Hard Floors May Be to Blame for Foot Pain: UK Podiatrist
Hard floor surfaces can exacerbate all sorts of common foot problems, including flat feet, plantar fasciitis, and metatarsalgia. Helen Branthwaite, a consultant podiatrist and musculoskeletal lead for the Royal College of Podiatry, says this is true for many British people because we largely grew up with more carpets in our homes. "It’s about conditioning," explains Branthwaite, who is also a senior lecturer in clinical biomechanics at Staffordshire University.

Carpet is soft and textured, offering feet similar support to footwear; each step on carpet spreads the load slightly differently, allowing our feet greater flexibility of movement. A hard floor focuses the load onto certain parts of the foot though, and the increased amount of impact forces going through our feet makes the muscles work harder than carpet, with a reduced range of movement, leading to a repetitive strain effect, explains Branthwaite.

CA Podiatrist Discusses How to Get Insurance to Cover AFOs
Don Pelto, DPM recently discussed the same or similar rules and how to explain AFOs to your patients with Doug Richie, DPM on Dr. Pelto's podcast Podiatry Practice Mastery. Dr. Richie recommended when prescribing AFOs to use guidelines from Medicare as the standard for AFO coverage. He continued by saying that the useful lifetime of an AFO should be five years.

Regarding the same and similar rules, Dr. Richie stated there are other types of DME items in the same or similar description categories that can make prescribing braces more challenging in private practice, and there is a Medicare portal to determine if the patient has received DME in the past five years. If there is a different device, document why it is medically necessary for your charts and be prepared to file an appeal if denied.

Query: Reimbursement For Ultrasound Studies
CPT 76882 and CPT 76881 ultrasound studies are not covered by Horizon or Novitas insurance companies. I am not sure if Aetna, Cigna, or United Healthcare cover them. The rejected claims we receive are returned as “not medically necessary” or “this provider not authorized.”
Is it possible to become certified as an ultrasound technologist to obtain reimbursements? I would appreciate a response from those who have been reimbursed or perhaps from the ultrasound sales representatives who promote their product to podiatrists.

Response: In my opinion, this is payer speak for: “we just don’t want to cover this for you.” There are but a few locations that require a certified technologist to do these tests. They are perfectly within the scope of the DPM to be performing, evaluating, and documenting for these codes. These are often rejected outright in some states, such as Massachusetts. It does vary from state to state. The “not medically necessary” claim is bunk and should be appealed. They are commenting with no regard to your notes. The “this provider not authorized” could stem from a state regulation but those are few and far between. I would appeal and reach out to a provider advocate, if you have one, or the private insurance representative from APMA in your state.
