APMA Lauds House Passage of VA MISSION Act
The American Podiatric Medical Association (APMA) applauded the approval of critically important legislation to support America's veterans. S 2372, the John S. McCain III, Daniel K. Akaka, and Samuel R. Johnson VA Maintaining Internal Systems and Strengthening Integrated Outside Networks Act of 2018, or VA MISSION Act, passed the House of Representatives May 16 and will head to the Senate for consideration next week.

IL Podiatrist Speaks to Ministry of Health in Poland
Message: FIP President and APMA past president Matthew Garoufalis, DPM was invited by the Ministry of Health in Poland to make a presentation about the expansion of podiatry services in Poland. The presentation took place in the restored Royal Palace in Warsaw where the Ministry of Health is now housed. The "Gilded Room" was used as a retreat and meeting room of past monarchs.

The meeting was attended by members of the Polish Podiatry Association; a large contingent form the Polish Ministry of Health; Christian Jerome, Vice President of FIP; and Caroline Teugels, the Executive Director of FIP. At the conclusion of the meeting, the Ministry of Health stated they now have a perfect blueprint and pathway for the expansion of podiatry into Poland.

MO Podiatry Group Settles Medicare Fraud Charges Over Toenail Clippings
Foot Healers podiatry clinics in St. Louis settled with the federal government for $125,000 for Medicare fraud on false claims from 2010 to 2016. The clinics knowingly billed Medicare for medically necessary toenail removals, when the services provided were routine nail clippings that are not covered by the government insurance program for people older than 65 and others with disabilities, according to the U.S. Department of Justice.
“After becoming aware almost five years ago of some billing errors, we successfully worked with the government to correct this. At all times, we have been and remain in good standing with Medicare. We appreciate that the government worked constructively and cooperatively with us to resolve this matter,” John Murray, president of Foot Healers, said in a statement. Under the settlement, Foot Healers will repay the government $125,000 for the false claims. The company also signed a three-year agreement with the government for extra oversight in its compliance with Medicare regulations.

7 Financial Reports Your Practice Needs to Run - Part 3
Most physicians readily admit that classes on how to run a successful medical practice were not part of the curriculum in medical school. While "financial reporting," might not have been a class at your medical school, experts say you need to at least become familiar with understanding some of the basic reports. Here are seven to track.
3. Balance Sheet - "A balance sheet shows what your practice owns and what it owes. There are items that show up in a balance sheet that might not show on a P&L report. For example, a balance sheet will show you where the practice needs to invest in terms of capital improvements. This appears to be Radiasse as the filler in question. It has calcium hydroxyapatite and is the only filler that shows up on x-ray. I have not used a filler in this location. It appears that there is a lot of filler in a small space and that may have caused the complication. For a fifth toe corn, I suspect you would use a very small amount of filler and it appears that too much filler was used. I suspect you can use too much and that may affect the vessels to the toe. We generally use fillers on the plantar aspect of the feet under the metatarsal heads.

This is lollipop toe syndrome (LTS) caused by 1. too much Restylane injected into the toe (way too much in this case) and 2. not padding the injected site with an aperture pad post-injection. All of the Restylane migrated to the tip of the toe instead of staying over the lateral head of the proximal phalanx. It may take 6-12 months to absorb.

Unfortunately, this podiatrist signed a contract that did not protect him. The billing company that I have been using for years has a contract that states that they have a contractual obligation to submit all claims AND follow-up on all ARs, and they even pay for the EHR and practice management software. It's hard to prove lost income, but the podiatrist should have a lawyer send a letter demanding to know why claims were not paid. If the billing company doesn't respond or can't explain why, the podiatrist would probably have a good case for negligence.

Did you ever hear the expression, "Tell it to the judge"? Ultimately, the doctor signs the forms and is responsible for all the charges. It does not really matter that you are paying more or it is the best billing/coding people in the world.

The answers to Dr. Borreggine's questions are long and complicated ones dating back approximately 60 years. Minimally invasive surgery is now the standard of care in many countries throughout the world. However, I can say that resurgence of MIS in the United States can be traced, in no small part, to the exceptional outcomes our surgeons are seeing. This includes less patient downtime and the maximum use of the surgeon’s time and the ability to perform many of our procedures in an office setting or an outpatient ambulatory surgical center.
I believe that the future of MIS lies in the ability to teach the procedures in the schools and the residency programs. I am happy to note that the next Academy Of Minimally Invasive Foot and Ankle Surgery LSU lecture cadaver seminar to be held in New Orleans on May 31, June 1, and June 2 will include several residents from programs throughout the United States.

Having witnessed the origins of MIS and the political squabbles within the profession, I now have a broader perspective on it. Yes, there were two schools back then. There were the "open" surgeons and there were those who took the late Dr. Ed Probber’s one-week course on how to do MIS (in the back of his Long Island office). The training was often supplemented by learning at other doctors’ offices and at MIS conventions.
There were conflicts and a lot of name calling. Some of the criticisms of MIS were valid and some were purely political. At that time, podiatry was striving to be part of the medical/surgical establishment and MD surgeons did not know from MIS. Today, things are different. MD surgeons are gravitating toward minimal incisional techniques. They do spine surgery, knee operations, gall bladder removals, and hernia repairs using very small incisions. The healing time is reduced. Hence, there should no longer be a need for two schools of thought in 2018. It’s time for foot surgeons to learn open and minimal incisional techniques and choose the best one for each and every patient who needs foot surgery. Let’s leave the politics of it back in the 1970s.

I read with great interest that the expanded podiatric scope bill passed in South Carolina. Congrats to all those involved in helping with the expansion. I am curious why many of these recent expansions completely leave out the ABPM board and what if anything ABPM is doing to rectify the situation. I am also curious if there is an opportunity for advancement in the scope for all those members of the profession who graduated prior to the implementation of a 3-year RRA program.
There are many conditions such as skin lesions, wounds, etc. that should not require three-year training in an RRA program and passage of ABPS testing in forefoot, rearfoot/RRA to treat these aforementioned conditions. One should not have to master a compound ankle fracture in order to prescribe a steroid cream for a dermatitis on the leg. If those with the ABPM credentials can't increase their scope like their ABPS brethren, what good is that certification?
