Orthotics Overprescribed in the General Population: NY Podiatrist

Although many podiatrists routinely recommend orthotics, their increasing use is controversial—especially since the cost (usually hundreds of dollars) is rarely covered by insurance. Robert Eckles, DPM, of the New York College of Podiatric Medicine, thinks orthotics are overprescribed for the general population. The appropriate medical strategy depends on the primary cause, he says: If malfunctioning feet are caused by overtraining or overweight, orthotics aren't going to fix that.

Some practitioners also maintain that cheaper therapies—icing, stretching, less specialized shoe inserts—are as effective as prescription orthotics. Studies by Irene Davis, professor of physical therapy at the University of Delaware, found that "semi-custom" models (the lab uses a mold of the patient's feet to identify the best-fitting prefabricated insole) work as well as the totally customized, built-from-scratch kind.

OH Podiatrist Named Co-Director of New Wound Care Center

Ian Nunnally, MD and Brock Liden, DPM have been named co-medical directors of Berger Hospitals Wound Healing Center with Hyperbaric Oxygen Therapy. Nunnally and Liden will be responsible for reviewing patient care and results, evaluating new clinical products and providing oversight and guidance on policies and procedures.

Dr. Liden is an Ohio licensed podiatrist treating ankle and foot pain. Liden received his degree from the Ohio College of Podiatric Medicine. He served his residency at the Veterans Medical Center in Cleveland, Ohio.

Query: Reverse Weil Osteotomy

I have a patient with no fourth met head and severe pain at the 3rd and 2nd met head regions. She is too young for a Hoffman procedure and she has not responded to off-loading of the site.  Does anyone have any experience with a reverse Weil osteotomy; instead of cutting the metatarsal from distal dorsal to proximal and plantar, cutting the metatarsal proximal to the neck and aiming plantar and distal to slide the head proximally and dorsally?

QUERIES (NON-CLINICAL)

Query: Becoming a Professional Corporation

I am contemplating becoming a professional corporation. I was advised by some of my colleagues that this could turn out to be a veritable "headache" in that I would have to fill out another Form 835 from Medicare, receive a new Medicare number, and I would have to apply for an "organization NPI" and then notify all my insurance payers of the new situation and reapply to participate with them. Some colleagues have told me that when they reapplied as a new entity, they were refused because the "panel was closed."

What is the correct way to go about this? I am a bit leery as well as confused due to all of the differing information.
