The Way You Tie Your Shoes Can Affect Foot Pain: DC Podiatrist
The secret is out about the mysterious shoestring holes on the top of your sneakers — it turns out they are there for a reason. Most runners, hikers, and walkers don’t even bother with them. But anyone who gets blisters along their toenails should take a second look. “The top two eyelets are there to lock the foot and ankle towards the rear of the shoe,” says Dr. Lee Firestone, a podiatrist with Foot and Ankle Specialists of the Mid-Atlantic. He says when people run or hike downhill, the foot tends to slide forward, and that can create blisters on toes. Athletes tend to think they need different shoes or socks, when all they may really need is to lace properly through the top eyelets.

It’s a technique Firestone — an experienced marathoner and running coach with the Montgomery Country Road Runners — has taught to patients and elite athletes. And he says he is not surprised that a video explaining “the lace lock” has gone viral on the Internet. Firestone says a good athletic shoe is essential, and reputable runner stores have staffers who can teach customers the proper lacing for their feet. It is important year-round but especially in the sticky D.C. area summer when feet swell and sweat more, and the risk of blisters is higher.

OK Podiatrist Discusses High Heels and Foot Pain
Women spend an average of $20,000 on shoes in a lifetime. It’s a high price, but you could be spending a lot more at the doctor’s office. “Pinched nerves or hammertoes or bunions; they also have increased tendonitis and heel pain,” said Dr. Scott Shields, a metro podiatrist. “There’s a lot of evidence that it can hurt your knees and your back.”

One study found a 26 percent increase in knee-joint pressure when a woman wears her perfect pumps. “It can get to the point you have to do surgery,” Dr. Shields said. “I would tell women to enjoy their life, to wear what makes them happy, but pick a shoe that doesn’t hurt,” Dr. Shields said.“Really try to stay away from the sale bin,” Dr. Shields said. “If you’re going to wear a dressy high heel shoe, buy a better one.”

High heels cram toes into a tight pocket, directing excessive amounts of force to the heels. Sarah Jessica Parker, who was famous for wearing them, had to swear off them after her foot rebelled by growing a protective layer of tissue. But most women have long accepted the pain.

“My patients come to me and beg me to fix their feet,” says Dr. Michele Summers Colon, a podiatrist in El Monte, CA. She recently started 34 Minute Shoes—a reference to how long women can stand in normal heels without pain—which is also rethinking the heel. “They ask me to operate on them and inject them—anything so that they can keep wearing high heels.”

Singapore Podiatrist Discusses Bunions
A bunion is a bony bump on the side of the big toe that occurs when the joint at the base of the big toe is out of alignment. It is considered a deformity of the foot, said Dorcas Sholanke, head of the podiatry department at Tan Tock Seng Hospital (TTSH). To reduce bunion discomfort and pain, Sholanke said, the first line of treatment usually involves non-surgical methods. For instance, wearing suitable footwear with orthotics and insoles can help relieve the pressure on the bunions.

Surgery is usually recommended when high levels of pain are experienced over a prolonged period of time and/or when the symptoms start to limit the patient’s daily activities, said Sholanke. Left untreated, bunions may sometimes lead to further problems, including arthritis in the joint of the big toe, said Sholanke.

From there, you can make your diagnosis.

My patient stepped on a sea urchin receiving 20 spines (subcutaneous) in the forefoot. Do I bill CPT 28190 x 20?

Response: If these were not sticking out of the skin, leading to you just pulling them out, but rather you did 20 separate procedures to remove them, then you can bill as you described as a single line item x 20 units, or you could bill CPT 28190 on 20 different lines with a "-59" modifier on 19 of them. I would expect either of these options to be met with resistance by the payer, and suggest submitting a report and maybe even photos along with your claim if you try either of these.
The other option would be to bill CPT 28190 with a "-22" modifier, which indicates that the work performed was substantially greater than typically required for that code.
