CA Podiatrist Discusses Athlete's Foot

Athletes' foot, that stubborn, itchy fungal infection is most often seen between the toes and is contagious. The infection can result in cracking and peeling skin that may burn. The fungus thrives in damp socks and tight shoes and spreads in warm, moist places, such as public showers and gyms.

Use over-the-counter antifungal treatments daily until the fungus clears, plus another two weeks. Using a three-prong OTC treatment approach works best, said Dr. Alan Jones, podiatrist at Sharp Rees-Stealy Otay Ranch. He recommends using antifungal lotion on the affected skin, liquid medication on the toe nails, and medicated powder in the shoes. If you have foot fungus that doesn’t improve or worsens after self-treatment, see a podiatrist who may culture the skin to determine if symptoms are caused by athlete’s foot or another skin disorder.

CA Podiatrist Receives Highest Recognition from Scholl College

Approximately 170 alumni, faculty, students, and friends attended the 29th Annual Dr. William M. Scholl College of Podiatric Medicine Scholarship Benefit and Centennial Reception at the Fairmont Hotel in Chicago. Amol Saxena, DPM '88, received both the Scholl College Honor Medallion and the Alumnus of the Year Award. The Alumni Association, Bako Podiatric Pathology Services, Dr. Comfort, PICA, the Midwest Podiatry Conference, the Illinois Podiatric Medical Association and Pedinol Pharmacal, Inc. were recognized as scholarship sponsors.

Six second-year students were selected as Alumni Association scholarship recipients: Rachel Baisch (Alumni Association Scholar), Yashraj Chauhan (PICA Scholar), Amanda Harwood (Pedinol Pharmacal, Inc. Scholar), Jessica Nowak (Midwest Podiatry Conference/Illinois Podiatric Medical Association Scholar), Amy Theppote (Bako Podiatric Pathology Services Scholar), and Morgan Zellers (Dr. Comfort Scholar). The Scholl College Alumni Association also announced that it would award an additional $40,000 in scholarships to third and fourth year podiatry students. The Association will also sponsor a design competition for an alumni application for iPad and Android phones to be used on clinical rotations.

CA Podiatrist to Conduct 28th Annual Shoe Drive for the Homeless

Since 1984 Dr. Marc Spitz of the Foot Pain Center has been conducting a Shoe Drive to distribute thousands of shoes to local homeless shelters in Long Beach and Orange County. The 28th annual Shoe Drive takes place June 11-15, 2012. Dr. Spitz started the Shoe Drive program after hearing from his patients – especially from women – they often had a closet full of shoes in excellent condition that were no longer being worn.

"I thought that these shoes could be put to good use at homeless shelters. Many residents of these shelters suffer from a wide variety of foot problems from wearing improper shoes – conditions like bunions, corns, calluses and heel pain. Some have diabetes and peripheral neuropathy which is particularly dangerous if the wrong shoe style or size is worn," says Dr. Spitz.

WA Podiatrists Speak at Spanish Foot Seminar

Drs. Mary Crawford and Dock Dockery, representing the International Foot & Ankle Foundation, were featured speakers at the XI Seminario Internacional De Cirugia sponsored by the Spanish Association of Surgical Podiatrists at the Hotel Wellington in Madrid, Spain, on May 11-12, 2012.

Medical Boards' Actions Against Docs up 6.8% in 2011

Driven in part by the Florida Board of Medicine, state medical boards' disciplinary actions against physicians increased 6.8% in 2011, and 4.9% more doctors had their medical licenses or license privileges revoked or suspended, according to the Federation of State Medical Boards Summary of 2011 Board Actions.

The national increase in total serious actions could be traced in part to the Florida Board of Medicine, whose total disciplinary actions increased 54.4% to 332 from 215. This figure included a 51.6% increase in revocation of medical licenses to 144 from 95.

Query: Talar Osteochondral Lesion Decompression

How should I code decompression of talar osteochondral lesion with percutaneous drilling?

Response: If you are doing it arthroscopically, then you would bill CPT 29891 (arthroscopy, ankle, surgical, excision of osteochondral defect of talus and/or tibia, including drilling of the defect).

If you are just drilling without performing arthroscopy, then I would think you would use CPT 28100 (excision or curettage of bone cyst or benign tumor, talus or calcaneus), or the unlisted code, CPT 28899.

Personally, I have never done the procedure without performing arthroscopy and drilling under direct visualization, unless you were to do retrograde drilling - but in that case, you would still perform an arthroscopic synovectomy and/or debridement of the joint.

I felt the need to respond to the last paragraph in Dr. Markinson’s last post regarding a possible malignant soft tissue tumor. Some people who read the statement: “Lastly, if you find yourself seeking advice on the surgical management of a soft tissue mass (that may be malignant) in PM News, I am reasonably sure you may get some great information. But I am positive that the patient could be in better hands.” may feel offended, intimidated, and hesitant to post here. I hope that does not happen.

This forum should be for all of us to share and we should not hesitate to share our interesting cases. I look forward to it every day. We all realize that this is just a forum and not a text book or “the written word”—Everything read here regarding patient care should be taken as “an opinion” not a hard fact - even Dr. Markinson’s posts.  As my mama used to say, “There are lots of ways to skin a cat,” and in most cases there are lots of ways to approach the problems we see. I thank Dr. Markinson and all the others who post here and would like to see more cases. As Dr. Markinson says—there is some great info here. There are also some differing opinions - as they say - that is what makes the world go round.

Dr. Lagman states in his recent post, "I am a little disturbed though when advice is given stating that if you have to ask the question on PM News, then you probably aren’t the best person to handle the patient and refer him to someone else. I feel that this is a terrible answer. Should this be our standard answer now when a clinical question is asked on this forum?"

In fact, when I posted that "advice" I wasn't thinking of Dr. Lagman. It had already been a few days and the discussion that ensued was full of very problematic statements. In fact, some of the answers shocked me more than the original query. But since he asks the question, I would like to answer. Questions asked in this forum are a great source of debate and information gathering.

However, there is a big difference between what Dr. Lagman originally asked as opposed to...

I signed up with ZocDoc a few months ago. I had a horrible experience and got no patients from them. They have great telemarketing people who call you and act like they care about you. The only reason I signed up was that some big people were behind it like the founder of Amazon. I wish I had not signed up and am currently trying to get my money back.

ZocDoc is NOT worth the money. Yes, it seems convenient and they promise to send you many patients. However, they get you to add their link on your website and when the patient makes the appointment, you have no way of knowing if they did so from a ZocDoc referral or your own efforts (Google, etc.). Plus, many times you're just scheduling already existing patients, rather than new ones.

RE: Sterilizing Bits Between Debridements

I agree with Dr. Kittay. There are no studies to support the board and they should have at most given him a warning letter. I suggest that anybody who has a similar problem consult their malpractice carrier, which usually covers board complaints, and fight back vigorously. Chances are that, with a competent attorney, the board will back off or reduce the punishment to a letter. In my opinion, the boards in many aspects are over-reaching and playing judge and jury, and that is wrong.

When Dr. Joseph speaks, I listen. And on this particular topic, I am in complete agreement and have been so for well over a decade. If there are any arguable points, and it still comes down to the jury being out, WHAT WOULD YOU WANT DONE IF IT WERE YOU OR YOUR LOVED ONE? This makes the decision pretty clear for most of us, I'd say. Properly cleaning and sterilizing the instruments we use is not at all difficult, and why should it not be an expected standard of care?
