Women Will Wear Heels Until Their Feet are Bloody Stumps: MD Podiatrist 
 
Heels are thought to be the cause of much of the roughly $3.5 billion that women in the United States spend each year on foot surgeries. The desire to wear them comfortably has led some women to get botulism—Botox—injected into their feet: Dr. Scholl meeting Dr. Frankenstein. That same desire is the source of “Haglund’s deformity,” a painful condition that has been given the revealing nickname of “pump bump,” and also of bunions and hammertoes and many more extremely preventable podiatric ailments.

FDA: Avoid Cipro, Similar Drugs for Common Infections
 
Fluoroquinolone antibiotics, the class that includes ciprofloxacin (Cipro), should not be used to treat uncomplicated infections such as sinusitis, bronchitis, and urinary tract infections for which other drug types are effective, the FDA announced Thursday. Only patients with these conditions who can't be treated with other agents should receive fluoroquinolones, the agency said.
 
"An FDA safety review has shown that fluoroquinolones when used systemically (i.e., tablets, capsules, and injectable) are associated with disabling and potentially permanent serious side effects that can occur together. These side effects can involve the tendons, muscles, joints, nerves, and central nervous system," the FDA said.

IN THE COURTS
House Republicans Win Affordable Care Act Lawsuit
 
In a major win for House Republicans, a federal judge ruled Thursday that the Obama administration inappropriately spent money to pay for part of the Affordable Care Act. The Obama administration is expected to appeal the decision, and the judge said she would stay her ruling pending an appeal.
 
House Republicans had alleged that the administration illegally spent money that Congress never appropriated for the ACA's cost-sharing provisions. Those provisions include reduced deductibles, copayments, and co-insurance many Americans receive, depending on income, for plans purchased through the ACA's insurance exchanges. U.S. District Judge Rosemary Collyer wrote Thursday that the ACA “unambiguously” appropriates money for tax credits under the law but not for cost-sharing reimbursements to insurers.

Query: Medicare Denial for Neuropathy
 
We have recently been getting Medicare denials for CPT 64450 with G60.9 (hereditary and idiopathic neuropathy, unspecified) for non-diabetic neuropathy. Is there a better injection code or diagnosis code to use for neuropathy?

Response: First, check to see if your MAC has an LCD for CPT 64450 which would list the allowable codes. Regardless, the doctor should avoid unspecified codes, if possible. The second consideration centers on medical necessity. Why are the injections performed? Is the anesthetic injection for pain management or some type of therapeutic injection to treat a systemic, not local "neuropathy"? Is this injection curative or palliative? Are there studies that prove this injection to be effective in the long run (especially in an audit or malpractice situation)? Is there a proximal nerve entrapment resulting in local symptoms or a local problem such as a neuroma? 
 
Many payers have policies regarding peripheral nerve blocks for neuropathy or nerve pains. It is not unusual to have them label the injections investigational.

Kudos to Dr. Christina and APMA for their efforts to move podiatry into the era of value-based podiatric care. Nonetheless, because of antitrust and other prohibitory reasons, APMA cannot unite its members into negotiating groups at the local level where network inclusion or exclusion of podiatrists occurs. This networking is up to podiatrists themselves to create locally, and if they are not already looking for ways to do that, sadly in my opinion, that is valuable time lost.

RE: CMS and the Downfall of Western Civilization (Robert Kornfeld, DPM)

Kudos to you Dr. Kornfeld. You hit the nail on the head. I urge all of my colleagues to read his post on this subject. After reading it, reach deep down in your soul and muster up some courage to make changes in your practice and in your life. Quite frankly, we have become a profession living in fear of insurance regulations and abuse. I attend my division meetings and the core of the discussion is centered around how to get paid for cutting a mycotic nail. Ridiculous. 

Years ago, we cared about learning and becoming better doctors. All everyone does is complain about practice and the insulting fees insurance companies trickle out. Well, either do something about it or stop complaining. Please drop out of every insurance plan that abuses you on a daily basis. It takes testicular fortitude to make important changes in your life, but the positive effect of those changes are well worth it. Remember, you can see all the patients you want if you want to see them for free. If there's one important thing I learned over the last 30 years in practice, it's that volume does not equal income generation. Let's stop all the podiatric neuroses and take a stand. Be proud of what you've accomplished and do something about it.
