Compounding Provides Pain Relief via Topical Meds: MI Podiatrist
 
According to Conway McLean, DPM, "Compounded medications are an excellent option when the oral route is not feasible, a common challenge in the geriatric or pediatric population. When the FDA-approved medication is manufactured with a material to which someone is allergic, once again, compounding is a popular and successful choice. Or when there are special dietary restrictions, it is frequently utilized."

"The most common compounded medications are those prescribed for pain management. Various drugs can be used in one cream, from local anesthetics to anti-inflammatories, all to treat a single painful structure, but in different ways. APMA Represented at CPT® Meeting
 
APMA was represented at this past weekend’s meeting of the AMA CPT Editorial Panel Meeting. The AMA CPT Editorial Panel meets three times a year to revise (i.e., create, edit, and/or delete) CPT codes. APMA was represented at this meeting by CPT Advisor Ira Kraus, DPM, and Health Policy Advisor Jeffrey Lehrman, DPM. APMA representatives participated in this meeting as well as in the creation of the CPT code set, all while protecting the interests of podiatrists.
 
APMA representatives also attended the AMA Relative Value-Based Update Committee (RUC) meeting April 27–30, including APMA Trustee and RUC Advisor Brooke Bisbee, DPM; RUC Alternate Advisor Susan Walsh, DPM; and Andrei Pavelescu, DPM (Health Policy and Practice Committee) observing.

PODIATRIC PRODUCTS IN THE NEWS
Electronic Tuning Fork (ETF) Improves Diabetic Ulcer Prediction: Study 
 
O’Brien Medical has announced the publication of a key research study validating the performance of its product, the ETF, in the prediction of diabetic foot ulcers (DFUs). The study published in the Journal of the American Podiatric Medical Association combined vibration testing via ETF and light touch as tested by the Semmes-Weinstein monofilament in the assessment of DFU risk over a three-year period. The results demonstrated an improved ability to predict ulcers when both tests were combined as opposed to being performed in isolation.

These findings align well with current American Diabetes Association guidelines advising the use of at least two neurological screening tests for diagnosing diabetic peripheral neuropathy (DPN). Improved DPN screening can potentially provide better guidance to medical professionals when choosing preventative strategies aimed at reducing diabetic foot complications including lower extremity amputations.

Query: Matrixectomy Follow-Up
 
I have a patient who has Blue Cross insurance. He came in for a matrixectomy on February 8th and returned on February 15th for follow-up. I billed CPT 99213 using M79.673 and T81.40XA for the follow-up and the claim was denied. Is there another code or modifier that I should include?

Response: This is an issue with 10-day global procedure for CPT 11750-Excision of nail and nail matrix, partial or complete, (e.g., ingrown or deformed nail) for permanent removal. The only thing that can be billed for is an unrelated E/M. Your diagnosis you related is nice but does not get you a new E/M. T81.40XA-Infection following a procedure, unspecified and M79.673-Pain in unspecified foot. Why unspecified? However, the bottom line is that this is not an E/M because there is an infection.
 
Only if you take the patient back to the operating room or procedure room in the global period can you bill for it. That scenario would necessitate a modifier -78 - Unplanned Return to the Operating/Procedure Room by the Same Physician Following Initial Procedure for a Related Procedure During the Post-operative Period.
 
The only way an E/M is billed is if it is unrelated to this service which clearly it was not. This is called post-op care. In my practice, we use CPT 99024-Post-operative follow-up visit, normally included in the surgical package, to indicate that an evaluation and management service was performed during a post-operative period for a reason(s) related to the original procedure and it is a $0 charge. Don’t appeal and don’t append any modifiers.

I was honored to be one of many students of Dr. Elizabeth Roberts who was famous for her teaching of lower extremity anatomy at the New York College. She made the class interesting and challenging at the same time. She was a most kind-hearted teacher and a wonderful person. I got a lump in my throat when I was reminded of her and how wonderful a human being she was. I'm sure that those who knew her will feel the same.

The results to this week’s polling is puzzling. CDC recognizes that some 60% of the U.S. population has had COVID-19 which would appear to me a conservative number, yet the poll results show 65% of respondents stating they have not had COVID-19, which begs the question why is being a podiatrist "protective"? It would seem to me with our level of exposure, we would be as high if not higher than the CDC data.
