Cultural Competence Included in Training at NYCPM
 
A first for NYCPM occurred on March 10th, when “Cultural Diversity and Racial Bias in the Medical Sector” was presented virtually to its second-year class; this was the first time that cultural competence – the ability to understand, communicate with and effectively interact with people across cultures – was integrated into their Physical Assessment Course, abbreviated as PASS.

Ryan Crews, PhD has been promoted to Associate Professor at the Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science (SCPM).

Dr. Crews received a Master in Science degree in kinesiology at the University of Massachusetts in Amhearst and a doctor of science degree in life sciences from the Manchester Metropolitan University. He has been an assistant professor at SCPM since 2014.

FL Podiatrist Discusses Ingrown Nails in Dancers
 
Ingrown toenails are not unique to dancers. But dancing definitely makes them more likely. Commonly caused by improper nail cutting, ingrown nails can also be the result of repetitive microtrauma damaging the nail plate. "An ingrown toenail is basically a part of the nail plate that grows into the skin," says Alyson Slater, DPM, a podiatrist at Palm Beach Foot & Ankle and a former dancer. "And it's exquisitely painful."

Do not practice what Dr. Slater calls "bathroom surgery" by attempting to cut the ingrown yourself. Using dull, non-sterile tools and not knowing how far to cut are more likely to result in pain, irregular nail growth, and even infections. Do not dance through infections, no matter how high your pain tolerance. "If that infection festers, it can become very severe," says Slater, "causing a soft-tissue infection or even a bone infection." See a healthcare provider if the skin around an ingrown nail has redness, swelling, or fluid build-up, or if the nail itself is unusually thick and yellowed.

Query: Office Visit and Injections
 
I’m having trouble when billing an office visit along with an injection. The primary diagnoses are different and I’m also adding a -25 modifier to the office visit. The denials say that the “benefit for this service is included in the payment/allowance for another service.” Some also mention that it is part of the global, but we haven’t billed anything previously that would put the patient in a global period unless some injections are considered that.

Response: There are several possibilities without seeing the actual claim and EOMB. Are there other doctors in your group who could have performed a procedure with a global period? Global periods are tied to the Tax ID/group NPI.
 
Which injection code did you use and for what diagnosis? Certain injection codes are generally denied and difficult to be paid such as CPT 64450. Lastly, some carriers have almost routinely denied claims with the -25 modifier due to presumed abuse of the modifier. You may need to submit your chart note along with your appeal.
