MI Podiatrist to Ride 600 miles for Juvenile Diabetes Research
 
A Michigan podiatrist will be celebrating the big 6-0 with a bike ride at the end of May. Scott Grodman, DPM from West Bloomfield plans to ride 600 miles over six days to help raise awareness and support for juvenile diabetes research. The podiatrist’s goal is to raise $60,000 for the ride.

The Metro Detroiter is no stranger to extreme bike rides. For his 50th birthday, he rode 300 miles to raise the same awareness. The cyclist raised over $50,000 for his ride to Chicago. Dr. Grodman and his wife both have sons that were diagnosed with juvenile diabetes. In 1999 the family established The Grodman Cure Foundation. So far, the foundation has raised nearly $2,000,000 for diabetes research.

Every good clinic consult room is outfitted with desktop computers and a large monitor. When a patient is engaging over virtual care, there is a USB-connected webcam with integrated microphone mounted on the top of the monitor for the clinician to use for high quality audio video connection. This allows the clinician to maintain virtual eye contact while still having access to the record in support of the patient visit.

RE: Patients Who Refuse To Pay Their Bills

When I was still practicing law, there was a way that some lawyers would use to encourage a client to pay a bill that was due. They would send them an IRS form 1099, stating that they, the client, received whatever amount of money that was owed as income.
 
The legal theory is that the client was unjustly enriched by that amount that they did not pay, even though they owed it. Of course, a 1099 would be sent to the IRS also. Before you do this to a patient, consult a tax attorney as to how this might still work today.

RE: Availability of Local Anesthetics

It seems that every year there is some shortage of a supply like Celestone, local anesthetics with epinephrine, or the pandemic caused shortage of isopropyl alcohol. But the very long absence of lidocaine and bupivacaine is getting serious. My suppliers say that there is no indication from the manufacturers as to when there will be a resumption of supply. Does that mean we have to wait to the end of the year?  They have no answer.  How can we perform injections or local anesthesia when necessary without them? 
 
Are there alternatives, like carbocaine, that dentists use, that work in the extremities? Can 30ml 'single dose' vials of anesthetic be used as multiple dose vials?  I have purchased a .75% (higher than the .25% or .5% we are familiar with, although they too are available) 30ml 'single dose' vial of bupivacaine.  It "looks" almost the same, but looks can be deceiving. I draw 2ml of bupivacaine with a sterile loading needle and then use another loading needle to draw the steroid into the syringe. 1.7ml ampules of various anesthetics are available but the cost can be quite large---but versus none at all---they become acceptable.
 
I am searching for some knowledge or input on this issue beyond my own musings.

There are some very good reasons why we podiatrists should have a lower incidence of COVID than the general population. Doctors, in general, are among the most vaccinated population. We podiatrists are so well educated in infectious disease and the precautions that we need to take to keep our staff and patients safe. We spend so much time in operating suites where the principles of antisepsis have been followed long before COVID. 
 
I don’t find it surprising that our incidence of COVID as a group is lower than average, but rather, that it is a testament to the policies that were put into place and the practices that were diligently followed in our offices and clinics, as well as in our personal lives.

Dr. DiResta is correct in noting that two surveys taken on how many people have contracted COVID-19 may be totally inconsistent with each other. Expect more of this. When COVID-19 first became a pandemic, the strain caused severe morbidity and mortality. The only way of knowing if a person had the disease was to go to a public testing site, hospital, or urgent care facility. The results were documented and made public. Today, things have changed.
 
The strain today is more contagious but less virulent. Many are also vaccinated. People are, thankfully, not dying or winding up on ventilators as frequently. The symptoms for many people may mimic a mild cold. There is also an ample supply of home testing, the results of which, remain private and undocumented. Hence, today, we can only get rough estimates from government agencies and other surveys and the results from multiple studies may indeed vary.
