Working While Standing Harmful to Feet: South African Podiatrist

Hemingway did it. Churchill did it. So, let's do it. Let's all stand up while we work. Not so fast, says podiatrist Gina Badenhorst, secretary of the Podiatry Association of South Africa. "Long periods of standing in professions such as hairdressers and teachers create problems," Badenhorst explains. "The continuous upright posture causes sore feet, legs, and backs."

Gina Badenhorst

Medically, sitting is not good for you: numerous studies, including one published in February by the University of Western Sydney in Australia, have linked very elevated risk levels for a variety of serious medical conditions to sitting for more than four hours a day, regardless of lifestyle. "Never stand still in one place for long periods of time," she cautions. "One needs to move around to get the calf muscles working and blood to be pumped back up to the heart."

PODIATRISTS IN FICTIONAL LITERATURE

Army Podiatrist is Protagonist in New Novel

Lisa Scottoline's newest novel, "Don't Go," is a thriller that starts with a woman swimming in blood on her kitchen floor. Her husband, Army podiatrist, Dr. Mike Scanlon, comes home to find that the orderly life he thought he was returning to with his newborn daughter is spiraling out of control. The plot twists, and secrets are uncovered as Mike discovers his friends and associates may or may not be worthy of his trust. "I actually found a doctor who had served as an Army surgeon in Afghanistan and picked his brain relentlessly, as well as having had him read the manuscript to correct any of the errors. I also read almost everything written on the subject of the war, both fiction and nonfiction," said Scottoline.

Lisa Scottline (Photo: Daniel Burke Photography)

"That said, the podiatry research came about in a much more mundane way — I had bunion surgery! I learned a lot about what it's like to be a podiatrist, as well as how they are viewed in the profession. I instantly liked the idea that although podiatry wasn't necessarily a prestigious specialty on the homefront, the tables would be turned in Afghanistan and Iraq, where so many of the wounds were tragically the result of IEDs and injuries to the lower extremities. Mike Scanlon is the beating heart of this book, and the fact that he's a podiatrist says everything about him — he's smart, hard-working, and completely dedicated, but he feels like a second banana. He has to overcome that, and everything he goes through in the novel actually ends up being a blessing," said Scottoline.

PODIATRISTS AND THE LAW

Lawyer Arrested in Alleged Assault of NY Podiatrist in Family Court

Patrick Bisogno, 49, a Staten Island attorney working to settle a child-support case was arrested after allegedly assaulting the father, who is his former brother-in-law, police said. Bisogno is accused of punching Dr. John Libertella, 49, of Valley Stream, LI, in the nose in the altercation.

Libertella, 49, told the Advance that the altercation occurred in an elevator after Bisogno hurled insults at him. Libertella, who was with his father at the time, said Bisogno called him a "deadbeat dad" and said he would never see his daughter again. Libertella said he was treated at Richmond University Medical Center and released.

RE: Overlapping 2nd Digit Status Post Bunionectomy (Don Peacock, DPM)
From: Randall Brower, DPM, Jeff Kittay, DPM

A Lapidus with a Keller would be perfect. Look what happened with less.

Randall Brower, DPM, Avondale, AZ, footdoctor33@yahoo.com

In Dr. Peacock's presentation of his MIS "bunionectomy and Akin," the post-op projection is clearly supinated when compared to the pre-op, giving the suggestions of a more rectus hallux and reduction of the IM angle. The procedures performed appear to be well done, but a true AP angle and base of gait post-op x-ray would provide us a more accurate view of the correction achieved.

RE: Dragon Problem (Craig Breslauer, DPM)
From: Marc Garfield, DPM

Check the Dragon website for compatibility with your newer computer’s soundcard, or consider buying an external sound card approved by Nuance. Note that not everything they approve is necessarily the best.  But If they do not review it, it may not work well.  support.nuance.com/compatibility/Search.asp?PRO=DNS&CID=10

RE: Dragon Problem (Craig Breslauer, DPM)
From: Brad Makimaa, DPM

We have 8 offices and 6 doctors, and countless computers, all with EMR and Dragon. We have had numerous problems with Dragon (we thought  they were Dragon's fault). The bottom line is that the computer's operating system has to be happy with your EMR and both have to be happy with Dragon.

We usually wait to upgrade to the latest Windows as most everything is not compatible with that as it first comes out. Check with your EMR vendor and with Dragon for compatibility issues and recommendations. This fixed nearly all issues. Some minor tweaks in settings are easily addressed with Dragon's help. We currently have no issues with the new Dragon in all computers, and it is faster than before.

RE: Biomechanics of Barefoot Running
From: Jefferson J. Mennuti, DPM

I have been thumbing through some of the recent literature about the above subject. I think the reality of this debate is not, in fact, barefoot running per se. I believe the real issue is biomechanics; specifically forefoot running vs. heel striking. So, I would like to shift this thought process from "barefoot running" to "forefoot vs. heel strike running."

In my vast history with running (5 weeks), ...

RE: Dr. Lidtke's Remarks (Bret Ribotsky, DPM)
From: Mr. Jeffrey Root

Dr. Ribotsky wrote, “Last Tuesday, we were exposed to a theory that stability can be bad (sorry, Dr. Root) to forces and to orthotic therapy when dealing with arthritis and joint wear.” Although I didn’t listen to last Tuesday’s Meet the Masters discussion, I can tell you that Dr. Root believed that functional orthoses could be used to do far more than reduce or eliminate excessive motion.

While it is true that functional orthoses can be used to enhance stability, they can also be used to...

RE: Dr. Lidtke's Remarks (Roy H. Lidtke, DPM)
From: George Trachtenberg MS, DPM

I do not recall your making the website ncbi.nlm.nih.gov/pubmed available in your posting in PM News for the readership to refer to. I recall you only made a statement that would amount to an abstract of your conclusion. Also, I never expressed any issue that the shoe in question in your study reduced loads on the knee up to 20%. I simply pointed out that practitioners should be aware that the load in a closed kinetic chain can have an effect on another body region if not dissipated external to the body.

I have not gone to the website above to see if you demonstrate what happens to that load when you...
