OH Podiatrist Presented with International Aurora Award
 
James J. LaPolla, Jr., DPM, podiatrist and owner of Northeast Ohio Foot, Ankle & Wound Centers, is a recipient of the International Aurora Award. The award is presented by Kerecis Corp. to doctors who provide outstanding case presentations and research papers pertaining to the science of dermatology, wound care, and burn treatments. The recipients of the award are selected by an independent committee.

Dr. LaPolla, affiliated with Steward Sharon Wound Care Center, was one of the 12 recipients of this award from over several hundred submissions. In addition to receiving a recognition plaque and certificate, he also was provided with an educational stipend and was invited to present his abstract paper at a national or international wound care meeting.

NY Podiatrist Discusses What to Look for in a Wedding Shoe
 
What shoes are good for a wedding? Short answer? Comfort means different things for different people. "Comfort is subjective, so what is comfortable to one person may not be comfortable to the next," says podiatrist Emily Splichal, DPM. But, she says that those of you with flat feet, high arches, plantar fasciitis, or Achilles tendinitis may find flat shoes to be uncomfortable.

So, you'll want to opt for something with a bit of a heel, especially for a wedding at which you may be wearing the shoes from a morning ceremony until a late night reception. But, as Dr. Splichal says, comfort is totally subjective. And, since every type of foot is totally different, you'll want to find a shoe that caters to your needs.

We have a new provider who has come to our office from a previous practice. Some of her patients are following her to our practice. We do not have access to any of the previous records from the provider's previous practice. If she has seen them at a previous practice within 3 years, can we still bill for a new patient visit when she sees them at our office?

RE: Educate Patients About Insurance Companies

People need to be educated on what insurance companies really are. They are money exchangers or money transferring entities. They transfer money from their subscribers or customers, keep a large chunk of it, and give a small percentage to the actual health caregivers. It is funny that when you ask patients what is the purpose of health insurance, most of them say, "I have no idea; they just take my money." 
 
It would be great if we could post how much money all of the CEOs and CFOs and COOs make and put it into commercials, and juxtapose that with all the times they deny patients any type of care, or show how much they pay for a three-hour surgery, etc. People need to wake up.

RE: Being There

A podiatrist’s office manager calls the director of a recent meeting which I attended. The office manager states that they had paid tuition for the meeting, but had not yet received the link for the virtual transmission of the meeting which was to begin in 30 minutes. The manager was informed that the meeting was not virtual, but required attendance. The manager responded, "but that’s impossible. the doctor has patients all day both days, and cannot possibly be there." 
 
Thinking about this, I have started a new business. Remember during Covid, the cardboard cut-outs people had in the stands at sporting events? I am now producing these for use at live meetings (protects you from Covid) or to be placed in front of your computer during a virtual meeting. If you would like one, send $65 and your photo. Options include your name on a white coat, with either DPM or foot and ankle surgeon imprinted. Please request white coat, scrubs, or business casual.

I found the gap between the survey showing a majority of podiatrists are happy with their career choice, and the paucity of those who would recommend it to their child. Being a solo practitioner has to be one of the worst business models out there. My income, as well as my daily financial liability, depends solely on my showing up. That makes it one tragedy away from complete financial disaster. In addition, we are bound by reimbursements that do not align with inflation, and there is very little we can do about it. In other words, we have lost all control of our financial outcomes with the exception of working harder for less.
 
I’m a second generation podiatrist, and boy has medicine changed. When my father was in practice, he saw half the number of patients I see and made twice the income. Doctors were once able to use all diagnostic tools at their disposal to rapidly diagnose and treat patients. Today, patients are made to suffer for extended periods of time to meet the “requirements” of the insurance body in hopes a diagnostic modality will be approved. And, that is after the doctor spends extended time and employee wages to document and submit the necessary forms.
 
These are just a few of the reasons I would not recommend medicine as a career for my child.

Name Withheld responded to this question with a statement indicating that when he/she started podiatry school, it was his understanding that the APMA Resolution "Vision 2015" was to achieve parity with MDs and DOs as physicians. He/she further stated that while much has been accomplished, we are still seen as "second-class" doctors. He/she regrets that despite "Vision 2015", podiatrists cannot sign off on diabetic shoes and are not being considered an essential provider, among other things. 
 
I appreciate this young practitioner's frustration and disappointment. How is it possible that even though we can perform complex foot and ankle surgery without the blessing of an MD or DO or can prescribe and administer virtually any pharmaceutical agent including antibiotics and narcotics, we still need a sign off for patients whom we conclude require diabetic shoes.
 
While this is absolutely absurd, it further emphasizes the importance for the profession and all its components to complete the obvious task of once and for all making it the highest priority of not just achieving "parity" with the MD and DO, but becoming integrated within the broad spectrum of medicine in the same manner that all the other medical specialties are (e.g., dermatology, OB-GYN, ophthalmology, etc.). That is what we already are, that is what we must do; let's stop dancing around this issue and make its achievement the highest priority in the profession.
