IA Podiatrist Discusses Bunions
According to the American Podiatric Medical Association (APMA), a bunion is a bump on the joint at the base of the big toe. “Bunions are a fairly common foot problem in women who wear narrow and high-heeled shoes,” podiatrist Dr. Mark Beers said. “The joint can become very tender and swollen, which can make wearing shoes difficult or even impossible.”

Bunions are mainly hereditary, but can be caused by foot injuries and other medical issues. Another common culprit is shoes that are too tight and squeeze the toes together. “If you are experiencing redness, swelling or pain near the joint of your big toe, or notice a bump that is firm along the side of your foot, you may have a bunion,” shared Dr. Beers. “If left untreated, this condition becomes worse over time.”

Foot and ankle surgeon Dr. Nicholas Giovinco, in collaboration with the hacker space Freeside Atlanta, is using CT scans to create life-size 3-D printed templates of complex, three dimensional deformities. This approach to surgery increases patient outcome and care, while decreasing surgeon stress. Giovinco works with other surgeons to implement 3-D printing in a variety of medical fields.

TX Podiatrist Represents Medical School at Faculty Advisory Council
George T. Liu, DPM, Assistant Professor of Orthopaedic Surgery, represented University of Texas (UT) Southwestern Medical School at the recent UT System Faculty Advisory Council in Austin, Texas. The Council is comprised of past presidents of the Faculty Senate of 9 academic and 6 medical schools of the UT system. Dr. Liu also served as a member of the Health Affairs Committee.

The Council met with the Chancellor and Vice Chancellor of the UT as well as the Texas commissioner of higher education. The mission of the Council is to provide a forum for communication between faculty, the Board of Regents, and the Executive Officers of the UT System.

Rogers is critical of the Affordable Care Act, but wants to repair rather than repeal it. His experience as a doctor and in dealing with health insurance companies has given him practical ideas on how to fix problems with the law and the healthcare system in general. That's why we prefer Lee Rogers, a doctor and moderate Democrat from Simi Valley."

Query: Can Candida and Cimetidine be Used Together?
I have used both Candida injections and cimetidine in the past for wart treatment, but never together. Is there any potential problem using both together, such as having the cimetidine, in theory, "cancel out" the response I am trying to achieve with the Candida injection?

This past year, the wife of one of my patients died from a squamous cell carcinoma that metastasized internally. It's rare for this to happen but tell that to her survivors. With this in mind, it would be best to have the patient admitted to a hospital and have the lesion excised.

My advice is to use 7-Zip, a free utility which comes with 256 bit encryption. It is usually pre-loaded on new computers (or you can download it for free from a myriad of safe websites). Ask your vendor for a recommendation. Simply encrypt the document and then provide it to the patient with a password.
Send the password under separate cover, or better yet, provide it to the patient verbally prior to sending the email. Once they receive the document, when they go to unzip it, they will be asked for the password. Once entered, the document can be opened and saved by the patient in any fashion they want.

I started using 10% NaOH about 10 years ago. It is faster to use than phenol. I apply it three times for 15 seconds each. Like phenol, each of us seems to have a favorite time to apply it. I have also read about one application for one minute. There are no real set guidelines. I see about the same number or regrowths with both chemicals. The advantages are, to my mind:
1. NaOH is shipped in a smaller bottle. If it crystalizes, or goes bad, or seems to be losing its potency, it is inexpensive to discard the old bottle and open a new one. In addition, I don’t have to pay any hazardous shipping charges for it.
2. It truly neutralizes with 6% acetic acid (grocery store white vinegar) rather than being lavaged out, but not neutralized, by alcohol. In real practical terms, I cannot state this really makes any difference, but intellectually, it just makes me feel better.
3. I believe any inadvertent spills of phenol are far more dangerous than the same volume of NaOH due to the ease of neutralization of the NaOH to a salt and H20 with acetic acid.
4. I do not see a major difference in the rate of healing with the use of the two chemicals.

After thousands of NaOH procedures over a 30 year period of time, I have seen very little regrowth. I think it is a much better procedure than phenol - better control and consistent, predictable results. One caveat - I never try to do the procedure on an infected nail. The acidic pH present during an infection neutralizes the effect of the NaOH cautery. Clear-up the infection before using the NaOH.
