PODIATRISTS IN THE NEWS
MN Podiatrist Discusses Critical Limb Ischemia

According to Conway McLean, DPM, "Critical Limb Ischemia (CLI) is a condition which develops when your arteries become severely clogged. This clogging process involves the deposition of atherosclerotic plaque inside arteries. As more of this material builds up, as the PAD increases in severity, at some point it becomes ‘Critical Limb Ischemia’ and, as the name implies, this can be a serious situation."

Dr. Conway McLean

"Although it is an extension of PAD, CLI carries with it some very dangerous repercussions. Those with critical limb ischemia have a higher rate of heart disease, since blood vessels throughout the body will be affected by this process. For this reason, CLI is strongly associated with cardiovascular risk factors such as cigarette smoking, diabetes, high cholesterol levels, and hypertension. CLI represents the most aggressive form of peripheral vascular disease, with a lower survival rate than heart failure, stroke, and most cancers," says Dr. McLean.

PODIATRISTS AND PODCASTS
CA Podiatrist Interviewed About His New Book

Doug Richie, DPM was interviewed by Ben Pearl, DPM on Fit Foot U regarding his book Pathomechanics of Foot Disorders. In attendance were approximately 30 podiatry and pre-med students and doctors, including members of the American Academy  of Podiatric Sports Medicine. Dr. Richie reviewed the development of the human foot and what distinguishes us from other primates and enables us to walk.

Dr. Douglas Richie, Jr

Dr. Richie presented the twisted plate theory as the pivotal developmental factor that set up the patterns for human foot function. Also discussed were controversies in static measurements commonly used in podiatry versus kinematic studies which contradict some of the long-held theories of what is normal motion. Richie opined that the windlass effect from motion in the foot joints causes more tensile load than the Achilles tendon in causing plantar fasciitis. He said provocative diagnostic maneuvers are sometimes overlooked in favor of MRI studies. An example he gave was the Achilles tendon resting angle (ATRA) which is measured with the patient prone and knee at a right angle, and can be used to detect a tear.

PODIATRIC INVENTORS
NJ Podiatrist Designs New Osteotomy Guide for Minimally Invasive Bunion Repair

Thomas Graziano, DPM, MD was recently awarded a U.S. Patent for an osteotomy guide specifically designed in "minimally" invasive bunion repair. The guide was initially tested utilizing cadaveric specimens to assess its suitability and effectiveness for the surgical procedure.

Dr. Thomas Graziano

After the initial testing, development and material fabrication for the guide was chosen to accommodate the uneven contours of the bone. While there are various minimally invasive extra-articular techniques for bunion repair, Dr. Graziano feels the guide allows for an accurate, stable, and reproducible osteotomy for multi-plane correction with internal fixation.

PRACTICE MANAGEMENT TIP OF THE DAY
Why Social Media is Important to Your Practice - Part 2

Social media gives you insight into the needs of your patients. Don't ignore the opportunity to market your practice to specific patient populations.

2. Social media helps you build relationships

Whether with current patients and their families or prospective patients, social media makes it easy to stay in the forefront of patients' minds, and by lending yourself to becoming a resource you can cultivate a community around your practice.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Geoffrey M. Habershaw, DPM

On May 14, podiatry lost a giant in the profession. Dr. Geoffrey Habershaw passed away suddenly doing something he enjoyed: walking on the beach with his wife, Nanette, in Hawaii. Geoff was my first residency director, first Chief, a mentor and a colleague. So much of what I learned and have become is because of Geoff. He taught me to be definitive and innovative in surgery. He taught me to treat all patients with respect. He served as a mentor to so many residents and future diabetic foot specialists. In fact, it can be argued that Geoff helped establish surgical care of the diabetic foot as a distinct specialty.

Dr. Geoffrey Habershaw

Geoff Habershaw succeeded Jack Donovan as Chief of Podiatry at the New England Deaconess Hospital in the early 80s. He started a collaboration with Drs. ...

QUERIES (NON-CLINICAL)
Query: Changes to Board Certification ABFAS Requirements

I recently received an email from the ABFAS that seemed to suggest that continued board certification will require additional testing requirements in foot surgery. Does anybody know what these requirements are?

Also, I do not understand how the ABFAS can change the rules on current board certified foot surgeons. In the past, I would assume grandfathering was done. The email I read would suggest that current ABFAS diplomats will be subject to continued exams, etc. Can anyone shed light on this development if my understanding of it is correct.

RESPONSES/COMMENTS (RELEVANT RESEARCH)
From: Andrew Heller, DPM

I was briefly affiliated with the Baja Project for Crippled Children and we saw an abundance of clubfoot cases in Mexico. Some were due to lack of correction at birth, but we thought the outside number of cases was possibly related to the widespread polio endemic there. A possible cause could be polio of the pregnant mother. This is strictly anecdotal.
