Student Research Poster Day at NYCPM
 
On May 13th 2024, the Class of 2025 at The New York College of Podiatric Medicine presented the results of their year-long research projects during the annual Student Research Poster Day. After obtaining CITI certificates for the Protection of Human Research Volunteers and completing IRB submissions, students in groups of four or five worked closely with faculty mentors to conduct research in keeping with good clinical practices. The research topics included original work in surgery, radiology, gait, and presentation of interesting and unique clinical cases.

The projects were judged by D. Scot Malay, DPM, MSCE, Director of Podiatric Research, Penn Presbyterian Medical Center with the assistance of Hye Kim, DPM, who in addition to being an alumna of NYCPM, directs the course on Research Methodology. Awards were made for the top three posters with the first-place winner receiving $1,500 generously donated by the Queens Division of the New York State Podiatric Medical Association. The students and faculty mentor of the first place poster are shown.

TN Podiatrist Sees More Running Injuries in Summer Than Winter
 
According to Cary Gannon, DPM, "Shoes that fit properly are critical. Too often, I see people wearing shoes that are far too small. The foot requires ample room to expand during a run. Proper attire is also a must. We recommend dry natural fiber socks, headgear, and layered clothing. Compression gear, such as compression socks, sleeves, or pants are also important for recirculating blood and for aiding recovery."

"I always see more injuries in summer as opposed to winter. People who run during the winter are typically seasoned runners who have been training for quite some time. Novice runners generally commit when the weather is nice. Oftentimes, these newly-focused athletes develop plantar fasciitis, stress fractures, and other injuries simply because their bodies are not accustomed to training," says Dr. Gannon.

San Antonio is Diabetic Foot Capital of the World: TX Podiatrist
 
Sky-high diabetes-related amputation rates in San Antonio have doctors sounding the alarm. Although Texas sees some of the highest rates of diabetes-related amputations nationwide, the City of San Antonio is the epicenter of this crisis. “It’s a huge issue in San Antonio and dare I say, it’s the diabetic foot capital of the world, in terms of complications,” remarked Michael Sobolevsky, DPM, a podiatrist with the Texas Diabetes Institute (TDI) in San Antonio, per The New York Times. “We are constantly amputating.”

The loss of a leg or foot due to diabetes diminishes a person’s five-year survival rate, according to the American Diabetes Association (ADA). In Bexar County, the diabetes mortality rate is 36.3 per 100,000 people, which is considerably higher than the national average of 25.4.

You Can't Train Your Feet to Wear High Heels: IL Podiatrist
 
By definition, a high-heeled shoe places the foot and ankle in an improper anatomical position," says Sidney Weiser, DPM, a podiatrist and founder of Quality Podiatry. As a result, the heel places unnecessary pressure on the forefoot and ankle, increasing instability of the ankle and heel, in turn, upping the risk of injury, he explains. And because of the instability and unnatural position that heels force your foot into, it's unlikely that your feet can actually be "trained" to wear pumps, Dr. Weiser says. After all, PopSugar previously reported that no one has feet perfectly shaped to fit the form of a high heel.

Dr. Weiser says, "I hate to be the bearer of fashion bad news, but no. Putting your foot in an unnatural position for an extended period of time is not healthy. Since a high-heeled shoe puts more pressure on the forefoot, it can contribute to pain such as corns and calluses, and increase pressure on hammertoes and bunions." Wearing high heels on the daily can also increase your risk of muscle imbalances, ankle sprains, foot pain, and swelling of the intermetatarsal nerves, Dr. Weiser says. Why? High heels naturally place more pressure on the ball of your foot, overloading the joints and leading potential to pain and inflammation.

After 40 years of providing dedicated podiatric services to patients, Mark Ryerson, DPM has announced his retirement from his practice with Northern Illinois Foot and Ankle Specialists (NIFAS), effective June 1, 2024. RE: Is the Limited License of Podiatric Physicians Costing the Healthcare System More Money?

I had an opportunity this week to attend a pharmaceutical-sponsored county podiatric meeting here in South Florida, and the speaker was from out of state, a well known expert, discussing wound care grafts to apply. The lecture showed multiple pictures of completely or almost completely granulated wound beds to which they were applying a placenta graft tissue weekly at $2,500 per application, up to a total of $25,000 per application. When I queried why not just put a split-thickness skin graft over this wound, I learned that it was not within the podiatric license in the state in which this person practiced.
 
This got me thinking, is our limited license actually costing the system more money because of the inability to apply all the best medical practices by limitations of licensing? I know that while I practiced, I often would apply a split-thickness skin graft on a fully granulated wound bed. I was quite happy with then using additional products as needed, if necessary, What do others think about using these expensive grafts where the company makes all the money and the doctors do not?

Since this thread is still going, I would like to bring up a really important point that Dr. Meisler glossed over. Patients coming from these concierge practices were willing to pay directly when they came and were "surprised that they did not have to pay at the time of their visit." That should tell you something about the value they are experiencing in a direct-pay practice. That's number one.
 
Number 2, I agree with Dr. Meisler that eliminating poor payors will always make room for more value in the practice. However, it is important to note that as long as you continue to accept insurance, you will always be fighting an uphill battle. You will need to see a high volume of patients which means a large office, large staff, and high expenses. You will still have to navigate the slippery slope of...
