KS Podiatrist Extols Virtues of Digital X-Rays

This year, Corin Q. Wilde, DPM treated himself to a digital x-ray, and said that the more he uses it, the more he appreciates it. “The unit I have gives the patient less exposure than the average dental X-ray, and that was one of the big selling points for me,” he said. “It’s extremely safe.” The x-ray develops in seconds, compared with traditional X-rays which take much longer and require chemicals to develop. “This uses no chemicals so it’s environmentally friendly,” Wilde said.

It’s also much easier to make a duplicate if a patient wants to get a second opinion or is moving away. “With traditional x-rays, you have to find somebody to duplicate them,” he said. “With this digital unit, I can put it on a disk, or e-mail it to the patient or the doctor they designate.” And the digital X-ray provides better visualization, which is a great help in making a diagnosis. “I can really get in tight on the digital x-ray, and I can sharpen the image, which I could not do before,” Wilde said.

MA Podiatrist Re-elected to State Podiatry Board

Dr. Raymond S. Murano, Chief of Podiatric Surgery for Hallmark Healthcare was recently re-appointed to the Commonwealth of Massachusetts Board of Registration for Podiatry by Governor Deval Patrick. He was originally appointed by Governor Cellucci in 1999, and then re-appointed by Governor Romney. He currently serves as the Board’s Secretary.

Dr. Murano is on the medical staffs at the Lawrence Memorial Hospital, Melrose Wakefield Hospital, and the Winchester Hospital. In addition, Dr. Murano was also elected to the position of Secretary/Treasurer for the Ohio College of Podiatric Medicine’s Board of Alumni Trustees.

MEETING NEWS

Diabetic Foot Highlighted in Final Day Keynotes at FIP, Amsterdam

The team approach was front and center in the two keynotes delivered on the final day of the FIP Symposium in Amsterdam. Professor Sir Karel Bakker, of the International Diabetes Federation (IDF), and Professor David G. Armstrong, of the University of Arizona's Southern Arizona Limb Salvage Alliance (FIP) teamed up to give a rousing review of the value of teamwork in amputation prevention. "I am so pleased to be here with David to talk about a real clinical marriage that works" noted Dr. Bakker. "Putting people together in teams is the name of the game. The data are now very good that we can limit problems, worldwide."

Professor Armstrong was equally positive about the present and future of care. "We are seeing the fruits of labor being harvested from seeds planted a generation ago. Works by organizations like the IDF, the FIP, the American Diabetes Association, the Society for Vascular Surgery, and the American Podiatric Medical Association are now showing that prevention can pay and that organizing arranged clinical marriages can lead to happy, healthy therapeutic unions." The two keynote addresses capped off nearly a week of meetings with foot specialists from around the globe.

There is a program you can subscribe to called Epocrates. (Epocrates.com). They offer access to patient handouts for all of the drugs they list. I checked to see if they offer a patient handout for terbinafine and indeed they do. It’s worthwhile checking out and using this program.

RE: Harkless Receives Boswick Award and Lectureship

I would like to congratulate Dr. Lawrence Harkless for being awarded the Annual Boswick Memorial Award and Lectureship at the 23rd Annual Symposium on Advanced Wound Care and Wound Healing Society Meeting in Orlando, FL. The John Boswick Memorial Award celebrates the memory of the late John Boswick, MD, a burn surgeon who contributed immensely to the field of wound care. This lifetime achievement award is the highest honor given by the Association for the Advancement of Wound Care (AAWC) and the Symposium on Advanced Wound Care (SAWC). I cannot think of another physician more deserving of this award than Dr. Lawrence Harkless.

Dr. Harkless is a teacher and mentor to many. His many contributions has tremendously impacted the field of medicine and wound care. A vascular surgeon from my area attended the Orlando Meeting in which Dr. Harkless was honored. This well-respected surgeon returned from the meeting impressed and enlightened. He has extended an invitation for me to treat his complex diabetic foot and ankle patients and wishes to collaborate more frequently. The efforts, professionalism and accomplishments of Dr. Harkless have created additional opportunities for me and other podiatric foot and ankle surgeons across the country.

RE: 50Th Anniversary of Morton’s Death

Fifty years ago this month (May 1960), Dr. Dudley J.  Morton died. The last article he ever wrote called "Immaterial Factors in Common Foot Disorders" was published in the Journal of the American Podiatry Association. During his lifetime, his work concerning the hypermobility and/or abnormal shortness of the first metatarsal bone was acknowledged by medicine as being one of the basic tenets of why and how people had foot pathology. Since his death, this basic and vital information has fallen off the radar screens of most foot specialists.

This is a shame because many of your patients who present daily with some type of orthopedic problem such as bunions, hammertoes, heel pain, a neuroma, or metatarsalgia will have either or both of these changes on x-ray. This is not a coincidence. Just look for it. It will be there on most of your DP views. The good news is that a simple pad along the first metatarsal bone shaft will stabilize the whole forefoot and help limit the abnormal pronation at toe-off, caused by Morton’s toe. We do this with forefoot posting in our orthotics, but I believe it is not as good as the pad along the first metatarsal bone shaft. This pad does not extend onto the hallux. I have used this pad daily for almost thirty-five years in my office. It works. Try it. On the fiftieth anniversary of Dr. Morton’s death, I hope many of you can rediscover his work, and help your patients.

I called the company on Thursday and asked them some questions about "meaningful use" and some technical questions. They said that once the requirements come out that are set for podiatry, they will add them in. I also e-mailed them other questions to see how responsive this free program would be. They seemed knowledgeable and...
