Rep. Wenstrup Visits MedStar Washington Hospital Podiatric Residency

Rep. Brad Wenstrup, DPM, (R-OH) visited MedStar Washington Hospital Center in Washington, D.C. this week to discuss his career in podiatric medicine, the U.S. Army, and now the U.S. Congress. The dialogue covered topics ranging from the impact of healthcare reforms to the current podiatric residency shortfall for graduates. The Congressman was accompanied by his Legislative Director, Lisa Collins, who also provided helpful background information on the healthcare topics and offered follow-up for the questions raised by the participants.

The audience members included podiatric residents, students, and attendings of the training program. One key point of the discussion was an emphasis on the importance of contacting your congressional representatives and making sure your opinion is heard. MedStar Washington Hospital Center is a 926 bed major teaching and research not-for-profit hospital in the nation’s capital, and  sponsors a 3-year podiatric medicine and surgery residency program with 15 residents.

RE: Difficult Post-Trauma Case

The history, aspirate, and MRI all indicate this is a hematoma. Assuming adequate vascular status and healing potential, I recommend incising it, drain all fluid/thrombi, explore the dead space and cauterize any vessels bleeding into it. Leave at least a portion of the incision open without sutures so it can continue to drain as it heals, and let it heal by secondary intention. This way you can manually compress out any thrombus that accumulates later. The pain usually resolves almost immediately and the incision should heal in a month or so with adequate wound care.

It appears as if one or more of the branches of the intermediate dorsal cutaneous nerve were damaged with this, which is often overlooked in most blunt traumas. Start applying some type of topical compound pain medication with anti-inflammatory medication in it as well. See how the patient responds. Performing a more proximal block with Marcaine in the superficial peroneal nerve may be helpful as well. Surgical repair may be needed if the patient does not improve over the next few months. The cysts may just be adding extra compression to the affected nerves.

RE: Painful Bunion in a 12 Year Old

Additional non-surgical treatment should be attempted first due to her youth. Make her new properly-designed custom orthotics to stabilize her foot and suggest proper shoes. See if you can improve her symptoms, and allow more growth and maturity for at least 4 years.

She is still a bit young for bunion surgery unless good conservative care is tried and failed. An "aching" joint on an initial presenting visit and a diagnosis of bunion should not automatically mean...

The limit for bone graft in brachy metatarsal correction is about 1cm. This option is technically easier with adjacent metatarsals. In this patient, the deficit appears greater than 1.5cm. A grafting procedure can still be performed if suboptimal correction is acceptable to the patient. We have performed simultaneous adjacent callus distraction procedures with excellent results.

Patient selection is important as they have to participate in the process. In the attached picture, we used the Coretrack fixator from Wright Medical due to anatomic constraints. We have used the EBI Mini-rail on adjacent metatarsals as well.

RE: Dragon Problem (Rob Eells, DPM)

The main feature that is important with Dragon is the fact that you can do macros, and it navigates through your note via commands. If you master all of the commands and tricks, it is a powerful tool that you will not find in any other program. Take the time to read the manual and blogs out there so you can see how powerful Dragon can be. Of course there are other options for speech recognition, but that is only a small part. The cost of Dragon is a drop in the bucket when you look at the efficiency gained in your practice when the program is used to its full extent. So I respectfully disagree, but it is pretty cool that the iphone works that well.
