According to Robert Weil, DPM, "The link between drugs and sports runs deep! Drugs and sports have been connected for about as long as sports have been played. This includes the whole array of painkillers and anti-inflammatory drugs, both prescribed by doctors and available over the counter."

"One of the concerns I’ve always had, especially at the elite and professional level, is that the physical and mental demands of sports all too often are extreme. This often leads to the use of pain meds and anti-inflammatories, even in younger athletes, to survive. This “routine use” is alarming! The acetaminophens and ibuprofens are too often overused to keep these young athletes “in the game.”

NJ Circuit Court Won't Revive Blind NJ Podiatrist's Bias Claims
The Third Circuit won't give a blind podiatrist another shot at his claims that he was the victim of discrimination after he was removed from a list of referring physicians participating in New Jersey's medical marijuana program, the federal appeals court said Wednesday. In a brief per curiam opinion, a three-judge panel noted that "by his own admission," plaintiff Dr. Darren James, who claims that he was unfairly booted from the program in 2017, is "a podiatrist —a DPM — and not a medical doctor (MD)."
"The fact that he previously was able to register online for the MMP as a 'medical doctor', even though he was not one, did not somehow confer qualified status under the program," the circuit court told James, who is by his own description "totally blind" and brought claims under the Americans With Disabilities Act, the Rehabilitation Act, and the New Jersey Law Against Discrimination, among other statutes.

Digital Front Doors Improve Patient Health, Remove Barriers to Physicians

For my colleagues interested in treating hyperuricemia/gout, I highly recommend the book Drop Acid by David Perlmutter, MD. It is very enlightening how high uric acid levels contribute to a multitude of chronic health problems. Within just the last couple of years I have had patients with chronic gout left untreated by their primary care doctors. Most recently, a 60 year old obese male consulted me because his physician told him the painful red bump on his foot was just a bunion and he had bursitis on his elbow.

The physician was very embarrassed when I shared the lab results showing a uric acid of over 14! This patient has suffered for over a year. Initial therapy failed using urolytic agents and probenecid (which I knew would fail, but we had to do to please the bean counters). After hours of paperwork, and run around with BCBS, he will be starting Krystexa infusion soon. Previous patients have benefited immensely from this biologic enzyme that degrades uric acid.

The treatment of hyperuricemia is not within the province of a podiatrist, (or, in fact, most medical specialties), anymore than the discovery of hyperglycemia should result in the treatment of diabetes. The issue is twofold. Firstly, hyperuricemia may result from any number of etiologies, such as the effects of medications, occult myeloproliferative disease, CKD, etc. The etiology of the elevated uric acid is the major issue, not the hyperuricemia per se. In addition, the presence of hyperuricemia in and of itself does not necessarily require treatment. Secondly, drugs such as allopurinol and its alternatives require monitoring for adverse sequelae such as renal dysfunction.
The diagnosis of various acute onset monarticular arthritic disorders (e.g.: psoriatic arthritis, septic arthritis, CJD) and initial treatment is an important part of podiatric practice. Education of patients regarding gout and hyperuricemia is an important contribution we can make to patient care. However, treatment of hyperuricemia is, in my opinion, not in the spectrum of what we are trained to do, and there are others better positioned to do so.

There are a variety of online payroll solutions that will create 1099s for a relatively low cost. Our office uses Patriot Software to create the 1099. They will file the 1099 electronically for a nominal fee. This is a great solution to a very irritating issue.

Does anybody really believe that topicals for onychomycosis make one bit of difference. Once the nail has thickened, discolored, and has texture changes, there is not a topical, prescription, or OTC that will do any good.

Dr. Udell asks the question "which products are most effective, the OTC products or the ones that require a prescription?". My answer is unequivocally, neither of them, and I am sure there are many of you who disagree. That's fine. After 32 years of practice, and with the exception of those few patients who present with superficial white onychomycosis, I never see topicals being an effective treatment for the types of severe fungal nails that we see on a daily basis. Certainly not infections that permeate the nail plate, and go deep into the nail bed.
My treatment of choice for almost 100% of patients who want to get rid of their fungal toenails is...
