OH Podiatrist Discusses How to Buy Running Shoes
 
Mark Mendeszoon, DPM and the owner of the Achilles Running Shop stores in northwestern Pennsylvania and northeastern Ohio, spoke to the Running Revolution women about the importance of wearing the right shoes for walking and running. He said his life is about running, as he ran in college, retired, and then returned to the sport.

"The majority of runners are in the wrong shoes," he said. As a retail shoe store owner and a doctor, he said, "You can see me now, or see me later." Thick heels also force "heel strike", where the heel strikes the ground first. He said runners from Africa who grew up running in bare feet have fewer foot and ankle problems, and they are fast. But changing from thick heels to bare feet causes tendons, bones, and muscles to respond in a way that can be harmful.

NJ Podiatry Guild Representatives Meet with NJ Senate President
 
A delegation from the New Jersey Podiatric Medical Society, including Drs. Jeffery Katz, President, Harvey Karpo, George Nassoor, Herman Hammerschmidt (Executive Director), and Ms. Laurie Clark, NJPMS Legislative Agent met on May 5th with NJ Senate President Stephen Sweeney in his office at the State House in Trenton.

This private meeting centered around current New Jersey healthcare issues and how they have affected both the patient and the practicing podiatric physician. Dr. Nassoor, the Guild representative, commented that this important face-to-face with the Senate President was a direct result of the assistance of the AFL-CIO Legislative staff and membership in the Guild. "We believe our issues and concerns... will be given serious consideration, with future meetings planned."

TX Podiatrist Uses 3-D Printer to Develop Inexpensive Pediatric Orthotics 
 
The idea of using a 3-D printer for pediatric orthotics was presented by a small group of students from the Department of Family and Community Medicine at Texas Tech University Health Sciences Center School of Medicine at the Permian Basin at the 25th Annual Permian Basin Research Day Tuesday. Dr. Jenet George and co-authors Drs. Brian Tong, Tara Deaver, and Erik Wilkinson, regional library director, worked on the orthotics research to develop orthotics to help children with flat fleet. Deaver, a podiatrist who is on the faculty at the health sciences center, was the faculty advisor.

“When we found out that there was a 3-D printer, I asked Mr. Wilkinson if we could try and reproduce a pediatric orthotic that would be cheaper. That’s how it all got started,” Deaver said. “I know they’re doing a lot of other things in medicine with 3-D printing. If this is something we can help out locally, then we’re all for it. That’s the whole goal.” Deaver said the main reason behind the research is the local patient population. “We see a lot of Medicare, Medicaid, indigent patients, and self-pay, so we’re looking at cheaper options for parents for their children," Deaver said.

Cuban Ambassador Leads Delegation to AZ Podiatrist's Clinic and Labs 
 
A delegation led by Jose R. Cabañas, Cuban Ambassador to the USA, spent two days in meetings, discussions, and events focusing on the Southern Arizona Limb Salvage Alliance (SALSA) and the Arizona Center for Accelerated Biomedical Innovation.

The visit included discussions of a novel growth factor that may aid in healing diabetic foot wounds, developed by a team led by Jorge Berlanga Acosta, a colleague of Dr. David G. Armstrong in Cuba. Marvin Slepian, Professor of Medicine and Director of ACABI said that “We see this visit as the next step in a strong bilateral relationship between our teams at SALSA and ACABI."

A recent visit to a veterinarian prompts this question: Are we able to charge biohazard material disposal fees?

Editor's comment: PM News does does provide legal advice. This practice seems to be spreading among veterinarians, with fees ranging from $3-$7. We know of no rule that prevents doctors from charging this as a non-covered service. However, unless this practice is widely adapted in the medical profession, at the very least, this would be a good way to lose patients.

APMA staff and member leaders are in regular contact with CMS, and we remain a staunch champion for our members and their interests, just as we were during the implementation of ICD-10. In the wake of the MACRA proposed rule, APMA has created a dedicated MACRA/Quality Payment Program information page for members at www.apma.org/macra. Also be sure to reserve your seat now for the MACRA Proposed Rule: A Look at MIPS webinar on May 24 at 8 p.m. EDT.
 
The Health Policy and Practice Committee will meet at APMA headquarters later this week to discuss numerous issues that affect APMA members’ bottom line, including MACRA, MIPS, APMs, and the future of healthcare reimbursement. During that meeting, the committee will explore the creation of a MACRA Task Force as well as the possibility of a summit to discuss potential APM models. APMA also is preparing formal comments to CMS in response to the proposed MACRA rule.
 
I encourage you to watch the APMA website and your Weekly Focus for outcomes from the meetings, as well as more information about MACRA and reimbursement issues in general. In the meantime, continue to count on APMA as your voice on Capitol Hill and at CMS.

I have read the recent threads regarding the end of private solo practice, CMS, and the downfall of western civilization, etc. It is patently clear that most of us recognize that what you must put into a medical practice has significantly increased and what you get to take out of it has significantly decreased. 
 
As a 1980 graduate of NYCPM, I have been around long enough to have seen dramatic changes in Medicare and health insurance reimbursements. And these changes have been ongoing for at least 30 years (the first Medicare fee freeze I believe was in 1985). Suffice it to say there has been a spate of new guidelines and administrative requirements imposed on...

RE: AZ Bill Restoring Podiatric Care in Medicaid

After a sustained effort by members of the Arizona podiatric medical and vascular surgery community along with support from academics and healthcare economists, House Bill 2704 passed both the Arizona House and Senate and was signed into law yesterday by Governor Doug Ducey.
 
It is heartening to see so many people from the medical and lay community come together to support this data-driven effort to educate our colleagues in the House and Senate. When they saw how podiatric care can preserve mobility, limbs, and resources, they were able to come to a bipartisan consensus that this care helps people.  A study conducted by the SALSA (authored by me), and co-authors vascular surgeon Joseph Mills and economist Grant Skrepnek was widely considered to be instrumental in the effort to educate. The study, entitled “Foot in Wallet Disease” found that, following the repeal of podiatric care from Arizona Medicaid (also known as AHCCCS), hospitalizations increased by more than 37%, length of stay by 23%, per patient hospital charges by 45% and severe aggregate outcomes (sepsis, amputation, death) by 50%.
 
Patients on Medicaid should now soon be able to receive urgent and preventative services from podiatric physicians.

"Hill’s bill would require providers to notify patients in writing if they are facing disciplinary actions by the medical board, osteopathic board, podiatry board, acupuncture board, or Board of Chiropractic Examiners for serious offenses that could compromise their ability to practice competently."
 
Really? The Board that decides who gets licensed and who does not is on the fence about a proposed law that would, in effect, cause any physician who might be facing disciplinary action from his/her board to write to each patient and inform them of this action? Even if this disciplinary action turns out to have no merit, or results in a minor reprimand, or some required CME, the doctors are required to publicize their probation and thereby, in effect, end their relationships with all their patients and close their doors. How soon would you schedule an appointment with a doctor who has informed you that he/she is facing disciplinary action by his/her licensing board? Are not the actions of the medical boards matters of public record, so that patients can look up on state websites if their doctor is the subject of punitive disciplinary action and then make their choice as whether to attend this particular doctor or not? Will the doctors then be required to again notify each patient when their disciplinary punishment has been fulfilled and their period of probation finished or will the board, in its omnipotent wisdom, so inform the public it purports to protect?  
 
What utter nonsense that the very board overseeing these actions has no opinion on a proposed law that could potentially cause some of its members to lose their livelihoods. Much better to stick their heads in the sand.
