TN Podiatrist Finds Practice Extremely Rewarding

While Webster’s refers to podiatry as the medical care and treatment of the human foot, others might refer to it as, “heaven.” Foot problems are nothing new, dating back to the beginning of time. Luckily for Nashville, Dr. Berkeley Nicholls is available to fix all the ailments a patient has to offer. Nicholls graduated Magnum cum laude with a Bachelor of Science degree he obtained in London, England. With an engineering background under his belt, he decided to switch to medicine. He attended the New York College of Podiatric Medicine and after completing his residency in New York, moved to Nashville in 1989.

Univ of Utah Hospital Restricts Podiatry Privileges

The University of Utah is limiting the practice of podiatrists in its medical facilities, severely restricting their ability to perform surgeries and ending their existing adjunct faculty appointments. University Health Care officials say the changes and transfer of oversight from the department of surgery to the department of orthopaedics are important for the U.'s educational and treatment mission.

Podiatrists, including one who has practiced and performed surgery at the U. for a quarter-century, describe the move as a poorly executed turf battle over surgery patients. The motive is not money, they say, but feet on which to train future orthopaedic surgeons.

Podiatrists would have to agree not to operate on U. patients at non-U. facilities, too. "Basically, that means that podiatrists will be able to come up and clip toenails for them and take care of painful calluses — the kind of thing they refuse to do anyway," says Dr. Terry Smith, who has practiced podiatry at the U. part-time since 1980. What was not provided, according to Utah Podiatric Medical Association President Dave Edwards, was an appeals process.

Sexual Battery Trial Begins for CA Podiatrist

Eight women are expected to testify in Fresno County Superior Court that podiatrist Dr. Matthew Sciaroni touched them, in some cases under their bras and panties, when they saw him for problems with their feet. Sciaroni's trial began Friday with Deputy District Attorney Galen Rutiaga saying that Fresno podiatrist Sciaroni, 52, "used his position as a doctor to touch, grope, grab, hug and kiss his patients." The incidents happened between about 1999 and 2006 and the youngest patient was 12 at the time, said Rutiaga. Sciaroni was arrested in April 2007 on charges of lewd and lascivious acts with a minor, sexual battery and child molestation.

Defense attorney Steven Smith said testimony will show that in one case Sciaroni did "a breast exam that was mischaracterized as a breast grab" by one patient. "There are two sides to every story," Smith said. Evidence during the trial "will back up every medical procedure he did," Smith told the jury.

Dr. Leonard Greenwald, a San Jose podiatrist and consultant for the state Board of Podiatric Medicine, testified Friday afternoon that he reviewed a complaint against Sciaroni by one of the women, who said he touched her breasts. "I came to the opinion that the standard of care was not met; it was well below," Greenwald said after reviewing the complaint. Greenwald said he has never checked a woman's breasts for the presence of an infection that may have spread from the foot.

QUERIES (CLINICAL)

Query: Bilateral Tarsal Tunnel Syndrome

My patient is a 48 y/o female with bilateral heel pain. Patient had tarsal tunnel syndrome diagnosed via NCV/EMG four years ago. She was only symptomatic on the right. The right side was released at that point. She had a year of relief on the right. The left heel and foot has developed pain and the right has returned. She has a positive Tinel's bilaterally with exam. NCV/EMG has been repeated and PM&R physician stated the right conduction times are better than original NCV four years ago, but the left is the same. The patient was given options of repeat surgery versus neural stimulator. She opted for repeat surgery bilaterally. Should Integra Neural Wrap be used on the right side only, or should it also be used on left side since it tested abnormal four years ago and is only now showing symptoms?

RE: Traumeel for Capsulitis/Plantar Plate Injuries (Neil Levin, DPM)

In response to Dr. Kass's post, it is a huge misconception to believe that a single homeopathic injection should be equal in relieving symptoms to a cortisone shot or NSAIDs. It is not designed to relieve symptoms and therefore, should not be compared in that way. Where homeopathic injections shine is in the fact that they enable cellular healing over the course of time. Multiple injections are needed along with a comprehensive support protocol. The patient's subjective report is reliable to base the therapy on because the symptoms are not suppressed. I implore everyone to please be well informed about its use before making any snap decisions on its efficacy. I have had amazing success with it as well as other homeopathic injectables.

As far as I am aware, any of these homeopathic medications have no clinical evidence whatsoever to show that it is any more effective beyond placebo effects. I understand that all the European countries with socialized medicine are dropping their prescription coverages on homeopathic medications. In this day and age of evidence-based medicine, why are podiatrists injecting their patients with "snake oil"?

Our office started selling Rx Crocs about one-and-a-half years ago. We placed them on a shoe tree in the waiting area and for the first couple of months we dispensed about three to five pairs per day. Sales have slowed down now, likely because everyone who would buy the shoes have already purchased theirs, or people have finally looked down at their feet and said to themselves, "Hey...these things look stupid!" Regardless, we only dispense a few pairs per week at this point.

I have been selling Crocs Rx for over a year with good success. It is not a big money maker but once you get past the original cost of inventory they do sell themselves in most cases. I also recommend them for my diabetics as an alternative to going barefoot in their homes. I will caution you, however, that state regulations will require you to get a retail sales tax ID number and you will have to file and pay sales tax on these items. It takes a few months to get that set up and Crocs will not send your 1st order until you have that tax number. Also, they are not 100% honest in the claim "only available" through doctors’ offices, as they sell them on their own website to the public and I have seen them for sale in the FootSmart catalogue. I just keep my prices lower and the consumer saves on shipping fees.
