MA Podiatrist Helps Fund Scholarships While Having A Little Fun
This past weekend, Louis DeCaro, DPM, incoming president of the American College of Foot & Ankle Pediatrics, helped raise money during the second annual three-on-three Chief John F. Skroski Memorial Wiffle Ball Tournament held in Whately, MA. Organizer of the event Chris Skroski accepted donations from sponsors like the DeCaro Total FootCare Center.

Over 50 players attended the event which was filled with activities for both kids and adults. Enough money was raised in order to fund a pair of scholarships for senior boy or girl basketball players at the local Frontier and Hampshire Regional high schools. "Having fun and raising money for kids is what it's all about. It doesn't get any better than that," said DeCaro.

RESPONSES/COMMENTS (CLINICAL) - PART 1A |
From: Thomas Graziano, DPM, MD, R. Kurt Meier III, DPM
The best treatment for this "displaced" 5th toe fracture is time. From the one radiograph presented, it does not look grossly displaced. Leave it alone. It is only "mildly sore" with edema by your admission. This is normal healing for this period of time. Don't let her pressure you; leave it be. Time and patience are great healers.
Given the location, I would just remove the head of the proximal phalanx....make it like an everyday arthroplasty. To try to fixate it, you may be asking for more problems.
|
|
RESPONSES/COMMENTS (CLINICAL) - PART 1b |
From: Andrew Levy, DPM, Jon Purdy, DPM
I don't think I would characterize the fracture as more than minimally displaced, if at all. I also don't think I would be in a hurry to put a screw across that fracture. Have you made sure she is not smoking or around cigarette smokers and is Buddy splint wrapping it each day with Coban to the 4th toe? I would give it more time with the Coban wrapping. In my practice, I teach the patients to wrap it one way on day one, and then the opposite way the next day, and then have them demonstrate it to me at a one week check-up. If she really wanted to accelerate the healing, consider a bone growth simulator.
I had a similar case recently. I took the gentleman to surgery within a week of the injury and performed a simple arthroplasty procedure. Even if fixation could be accomplished, it seemed silly to attempt when comparing the healing times. This gentleman was back in a boot and at work in two weeks.

RESPONSES/COMMENTS (CLINICAL) - PART 2 |
From: Bryan C. Markinson, DPM
Dr. Udell posts a query regarding proper work-up of skin cancer. First of all, the original case reported was of a homebound patient where the DPM performed a biopsy and wanted advice on topical treatment for squamous carcinoma. The imagery of that DPM getting an answer in this forum and then initiating treatment caused me to post a word of caution. In my experience, which is hospital-based, all skin cancer patients, regardless of type, get a full body skin examination.
If the lesion in question has any serious potential for metastasis, there would be an oncology referral to determine any evidence of metastasis before definitive treatment of the specific lesion is begun. It takes an oncologist (or any one trained in physical diagnosis) five minutes to palpate the lymph nodes. Then, depending on the oncology physical exam and pathology of the initial biopsy, the oncologist may decide to order x-rays and/or PET scans to check for any sign of mets.
In my opinion, it is critical to do this before wide excision is done, which by the way would include a simultaneous sentinel node biopsy, if dictated by the pathology report. Although metastatic squamous carcinoma is not common, it can be deadly. Initial biopsy and wide excision of lesions is in the domain of the podiatrist, but the staging and full body skin exams must involve other specialists, and not be left out simply because the wide excision is technically easy.

RE: Security Risk Analysis (Thomas Fitzgerald, DPM)
From: Joseph Borreggine, DPM, Ray Posa, MBA
This should be done every single reporting period for meaningful use when you attest and answer "Yes" to the required core objective. You must have proof that you did this security risk analysis. This is also required for HIPAA.
Joseph Borreggine, DPM, Charleston, IL, footfixr@consolidated.net
Dr. Fitzgerald, asks a good question and in light of all of the audits going on, it is a timely question. If one reads the CMS website regarding this, one of their recommendations is that you use a professional outside organization to do this work.
Unlike the Privacy portion of HIPAA, the Security portion gets into technical areas that are beyond what most staff members can do in a credible way. There is a threshold being used by the government when it comes to your compliance efforts, and that is - if you have a credible effort. That means, ...

RE: Residency Transfer Positions
Residency directors, I am a second year resident who is looking to transfer into a third year position at a different residency program this upcoming year. Last year, there were multiple second and third-year residency positions that were being advertised on casprcrip.org/. This year, there appear to be no positions advertised. If you are a residency director with an open position, please post your open position(s) on the casprcrip.org/ website so that qualified applicants can apply and a mutually beneficial relationship can be established.

RESPONSES/COMMENTS (IN THE COURTS) |
From: Bryan C. Markinson, DPM
To all my colleagues who are DME and/or diabetic shoe providers: Yesterday, in PM News, a brief synopsis of a case in Ohio involving CMS and a DPM was posted. I went to the HHS web site. The full text of the decision is at this link. I am posting this to make everyone aware of how ridiculous this case is, and I will do all I can to get APMA to respond to this outrageous and discriminatory technicality that can happen to anyone. Read it and see what he government has in store for doctors.

RESPONSES/COMMENTS (NEWS STORIES) |
From: Don Peacock, DPM
I think it’s off-putting that a company like Vibram is under attack and is obligated to pay out a large settlement in a class-action suit. I use my Vibram Five Finger running shoes in my daily walk of 30 to 60 minutes. I also sprint 8 to 10 times for 30 seconds once a week wearing these minimalistic shoes.
I experienced some soreness when I first began wearing these, but I have no pain at all with exercising in these shoes after the break-in period. At first, the foot is strengthening and...
