CA Podiatrist Discusses Gout
 
If you have an enlarged, painful big toe, it could be from what you ate. Did you gorge on wine and steak? The painful aftermath could be gout, a type of arthritis that usually affects the joint of the big toe. Foods high in purine, a chemical compound found in red meats, fish, and certain alcohol, can trigger an attack by raising levels of uric acid in the body. “You’ll see the deposition of the uric acid in the joint, most commonly the big toe or the ankle,” says Bob Baravarian, DPM, a podiatric foot and ankle specialist at Providence Saint John's Health Center in Santa Monica, California.

“The patient will wake up with a hard, red, swollen joint. It’s extremely painful,” says Dr. Baravarian A doctor may prescribe anti-inflammatory drugs for short-term relief and suggest a low-purine diet for long-term prevention.

OH Podiatrist Cautions Against Summerlong Nail Polish 
 
Don't leave nail polish on all summer. "The chemicals and dyes in nail polish can discolor or crack nails. People often mistake this for a fungal infection," says Dr. Lauren Kishman. "Don't leave polish on for extended periods of time – remove after about two weeks and give your toes a break before re-applying."

Look for polishes with fewer chemicals or even try anti-fungal polishes. If you're not sure if it is fungus, see a podiatrist. Getting a pedicure? Dr. Kishman cautions to make sure the salon where you go sterilizes their equipment.

PM JURY VERDICT REPORTER
Alleged Damage Due to Steroid Injections
 
Facts: An adult female presenting with right foot pain, reportedly suffered flexible hammertoe/deformity with significant instability in the second metatarsal phlangeal (MTP) joint of the right foot, and chronic pain when defendant, a podiatrist, who had diagnosed capsulitis in the second metatarsal phlangal joint of the right foot, injected a steroid three times over a period of 27 days, despite increased pain in the joint after each injection.
 
The plaintiff ultimately suffered scarring and pain, primarily on the top of the second MTP joint, a knot over the proximal phalanx, the inability to stand or walk for a lengthy period of time, the inability to engage in running, and continued pain around the joint when she later elected to have forefoot reconstruction surgery involving a second hammertoe correction with a PT tendon transfer by an orthopedist who advised that the surgery would not make her foot normal and would not guarantee that she could go back to running.
 
The plaintiff contended the defendant breached the standard of care by injecting the plaintiff within a limited time period. The defendant denied liability and contended that the injections did not cause her injuries or necessitate her subsequent surgery. Jurors returned a verdict in favor of the defendant.

APMA Calls on Congress to Repeal Independent Payment Advisory Board
 
The American Podiatric Medical Association (APMA) has joined over 500 national and state-based organizations, representing virtually every state in the union, in signing a letter to Congress calling upon lawmakers to repeal the Independent Payment Advisory Board (IPAB) provision of the Affordable Care Act. IPAB would impair Medicare beneficiaries’ access to care and remove public accountability from the Medicare decision-making process.
 
IPAB is a board comprised of executive branch appointees, which would be charged with making cuts to Medicare expenditures if program spending reaches a certain level established by statute. Once the Secretary of Health and Human Services implements the IPAB recommendations, the action is not subject to administrative or judicial review. If IPAB members are not appointed by the President, or IPAB does not act to cut Medicare spending within the law’s required timeframe, the board’s powers are automatically shifted to the HHS Secretary.

SOLS Aims to Make Othotics Cool
 
Old-school orthotics are custom-made after a podiatrist takes a plaster cast of a patient’s foot. At SOLS, customers visit one of the company's showrooms or a participating podiatrist, where a technician takes three sets of photos of both feet to create a digital model of the suffering tootsies; a 3-D printer does the rest. The podiatrists can bill the insurance using the same codes as with other orthotic devices.

Some podiatrists approve of SOLS to relieve foot pain. Dr. Andrew Glass of Manhattan uses them himself and prescribes them – but cautioned that orthotics can make shoes too tight. As such, he recommends always seeing a trained professional before getting a pair of orthotics.

I have done an extensor lenghtening along with a Y to V skin plasty and have had good outcomes. However, the patients I've done this on have had a substantial large extensor tendon. The point is, I wouldn't try this on a patient with a small/medium foot.

RE: EMR is Here to Stay

Like it or not, EMR is here to stay. If you are not utilizing it now, very soon you will be forced (in how long, is debatable). That being said, Web-based EMR (so called “free” EMR) must be considered with a great deal of care. When EMR software is being sold for 30K, 40K, 50K and above, what makes you think that a company will give something away for free? This is something they have spent a great deal of money developing. They get paid in a variety of ways, but they get paid nonetheless. Whether this cost is upfront, deferred, or if it’s imposed on labs or imaging companies, it still exists. Frankly, if it costs a few grand to get records in a workable format, then so be it.  Nothing is really free, is it?
 
Additionally, I believe it is better to do your homework; find an EMR that suits your needs (which admittedly, can be difficult) and implement it now in a controlled way. Paper charts are no longer the standard, like it or not. Just think how much it will cost your practice if we are all forced to convert by 2016? Admittedly, this is a hypothetical, but certainly a possibility. How many “free” EMRs will exist then? If you think it’s expensive now, wait until we are mandated to use EMR. Get that checkbook ready.

MD Woman Put Dead Skin from Feet in Milk, Says Report
 
A St. Mary's County woman is accused of serving contaminated milk to her family members, according to charging documents obtained by Southern Maryland News Net. According to the media report, 56-year-old Sarah P. Schrock of Mechanicsville is accused of putting shavings of dead skin from her feet in the milk at her home. Schrock was arrested on May 6 and charged with poisoning/contaminating of food, three counts of assault, and failure to comply with a peace order, says Southern Maryland News Net.
