PRACTICE MANAGEMENT TIP OF THE DAY |
7 Financial Reports Your Practice Needs to Run - Part 2
Most physicians readily admit that classes on how to run a successful medical practice were not part of the curriculum in medical school. While "financial reporting," might not have been a class at your medical school, experts say you need to at least become familiar with understanding some of the basic reports. Here are seven to track.
2. Profit and Loss Statement (P&L) "A P&L tracks revenue and expenses, showing the profitability of your practice. It can also track key performance indicators such as total encounters, number of procedures, total charges, and collections. Red flags could indicate a sudden drop in collections. Before deciding a course of action, look for an explanation behind any warning signs. For example, a decrease in collections this month should have been preceded by a decrease in charges last month." - Ada Stewart, MD, AAFP Board of Directors

QUERIES (CLINICAL) |
Query: Post-Restylane Injection Complication
A 58 year old male presented with a painful corn on the inside of his little toe. Three months prior, a podiatrist treated the corn with an injection of Restylane dermal filler to the little toe, with temporary results. His toe is slightly bulbous and has a spongy (not hard) feeling and is excessively flexible.
|
X-rays post Restylane Injections |
The x-rays (DP & oblique) are quite disturbing in appearance. His past medical history is unremarkable other than kidney stones. The remainder of the x-rays are unremarkable. Is anyone familiar with the use of this dermal filler and is this a normal appearance post-injection?

QUERIES (MEDICAL/LEGAL) |
Query: Billing Company Negligence
8 months ago, I hired a new billing company highly recommended by other physicians in my area. My income dropped, which I did not notice immediately. I did a random audit of my accounts consisting of a small sample of 17 accounts and found that 4 of the accounts were not paid or followed up in the period of 8 months. I fear that this small sample represents what has been happening for the entire 8 months. I am sure some of the claims with insurance companies will not be paid due to the 3 or 6 month statute of limitations. Does the billing company have any responsibility to pay me my lost income due to their negligence? The contract with the billing company mentions nothing regarding this issue. Has anyone had a similar experience?

CODINGLINE CORNER |
Query: Hallux Rigidus - Arthofibrosis
I have a patient with hallux rigidus and arthrofibrosis. I am planning to take this to the operating room to preform manipulation under anesthesia. I am not sure if there is a CPT code that represents this procedure. Thoughts?
Douglas Robinson, DPM, Campbell, CA
Response: There is no specific CPT code for this so you would use the unlisted procedure code.
Make sure you pre-authorize this as I do not think many carriers would cover this procedure for the foot. The patient could then be responsible for all costs

RESPONSES/COMMENTS (NON-CLINICAL) |
RE: MIS Returns to Podiatry
From: Joseph Borreggine, DPM
With the recent article that appeared in PM News, I began to wonder why this type of foot surgery has had an unbelievable resurgence in light of the years that have passed that caused podiatry to distant itself from this type of surgery in lieu of the more traditional techniques taught in the APMA and CPME mandated three-year, post-graduate DPM surgical residency?
I graduated in 1988 and received my DPM from Scholl College. One of the reasons that I decided to become a podiatrist was because I found an interest in minimal incision surgery or “MIS” as it was...
Editor's note: Dr. Borreggine's extended-length letter can be read here.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) |
From: J. Scott Rosenthal, DPM
I agree with Dr. Roberts that attire is a personal decision and varies greatly with the locale and spirit of the office and patient population. I am admittedly one of those shameful podiatrists who dresses casually with slacks and a button down shirt. No tie, no white coat, no scrubs. Never have I had a patient question my attire, except for the few times when I happen to wear a tie for a meeting, when they often ask, "Why are you so dressed up?"
J. Scott Rosenthal, DPM, Los Angeles, CA

RESPONSES/COMMENTS (NEWS STORIES) |
From: Bruce Jacob, DPM
In response to Dr. Beaton, I am not sure of Florida laws, but here in Michigan, ANY physician can delegate to ANY employee whatever medical tasks he or she is willing to delegate as long as that physician is within the confines of the office. It is the old "captain of the ship" policy.
Bruce Jacob, DPM, Sterling Heights, MI

RE: Outrageous Shoe of the Day
|
Ugly (Duckling) Shoe? |
Inspired by the fairy tale by Danish author Hans Christian Andersen. The story of the Ugly Duckling tells of a homely little bird born in a barnyard who suffers abuse from the others around him until, much to his delight and to the surprise of others, he matures into a beautiful swan, the most beautiful bird of all.
