PODIATRISTS IN THE NEWS - PART 1
Ankle Replacement Best For Sedentary Seniors: PA Podiatrist

Dr. Benjamin Overly has become known for his expertise in ankle replacement surgery and attracts patients from around the country. Overly leads the newly created Coventry Foot and Ankle Surgery practice, a specialty recently formed by Pottstown Memorial Medical Center. Ankle replacements in the U.S. began in the early 1970s, but Overley said the practice was completely abandoned for about 10 years. He said instruments are better now, and there are several approved systems in the U.S. As a result, the number of replacement procedures done each year is increasing.

Overley said the surgery is generally best for someone age 60 or older, but the patient’s activity level is also a factor. “Someone who sits behind a desk would be a better candidate than a marathon runner, sports hobbyist, roofer, or construction worker,” he said, adding that the procedure has been offered to patients in their 20s and 30s with rheumatoid arthritis. “They’re not looking to do anything exciting with the ankle — they just want pain relief,” he said. Overley said 70 percent of his patients suffer from post-traumatic arthritis and have a history of sprains, multiple sprains, or fractures.

PODIATRISTS IN THE NEWS - PART 2
NY Podiatrist Provides Some Summer Foot Health Tips

Summertime, and the living is easy. However, the warm weather can mean a lot of wear and tear on the feet as sandals become the footwear of choice. New York podiatrist Dr. Jackie Sutera, who also serves as spokeswoman for the American Podiatric Medical Association and a Vionic Innovation Lab expert, has some tips for keeping feet healthy as summer approaches. “The big mistake women make is waiting all winter before they look at their feet,” said Dr. Sutera. Since dry skin can actually be a foot condition such as eczema or psoriasis, women with these issues can be more vulnerable to picking up bacteria and viruses living in the salon.

Dr. Jacqueline Sutera

Let nails breathe between polish changes. “It’s important to take a break for a day or two,” she recommends. Don’t let polish remain on toenails for six weeks, slowly chipping off. Instead, remove it completely with a polish remover. “Nails don’t breathe well, and you can end up with stains or white lines as they become dry and dehydrated.”

PODIATRISTS OVERCOMING ADVERSITY
NJ Podiatrist Lives His Motto "Do Great Things"

Four years ago, Dr. Robert Conenello was enjoying life with his wife, Gail, their three kids, and a thriving practice as one of the top podiatrists in the area. A Ridgewood resident, he attends his kids’ sports games, is engaged in the community, and looks forward to running the Memorial Day Ridgewood Run. Life was good. But in 2012, he started to experience a sharp pain behind his ear like "someone sticking a knife in my ear and twisting it," he explains. He was diagnosed with glossopharyngeal neuralgia, a syndrome that consists of episodic ear and throat pain in the ninth cranial nerve. In May of 2012, he underwent a four-hour surgery followed by four months of recovery.

"I felt better after the surgery but not 100 percent," Conenello states. Over the next two years, the symptoms returned. "I was devastated," he recalls. "I just wanted a normal life and all the pain came back." He returned to his doctor who conducted more advanced testing and found the cause of the new pain, and more bad news – a cancerous tumor on his...

PODIATRISTS AND THE LAW
MS Podiatrist Accused of Animal Cruelty

A Corinth doctor is accused of animal cruelty in a case out of Tennessee. A crate of nine puppies was found abandoned next to the Wal-Mart store in Selmer Tennessee. Selmer police say by using store security cameras, they are able to link a Corinth podiatrist to the abandoned animals. Police say the podiatrist is seen on tape abandoning the dogs. She is facing animal cruelty charges and has until May 24th to surrender. The McNairy County Humane Society says all of the animals need medical care and one of the puppies has died since being dumped.

MACRA UPDATE
Tight MACRA Deadlines Could Result in Doc Penalties

Health information technology experts say the tight timelines surrounding a new round of clinical quality and IT measures may result in penalties for some physicians if those measures require significant changes to electronic health records and other systems. The agency has released a timeline to implement the two new payment schemes, which replace the sustainable growth-rate formula for physician reimbursements under Medicare. It plans to release the new quality measures that will start being measured on Jan. 1, 2017.

If there are big changes to the quality measures, “there is no way that this can happen in time,” said Michael Martz, chief information officer of Ohio Valley Health Services and Education Corp. in Wheeling, WV. “I would say they need to give us a year and two months, and not two months, to implement these new quality measures.” Payments or penalties under MIPS will be calculated against four performance categories: quality, resource use, clinical practice improvement activities, and “advancing care information.”

QUERIES (CLINICAL)
Query: Verrucae in Patient with Myasthenia Gravis

I have a patient with myasthenia gravis with a severely compromised immune system and multiple recurrent verrucae. We have used laser which made a short-term improvement, and have prescribed Tagamet which the patient could not tolerate. Can anyone suggest topical treatments for this patient?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Running Shoe Stores and "In-Store Custom Orthotics"
From: Keith Gurnick, DPM

Can't anything be done to stop the shoe fitters and shoe sales personnel who work at running shoe stores from selling "quasi-custom on-the-spot milled orthotics" from being made and sold to the public? Too often, I have athletes and runners come to my office with foot, ankle, knee, hip, or other postural aches and pains, who have purchased upon recommendation from a shoe salesperson these inserts that they are told are "custom foot orthotics." Unfortunately, these orthotics do not fully address their individual biomechanical deformities nor their specific conditions.

Although what they get may appear to look like custom prescription foot orthotics, they are certainly not because they were not ordered nor were they prescribed by any doctor for any...

RE: Residency Programs in O&P and Pedorthics

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Shoes of the Day

Few forms of sadistic torture in this world are more excruciatingly painful than having to walk across a field of LEGOs left on the ground by a careless child. Fortunately, there is now a way to protect yourself from these domestic minefields: LEGO, working together with French advertising agency Brand Station, has invented anti-LEGO slippers! It only took them 66 years, too!

The official way to prevent Lego pain

The slippers, which feature tons of padding, will be produced in a limited run of 1,500 and will be distributed randomly to people who fill out Christmas wish-lists on LEGO’s French website.
