Having a DPM on DFU Team Reduces Amputations by 36%: MN Podiatrist
 
According to Stephanie Kvas, DPM, "Two complications of diabetes are nerve damage and restricted blood flow to the legs and feet. Both increase the risk of various foot complications. Nerve damage decreases the feeling in your feet, a condition called peripheral neuropathy. Reduced blood flow to the feet makes it harder to heal an injury or resist infection. Left untreated, even minor cuts and blisters can become serious infections that ultimately may require toe, foot, or leg amputation."

First Lapidus Wedge with an FDA Lengthening Reference, Launched
 
Walter Strash, DPM of San Antonio, TX was part of the Nvision design team creating the first Lapidus wedge with an FDA lengthening reference. According to Nvision’s Senior Vice President of Product Development, Tom Zink, “Most Lapidus procedures shorten the first ray to some extent. The Trigon Lapidus Wedge System restores the natural anatomy which restores the natural function of the first ray. The ability to restore the natural length can be beneficial in revision cases and also in a primary fusion because it prevents possible secondary issues due to the loss of length.” The Trigon Lapidus Wedge will allow triplanar correction of hallux valgus deformity promoting proper alignment of the first ray.

5) Integration
 
You may need to establish an integration team entirely dedicated to executing and implementing the merger. There will be many major changes for the employees from both practices, and you need a strong and well-executed integration plan to ensure the transition goes smoothly. There is a great deal to think about with regard to how you will integrate your new identity and culture and you need managers who can focus on this while others focus on continuing the normal focus of your business. Do not execute any closing documents until you have first prepared an integration plan.

My patient returned to my office for painful onychocryptosis. I performed bilateral, temporary nail procedures at the medial border of the hallux nail. Do I bill this as CPT 11730 with a -50 modifier or CPT 11730 T5 and CPT 11730 TA?

Response: The best approach and most accurate when doing additional nails is the CPT 11730 and CPT 11732 for additional/add on. I do recommend the digital modifier as some payers demand it, although some do not. You do NOT need a -59 with add codes, so remember that. The add-on codes are designed when doing the same procedure but in a different location or expanding the surface area treated.

RE: Retronychia, a Rarely Diagnosed Nail Disorder

A presentation at the annual meeting of the 2022 American Academy of Dermatology on retronychia, a nail disorder of the toes, was made by Phoebe Rich, MD, director of the Nail Disorders Clinic at Oregon Health & Science University, Portland. The disorder was described as the growth of new nail under prior nail growth. Dr. Rich states that antifungals and antibiotics, even for years, have no place in treating this condition since infection is not commonly involved in retronychia. Microtrauma typically serves as a trigger to retronychia. Dancing, high heels, steel toed shoes and other sources of trauma have been implicated. Some but not all patients have abnormal function of the nail bed, according to Rich, increasing susceptibility to retronychia. Injury separates the existing nail from the matrix and nail bed so that newly formed nail begins to grow under the nail, pushing out the old nail.

Even when the nail is removed and regrowth appears normal at the end of a year, patients with very short nails cannot count on a cure. Slow nail growth may result in recurrence in 2-3 years. Rich remarks that there is no diagnosis by histopathology, so it cannot be confirmed by biopsy. Conservative treatments, including topical and intralesional steroids, have been effective in mild cases to more invasive and effective surgical avulsion. She states that only 50% of yellow thickened nails are onychomycosis, resulting in a plethora of patients with retronychia who are misdiagnosed.

Studies demonstrate that only 51% of physicians would recommend medicine to their children as a career. Only 20% of medical school students have a parent who is a physician. A highly motivated and hardworking podiatrist can have a rewarding career in podiatry. There is no question that success is more difficult to achieve in podiatry than it is in allopathic medicine. In my opinion, and to my observation, the APMA has been of little benefit in advancing our education or positioning within the medical community. 
 
Individual success in podiatry is almost totally dependent on individual effort at the local level. “Parity” as you discuss, can be achieved by your individual demonstration of knowledge and care, resulting in your own acceptance as an equal within the medical community. What the APMA has yet to appreciate is that you will not “legislate” podiatry into MD/DO parity. The recent USMLE debacle serves to reinforce this fact. When you are trusted with referrals of patients from other healthcare providers, you have achieved parity.

I applaud Dr. Kornfeld’s personal journey. I did not have the guts to do what he did and accepted Medicare. I always made a living .However, those 1978 fees adjusted for inflation are the same as 2022. My son could easily have gone into medicine but is in the business world. None of my friends' children chose medicine. My personal physician is leaving the big group as more and more patients are pushed on him. My excellent dermatologist looks tired and harried as the big group pushes him to do more. None of their children have gone into medicine and both sets are husband and wife MDs.
