a week later. “He had a good scan envelope right above the wound, so I was able just to harvest a small amount of skin to kind of get it over that granular bed. We put another wound VAC on, which was on for a full week, and by the time we took it off, the skin had fully grown into the wound,” said Dr. Bellezza. “Oh, he did a super job!” said Pollema.

OH Podiatrist Provides Tips on Avoiding Foot Injuries When Running Long Distances
 
Whether you’re gearing up for a marathon or just want to run longer distances, there are some things you can do to avoid foot injuries. Joy Rowland, DPM, a podiatrist with Cleveland Clinic said starting with the right running shoe is key. “The shoe should be cushioned. It should have a heel flare to absorb shock in the heel area, and it should have some cushion by the metatarsal, or the front part of the foot,” Dr. Rowland said. “You want to make sure that the toe box area is big enough, so your foot has a little bit of room. You don’t want your toes hitting into the shoe.”

Dr. Rowland said finding a couple of good running shoes to rotate between is important. Since all running shoes are different, changing up which ones you wear periodically can break up the repetitive stress running puts on your feet.

Anaïs De Cock, defending champion, again won the women’s ranking during the strides of Nogent-le-Phaye 2023. For the 31-year-old runner, who has more than ten years of running experience, this victory was “not a goal”, but rather for relief after a meniscus injury.

I see it in my office, there are more and more women who come to see me for problems related to running, it’s a sport that is becoming more and more popular among women."

I have been using 1.5 mg sublingual Ativan pre-surgically for patients undergoing bone procedures. I have noticed many patients are still very anxious. Does anyone use an anti-anxiety medication that has consistently positive results in reducing patient anxiety?

In regard to ABPM certification of non-USA trained podiatrists, I appreciate PM News for publishing our concerns as we have little opportunity to let the American podiatric community know of the true picture of practicing overseas. I have been in full time practice overseas since 1995, working in Asia and Australasia. In that time, I have worked with a consortium of DPMs who, like me, struggle with harassment and discrimination from non-U.S. trained podiatrists. 
 
I have shared Dr. Garoufalis' explanation with this group and all see the actions to certify non-DPMs (U.S. trained) as providing an American credential to those with inadequate education and experience. We feel it is discriminatory and will only harm us further. It was an ill-conceived idea seemingly based on limited first-hand experience of the true situation DPMs face overseas. We are in the trenches and ABPM is selling the other side bullets to shoot at us.
 
This ABPM International program should not be supported by the American profession of podiatric medicine and surgery. I think the other APMA-approved Board should also be concerned about this as it too would affect credibility of any American certifying activity. This will be used by bachelor degree podiatrists to claim they are certified in surgery to their local authorities and unwitting public.

I respect Dr. Tomczak’s defense of Dr. Carnett’s expertise, but I wish to correct a couple of misunderstandings. Firstly, the “ABPM International” is a legally-separate entity from ABPM, with a separate BOD, which certifies podiatrists who were educated and trained outside the United States and tests according to that standard. Dr. Carnett was educated and trained in the U.S. and would be subject to a U.S.-based board, especially if he wishes to return to practice in the U.S.
 
There is regrettably no pathway for a podiatrist who completed a one-year training program in the 1980s to now become board certified by either of the CPME-recognized boards. Secondly, in order to be eligible for the ABPM’s CAQ in Podiatric Surgery, a podiatrist must have completed a surgical residency, in addition to other requirements. I’m happy to discuss this further with either Dr. Tomczak or Dr. Carnett.

Dr. Tomczak's  note was very well written about the actual situation that ABPM created. It is quite interesting to see how somebody who would’ve done a one-year surgical residency and left the country for various reasons, can come back and not get certified through another pathway from a board that offers others a certificate of qualification in surgery, but will not enable him to do the same. This is a perfect example of how things are wrong in our profession. I was once certified by ABPM and when they kept on changing their names as they became different boards, they randomly grandfathered certain people who already had certificates versus others.

It was quite surprising to see over the last month or so that five ABPM board members voluntarily resigned with the assignment of new board members, but there was no vote from the people who are certified by this board. Obviously, when five board members resign at the same time, it suggests they were asked to or they would be let go by the president of that board. The fact that our association had a statement out last week stating that it appears that the discussions between the two boards with APMA‘s help has stalled, and may not continue, is extremely worrisome. Well said, Dr. Tomczak. It explains how ABPM is really not legitimate in trying to be fair.
