Advice on Getting Feet Ready for Summer: CA DPM
Whether you're a dude anxious to slip into Reefs or a lady shopping for this season's peep-toe wedge, a few easy steps can prepare your feet for sunshine - and public viewing. "A good foot inventory is the best thing to do after a winter that's been wet and cold," says TIMOTHY SHEA, a California podiatrist.
The first thing to check for is a possible fungus infection. If you have excessive moisture on the bottom of the foot, it could be a sign, says Shea. Check your nails, too. "If they are thicker or turning yellow or discolored, fungus is a possibility," he says. Prevent it by washing and drying your feet every day (note, shower water doesn't count). Use soap and water and a washcloth to get between your toes and dry them with a towel.
Sweaty feet? Shea suggests pouring rubbing alcohol over the feet once a week to disinfect them. If the fungal problem persists, see a podiatrist about over-the-counter products such as Lamisil. Dry Skin? Winter breeds dry, cracked skin, particularly heels. Not exactly sexy. Once they're clean and moist, use a pumice stone or paddle to slough off excess skin that collects, due to friction when your heel hits the ground. "After about a week, the good skin will start to surface," Shea says. It's important to moisturize on a regular basis, and not with popular body lotion. "They dry out the skin," Shea says. Scaly, dry heels should be treated with creams that contain lanolin, such as Eucerin, Nivea, and Kerasal.

70% of Docs More Worried About Drug Safety: Survey
Seven in ten physicians are more worried about the safety of drugs they prescribe after high-profile scares involving several medicines such as the withdrawal of arthritis pill Vioxx, according to a new survey. Sixty-eight percent of physicians would prefer prescribing a drug that has been on the market at least 10 years, even if costs among the possible medication options were equal. One-third feel that new or recently approved prescription drugs are less safe than drugs on the market for a decade or more, according to the survey.
Medco Health Solutions, a large U.S. pharmacy benefits manager, released the study, which was conducted by Forrester Research.

RE: Northwest Podiatric Convention Combined Symposium

I am writing from the Northwest Podiatric Foundation's Second Annual Combined Symposium on Dermatology, Dermatopathology, and Plastic Surgery. This event is shaping into a key meeting, with excellent speakers and a very enthusiastic audience. During today's didactic session, our profession was honored by having Clay Cockerell, MD, an internationally prominent dermatologist and dermatopathologist, and the current President of the American Academy of Dermatology give a major talk on how podiatrists and dermatolgists must work together for optimum podiatric dermatology care. He underscored the importance of accurate diagnosis and the use of the biopsy as a diagnostic tool. Saturday's session included a live workshop on biopsy techniques.

RE: Medicare Part D Problems From: Stephen J Bennett, DPM
My newest difficulty with the constant changes in the practice of medicine comes about from the new Medicare Part D program. Here's a real life example.
Mrs. Jones, a long-time patient, was seen on Monday and was given two prescriptions for common medicines, i.e., Celebrex and Lotrisone. On Tuesday she calls the office, upset because she couldn't get her prescription filled as the pharmacist said that these medicines require prior approval or aren't on the Medicare Part D insurers (i.e.. silver scripts) formulary. She asks that I call the insurer and get approval for the medicines.
So I make the call and after 20+ minutes and three transfer one medicine is approved and the other medicine needs to be appealed by writing a letter of medical necessity. I ask the insurer if I could get a copy of their formulary so that I could try to tailor my prescribing accordingly and they tell me that is not their policy. I ask if I could get some type of reimbursement for my time in getting a prior approval and my efforts to appeal the other prescription. Of course, they had empathy and asked if I would like to be transferred to a supervisor in another department, but no, there was no payment for my efforts.
This example is becoming more common. More administrative time in My office, more red tape to weave through, no additional compensation. Shouldn't this exercise be paid for by the prescription plan, or since there is a financial relationship between the pharmacist and the insurer, shouldn't they be taking care of securing authorization?
I think this my be a excellent opportunity to join up with other medical professionals (AMA) on an issue that should be more important to them than it is to us. We should offer our assistance and resources and thereby cementing a bond and enhancing our relationship.
