MA Podiatrist Discusses Foot Types
Louis J. DeCaro, DPM, President of the American College of Foot & Ankle Pediatrics, appeared on NBC’s Mass Appeal to teach foot typing and foot type the news crew. Dr. DeCaro talked about how important it is to know which particular foot type you are in order to prevent many orthopedic issues that can stem from it. "Foot types are congenital. Your foot type can predict so much about not only your orthopedic future, but also what exercise or sports you may or may not be inclined to play or participate in. This is exactly why it is so important to have kids screened at an early age.”

According to Dr. DeCaro, “There are 24 distinct patented foot types which can be easily placed into 6 major categories. And there are no good foot types. Feet were made to move a lot (pronate) on uneven surfaces, but we are forced to walk and do activities on hard surfaces in shoes, and that’s where so many problems arise.”

IL Surgeons Volunteer to Treat Ukrainian Victims
Sev Hrywnak DPM, MD, owner of Advanced Surgical Center in Chicago, is of Ukrainian descent and asked Illinois podiatric surgeons if they would volunteer in treating trauma victims coming from the Ukraine. “It was very heartwarming to get such a response. Within 48 hours after a request was put out by the Illinois Podiatric Medical Association, over 60 podiatric surgeons responded,” stated Hrywnak.

TX Podiatrist Sworn in as Newest Ft. Worth School Trustee
Podiatrist Camille Rodriguez, DPM said she wouldn't have gotten her doctorate without her family and the Fort Worth teachers and staff who supported her when she was in school. Now that she's the district's newest trustee, her goal is simple. "I truly want every Fort Worth ISD student to have the same opportunities that I did," she said.

The school board swore in Rodriguez to represent District 1 on Tuesday during a special board meeting. She replaces Jacinto Ramos Jr., who announced his resignation in March. Dr. Rodriguez, a former substitute teacher who served on the board from 2004 to 2008, was surrounded by family and friends as Justice Maryellen Hicks swore her in.

Digital Front Doors Improve Patient Health, Remove Barriers to Physicians
Regardless of whether a patient's needs are considered essential or non-essential, access to health information—timely and accurate—is something everyone seeking medical care should have access to.
1. Google it - It's not uncommon for the Internet to be a source patients use to try to figure out what their symptoms might mean. That can be a slippery slope and lead to rabbit holes of unaccredited “licensed” experts, incomplete information, and chatroom diagnoses.
The solution, hands down, is giving patients access to a digital front door. It eliminates hurdles to medical care by streamlining a typically complex process into a patient-centered and accommodating online experience. This innovative technology gives patients the convenience and accessibility patients need, without the long wait times and hurdles that can come with trying to get in touch with healthcare providers.

Query: Displaced Fracture
We had a patient last week who was diagnosed with a non-displaced, right fibula fracture S8264xD at an emergency room. She followed up with us and at this subsequent visit, the fibula fracture has now displaced as a result of walking on her right ankle. How do you code this new displaced fracture?

Response: A patient was originally seen in an emergency room with a non-displaced fracture of the right fibula. Apparently it was coded as S82.64XD which is defined as: Non-displaced fracture of lateral malleolus of right fibula, subsequent encounter for fracture with routine healing. If this was the first time that this patient was seen in the emergency room for this specific injury, it would have been more appropriate to code the encounter using S82.64XA which is defined as: Non-displaced fracture of lateral malleolus of right fibula, subsequent encounter for fracture initial encounter and treatment for a closed fracture.
The patient was followed up at the office. The fracture is now displaced as a result of the patient ambulating on the affected, right ankle. The most appropriate ICD-10-CM code to use to identify this fracture would be S82.61XA which is defined as: Displaced fracture of lateral malleolus of right fibula, initial encounter and treatment for a closed fracture. The reason that this is the appropriate ICD-10-CM code to use is based upon the fact that this is the scenario that the patient presented with when she presented for the initial encounter at the office following the ER visit.

We often see patients with an acute gout attack, often with a history of hyperuricemia which unfortunately was never managed. For simple cases, I try to manage the uric acid, but if the patient has other medical considerations, I refer the patient to a local rheumatologist for long-term management. Too often the PCP is unwilling or unable to manage the hyperuricemia.

In answer to your query, Dr. Klirsfeld, there is increasing evidence that hyperuricemia plays a significant deleterious role in patients with hypertension, diabetes, fatty liver, and obesity (i.e., metabolic syndrome). The days of seeing a patient with gout, prescribing a urate-lowering drug and sending them on their way are waning. Uric acid is not considered an innocent bystander any longer. So, yes, we should be recommending follow-up with our patient's PCP and coordinate care with him/her. We should also counsel our patients on the importance of maintaining a normal uric acid level.

My accountant started doing this to all deadbeat patients who received money from insurance companies and failed to send it to us. He was hesitant to do this to people who did not receive money, but when it showed it went to a patients' deductibles, all got a 1099. Many sent money so they could write it off, but I got paid.

RE: OTC Topical Nail Antifungals vs. Prescription Products

Both in drug stores as well as in our own offices, there is the selling and dispensing of over-the-counter topical nail antifungals. Some of these products sold at pharmacies are even advertised on television.
"Big Pharma" is also promoting products to treat onychomycosis. Jublia is advertised on television and marketed to doctors of multiple specialties. Based on this, what have most practitioners found in terms of efficacy and practice management issues as to which products are most effective, the OTC products or the ones that require a prescription?
