MI Podiatrist Discusses Treatment of Clubfoot
 
According to Conway McLean, DPM, "Technically termed talipes equinovarus, a clubfoot entails an abnormally positioned foot, such that the foot is twisted in three different directions. Treatment is recommended since the deformity makes it nearly impossible to have a normal gait pattern. Early intervention is vital since the abnormally positioned extremity becomes more fixed with time. But surgical correction of the deformity in newborns is rarely attempted initially as a form of treatment due to the scarring and high rate of complications."

"Manipulation of the foot out of the deformed position has become the gold standard globally following the widespread dissemination of the Ponsetti method. With this technique, continuous force is applied to the affected joints over time, stretching the tightened ligaments and joint capsules to relax gradually until they achieve the proper anatomical position. Once sufficient mobility of the pathologic tissue is achieved, an above-knee, long leg plaster cast is applied to hold the limb in the new attitude. The cast is changed weekly with most responding sufficiently after 5–7 weeks," says Dr. McLean.

The lerEXPO online event entitled Lower Extremity Skin Cancer: Detection, Diagnosis, and Treatment brought together over 300 clinicians for a 2.0 CECH event this past Saturday. The speakers included William Scherer, DPM and Mitzi Williams, DPM. “Attendee engagement is at an all time high, “said Rich Dubin, CEO lerEXPO. “While in-person meetings are open, the demand for online learning is stronger than ever.”

MS Podiatrist Gets 2 Year Sentence in Foot Bath Fraud Scheme
 
A Panola County man was sentenced today to two years in prison following his conviction for his role in a scheme to defraud Medicare and TRICARE by prescribing and dispensing medically unnecessary foot bath medications and ordering medically unnecessary testing of toenails in exchange for kickbacks and bribes. According to court documents, Marion Shaun Lund, DPM, 54, owned and operated a podiatry clinic, as well as an in-house pharmacy. Lund routinely wrote prescriptions for and dispensed antibiotic and antifungal drugs to be mixed into a tub of warm water for patients to soak their feet.

Mid-calf Nerve Block Promotes Fast Recovery after Total Ankle Replacement
 
A mid-calf nerve block may lead to early rehabilitation after total ankle replacement. The mid-calf nerve block may challenge the current standard of popliteal blocks for foot and ankle surgery.
 
“What this new block is allowing us is opening the door for a new opportunity for a fast-track, early rehabilitation for patients with foot surgery, especially total ankle replacements,” Enrique A. Goytizolo, MD said. “The surgeons are excited for this because we are breaking ground here,” he concluded.

Thank you Dr. Kleinman for your suggestion to take a skin biopsy. I absolutely agree and strongly recommend that a dermatopathologist is utilized for the biopsy analysis for this particular case, and in general, all skin ulcers. Multiple forms of biopsy are available; a punch biopsy is very popular for dorsal aspects of the foot and ankle. If using a punch biopsy, it's important to control depth to avoid periosteum, tendons, etc.
 
If there are questions concerning biopsy technique preferred or location of the biopsy sites, always feel free to contact your pathologist and coordinate the procedure. Pathologists sometimes have different preferences for techniques, etc. for various skin lesions. It's always best to use the method preferred by that particular dermatopathologist and, in my experience, always appreciated. It also serves as a best method to establish a relationship, which is a very important "adjunct" to your practice.

RE: Caught in the Employee Salary Squeeze

A friend sent this to me from a gas station called Buc-ee’s in North Florida. These jobs include health insurance and three weeks of paid vacation. It got me thinking about how doctors and hospitals can afford to pay significantly more than these prices to keep the employees they have from leaving?  
 
High paying jobs with "no experience necessary"
 
What are people paying employees? Will these prices in the gas-station world trickle up to medical offices? With insurance payments for procedures not increasing, how are groups handling the “squeeze”? Please don’t tell me you are closing your practice to apply for the GM Job.

RE: Hoka Should Provide Professional Courtesy to Podiatrists

Hoka is currently one of the hot brands in athletic shoes. Although they are not on the top of my list when recommending an athletic shoe to patients, I have referred a good amount of patients to certain styles within their line, most notably, the Bondi. Unfortunately, Hoka does not offer any professional courtesy discount to podiatrists. 
 
After inquiring about a discount on Hoka shoes for myself and my wife, I received an email back from Mary Williams who is the pro sales coordinator at Hoka, saying that they do not offer any discounts to the medical field. This post is not a personal vendetta against Hoka. On the contrary, I like their line and would never discourage a patient from buying their products. It does baffle me that a company that is promoted by our profession does not provide this benefit. To me, this is very poor judgment on Hoka’s part.

Kudos to Dr. Ritchie on his presentation on how to get insurance to cover AFOs. It was well presented and timely. Many past webinars and articles presented or written by podiatrists, orthotists, pedorthists, and others have discussed this subject. However, there remains resistance to providing AFOs, particularly when the provider identifies a patient has already received another AFO within the past five years (AKA Same or Similar). While I understand the resistance and difficulties and time costs associated with the appeals process, this should be straight-forward and can be streamlined.
 
It is important to remember that you as the prescriber and supplier, unlike with therapeutic shoes, have 100% complete control over...
