Men are Not as Attentive to Feet as Women are: NY Podiatrist   
 
It’s no secret that, as a species, men are not as attentive to their feet as women are. “Most men who come to my practice have waited until they have little choice,” Suzanne Levine, DPM, a board-certified podiatric foot surgeon and aesthetic podiatrist at New York City’s Millennium Podiatry tells InsideHook.

“Although cliches are frowned upon, they certainly apply here. An ounce of prevention is worth a pound of cure,” says Levine. For example, you might think you can push through the pain of an ingrown toenail, but it’s better to treat it early before it gets infected. A stress fracture or shin splints left untreated can progress to outright fractures and more prolonged treatments, while fungal nails can spread to other nails and to other family members, Levine explains.

Bahamas Podiatrist Discusses Relationship Between Back Pain and Foot Function  
 
According to podiatrist Monique Mitchell, "When you pronate, or roll your feet in toward the heels, you tend to lean forward, which predisposes you to more low back pain and aggravates any previous back injury. When walking or standing, rotating your feet to the outside often eases intermittent low back pain because it rotates your hips backward and shifts your center of gravity. Walking this way may help the back but it is not good for the feet and may cause foot pain and sprained ankles. Standing in a more upright posture will also give your back muscles a break. But, the best way to properly reduce the pressure on the spine and back muscles is to correct and balance your feet."

"Since the feet are the foundation of the body, shoes play a big role in making sure that this foundation is stable, ensuring that all joints work well together and are pain-free. Wearing poorly constructed shoes or shoes that are not for your foot type can cause a significant amount of foot and maybe even back pain. Shoes that can cause back pain are extremely high heels, especially the pencil-thin ones that don’t offer much support," says Mitchell.

Technologies Practices are Leveraging to Deliver Better Patient Care - Part 8
 
Here are technologies and their importance for practices in today's care delivery system.
 
8. Patient engagement solutions
 
While many of the technologies identified above encourage greater patient engagement in and control over their care, there is a growing group of solutions focused on accomplishing this increasingly important component of a patient's care journey. For example, patient portals provide patients with easy access to their health information and can include a host of other features such as the ability to request referrals and prescription refills, view test results, make payments, and read educational materials. Scheduling software gives patients more control over when patients meet with a provider and potentially how that interaction occurs (i.e., in-person visit or telehealth).
 
Recall solutions inform patients about recommended follow-up appointments and routine services such as annual wellness visits and diabetic foot exams. Solutions like text messaging and chat give patients more control over how they communicate with caregivers. These and the many other types of patient engagement solutions are not only helping empower patients to improve their health and wellness, but they are also helping practices adapt to value-based payment models.

Query: Needing a Primary Code
 
United Health Care is denying a claim for CPT 11103 stating that I need a “primary code.” Do you have any information on this subject?

Response: You billed for CPT 11103 Tangential biopsy of skin (e.g., shave, scoop, saucerize, curette); each separate/additional lesion (List separately in addition to code for primary procedure). The key verbiage here is "list separate" in addition to the primary procedure. This tells you that you must bill for the primary site for which you did the biopsy using CPT 11102. I am assuming that you either chose this CPT in error or you were supposed to bill for CPT 11102 and also then bill using CPT 11103 for each additional biopsy site. This is similar to CPT 11730 (nail avulsion) and CPT 11732 (each additional nail avulsed).
 
To put it simply, bill for the initial site you biopsied using CPT 11102 (that would be the primary code) and then for each additional site biopsied using CPT 11103.

I read with some interest the AMA resolution regarding a path to a plenary DPM license. While I empathize with those that might have frustrations in the past regarding the evolution of our specialty and our degree, I disagree with those that have impugned the outcome of this draft resolution. I see this as the next logical step in our evolution. The fact of the matter is this: Our degree should define us (our training, our background, our area of specialty), but it should not confine us. In other words, the silly things that have stood in our way over the past two or three generations that my father and many of our collective mentors fought for will yield fruit that my oldest daughter, now a rising third year podiatric medical student, will not even have to think about. That is progress.

Doctor Jacobs' observations about the AMA resolution are spot on.  In reading the white paper produced by the Joint Task Force of Orthopaedic and Podiatric Surgeons, it is clearly stated three times that currently, DPMs do NOT meet the common standard established by MDs to provide medical and surgical care.
 
In fact, this paper infers that patients as well as referring healthcare professionals do not have the "same high degree of confidence" that DPMs have met this common standard and DPMs will now need to pass all 3 parts of the USMLE in order to gain that confidence. How this document could have been endorsed by the APMA and ACFAS is confounding.

Regarding the USMLE step 1, 2, and 3 examination. My company has done review courses for USMLE step 1,2, and 3 for the past 18 years. I have personally taught medicine to allopathic, osteopathic, and podiatric medical students for over 25 years. I have written medicine questions for the podiatry boards part 2. The exam questions in the internal medicine course I taught at Scholl College were all based on USMLE-type questions and the students performed well. With that being stated, I can unequivocally state that with the proper curriculum, the right instructors, and a review course, a podiatric medical student can pass the USMLE step 1 and 2 examinations.

My son is a DO student and his school requires him to take the USMLE exam in addition to the DO exam. This is a stress they don't need and a financial burden that is unnecessary. They do it to "prove their parity" to a body that does not care. Everything the Project 2000 experiment has tried has only hurt our profession. Podiatry should quit trying to be "transmedical". If you want to be an MD, be an MD. Leave podiatry to the rest of us who want to be podiatrists. Thank you Dr. Jacobs for the solid information on the issue.
