Ultraviolet Shoe Sanitizers Reduce Fungal Recurrence: MD Podiatrist
The Claim: Ultraviolet sanitizers can rid your shoes of infection-causing fungi and eliminate bacteria that make them smell bad.
The Verdict: One device (the SteriShoe) has been found in a published study to reduce the levels of fungus in test pairs of athletic shoes and leather shoes. Scientists say further research is needed, particularly trials with humans showing that the devices have an impact in keeping feet free of infections when used over time. Despite a paucity of published data, podiatrists say the devices make a lot of sense, especially when used to prevent recurrence of infection after treatment.
|
Dr. Mark Spier |
“We know that the toe box of the shoe is the breeding ground for fungus, especially in the warmer weather,” when your feet are sweating, says Dr. Mark Spier, a Reisterstown, MD., podiatrist who sells the devices in his office to patients who had laser treatments for infected toenails. While Dr. Spier hasn’t done a study, he says he has noticed that recurrence of infection after laser treatments is higher in those who don’t buy the device, or who stop using it.

The American Health Care Professionals (AHCP) and Friends for Medicine in Israel (APF) has just released the details for a podiatric educational CME program in Israel. "This will be the first time in over twenty five years that there has been an educational program or mission to Israel for podiatric physicians in Israel," says Dr. Marlene Reid, co-chair of the program. PODIATRISTS AND THE LAW |
MO Podiatrist Admits to Defrauding Almost $1 Million from Medicare
Local podiatrist Dr. Lawrence B. Iken will be spending a year behind bars for his part in a Medicare scheme that stole almost one million dollars in fake healthcare claims. Iken pled guilty to charges involving the submission of false documents and reimbursement claims related to podiatry services he purportedly provided from 2006 through July 2014. With his plea, Dr. Iken admitted that, on thousands of occasions, he billed Medicare, Medicaid, and private insurance companies for the incision and drainage of abscesses and hematomas when he actually only clipped the toenails of the patients.

HEALTHCARE LEGISLATION |
Bill to Repeal IPAB Gaining Momentum
Now that Congress has slain the dragon called the sustainable growth rate (SGR) formula, momentum is building to repeal yet another Medicare cost-control mechanism that is loathed by most of organized medicine: the Independent Payment Advisory Board (IPAB). There are now enough votes in the House to pass a bill striking down this board, created by the Affordable Care Act (ACA). The Senate is considering an identical measure sponsored by Sen. John Cornyn (R-TX), which has 38 co-sponsors so far.
Republicans and many medical societies view the IPAB as a collection of unaccountable bureaucrats who will ration care and cut physician reimbursement just as the SGR formula threatened to do. Some political conservatives go so far as to call the IPAB a "death panel," even though the law explicitly prohibits the board from proposing anything that would ration healthcare or restrict benefits. IPAB backers say the board will be more effective in controlling Medicare spending than a Congress under the spell of industry lobbyists.

PRACTICE MANAGEMENT TIP OF OF THE DAY |
Tips to Collect More from Patients - Part 2
2. Write a Script
Calling patients is considered the most effective technique to recover debts. To help staff become more comfortable with this, provide them with a script. Consider including the followiing:
-
Identify the patient and the caller.
-
Request payment in full. If not possible, ask if a payment plan might work.
-
Document what the patient says.
-
Once you reach an agreement, make sure the patient understands that if he doesn't honor his commitment or follow his plan, he may be sent to collections without warning.
-
Document the agreement.

RESPONSES/COMMENTS (EMR) - PART 1A |
From: Ira M. Baum, DPM
EMR is a sound concept in modern healthcare technology. Unfortunately, developers of EMR do not have systems that are user-friendly. There is a disconnect between the developer-user (physician)-legal system. Use of templates helps bridge the gap but comes into question on "what the physician actually did." Some physicians have hired "scribes" at an additional expense to avoid use of templates. Proponents of EMR have a good argument for its use and opponents do as well. I think we all know who will win in the end. EMR is here to stay.
Providers of EMR will need to evolve. Private companies will need to be more economically feasible, and cloud-based "free" systems will need to develop improved practice management (PM) options. I used a cloud-based "free" system and paid the company as an insurance clearinghouse only. There were no annual fees, the support was as good as any, and their software constantly evolved based on consensus need requested by physicians. Practice management could have been more detailed, but for me was more than adequate. It's a bitter pill at first, but you will learn to tolerate it.

RESPONSES/COMMENTS (EMR) - PART 1B |
From: Alan Meyerberg, DPM
EMR is here to stay isn't a good enough reason for me to utilize it. At first it sounded like an improvement to health delivery. I actually got on board, especially when the government was helping defray the cost and my hospital also shared in the expense. Electronic transmission of prescriptions and the ability to retrieve the prescription history of a patient instead of relying on their memory makes a great deal of sense. The ability to share the results of various tests will reduce the frequency of duplicate test ordering. No more sloppy handwriting. Well, has everyone discovered the real reasons for jamming this monster down our throats?
First, after getting the $44,000 bribe, you can count on an audit that will result in...

RESPONSES/COMMENTS (EMR) - PART 1C |
From: Ray Posa, MBA
Dr. Mauriello is exactly correct, there are no free lunches and EMR is no exception. Currently, there are several manufacturers who offer “free” EMR software. So, having been in the technology field for over 3 decades, let me explain how this “free” software model works. There have been many software products over the years that were “free”, and then one day they become fee-for-use or they are acquired by a rival company.
What software companies try to do is build a large user base; this gives the product and the company value. When the user base hits a desired value, the company can then...
