AT THE COLLEGES

Graduation is Bitterweet Moment for WesternU Graduate

Western University of Health Sciences held the first of five commencement ceremonies on Wednesday, May 18, 2016 at the Pasadena Civic Auditorium in Pasadena, California. The College of Allied Health Professions and the College of Podiatric Medicine conferred degrees on 209 graduates. Graduates felt a mix of emotions as they prepared to walk the stage and receive their diplomas. "It's a little bittersweet, but mostly I'm pretty excited," said College of Podiatric Medicine graduate Mark Thompson, DPM '16. "It's fun to see all my classmates again who I haven't seen in a while and celebrate."

Dr. Mark Thompson

Thompson will enter a residency program at Heritage Valley Hospital in Beaver, Pennsylvania. He wanted to enter podiatric medicine because he was interested in surgery and he likes the work-life balance the profession offers. "I am ready to move on to the next step," Thompson said. "It's surreal. Something I've been looking forward to for four years is finally here. It has been a good journey."

RELEVANT RESEARCH
Prospective Comparison of Running Injuries Between Shod and Barefoot Runners

A prospective survey was conducted over the course of a year among 201 (107 barefoot and 94 shod) adult runners. Information regarding injuries and mileage was logged monthly using a custom, web-based database program. The number of injured runners, number of injuries per runner, and injury rates were compared between habitual barefoot and habitual shod runners. Both musculoskeletal and plantar surface injuries were assessed.

Conclusions: Barefoot running is associated with fewer overall musculoskeletal injuries/runner, but similar injury rates. A larger scale cohort is needed to more accurately assess differences in individual injuries between these two groups.

PHARMACEUTICAL NEWS
FDA Issues Warning On Use of Ketoconazole for Skin and Nail Infections

FDA is warning healthcare professionals to avoid prescribing the antifungal medicine ketoconazole oral tablets to treat skin and nail fungal infections. Use of this medication carries the risk of serious liver damage, adrenal gland problems, and harmful interactions with other medicines that outweigh its benefit in treating these conditions, which are not approved uses of the drug.

FDA approved label changes for oral ketoconazole tablets in 2013 reflecting these serious risks and to remove the indications for treatment of skin and nail fungal infections. However, an FDA safety review found that oral ketoconazole continues to be prescribed for these types of conditions. Since the 2013 labeling change, one patient death has been reported to the FDA due to liver failure associated with oral ketoconazole prescribed to treat a fungal infection of the nails. Healthcare professionals should use ketoconazole tablets only to treat serious fungal infections when no other antifungal therapies are available.

QUERIES (NON-CLINICAL)
Query: Densitometer Recommendations

Are there any recommendations for the type or model of densitometer used for an analog x-ray processor?

Darryl Burns, DPM, Monterey, CA

QUERIES (MEDICAL/LEGAL )

Query: Billing Insurance Company for Patient Records

If an insurance company wants patient notes not as an audit but prior to payment of a claim, can we bill the insurance company for a reasonable fee? If it's for an audit, can we bill the insurance company that wants all the notes?

Judith Rubin, DPM, Cypress, TX

Editor's comment: PM News does not provide legal advice. The definitive answer is found in the contract between you and the insurance company. While state laws allow for billing patients, hospitals, attorneys, etc. for copies of patient records, most insurance companies require that you supply them with patient records, if requested, as a condition for participation.

CODINGLINE CORNER
Query: ICD-10 Trauma Case

I have a 21 year old patient who had a farm implement wheel fall horizontally on the right foot causing a traumatic contusion to the great toe including nail plate disturbance with a displaced fracture of the distal tuft of the distal phalanx. There was the presence of a subungual and digital hematoma with pain in the great toe. What would the ICD-10 codes be for the above?

Joseph Borreggine, DPM, Charleston, IL

Response: I recommend M79.674 (pain in right toe), S90.211A (contusion of right great toe with damage to nail; initial encounter), S92.421A (displaced fracture of distal phalanx of right great toe; initial encounter), and last, but not least, W30.81A (contact with agricultural transport vehicle in stationary use; initial encounter)
