New NYCPM Symposium Targets Fourth Year Students
 
On April 21, NYCPM started something new. The College offered fourth-year students a “Senior Life Skills Symposium,” its first offering as a formal program about life after podiatry school, residency, and beyond. About 50 senior students heard from speakers in a variety of fields that are of utmost interest to residents and all post-graduate podiatrists starting out in the profession.

Among the speakers were Ross Taubman, DPM, CEO of PICA, who spoke about Medical Malpractice Insurance for Residents; Arthur Klein, MD, who spoke about the state of healthcare in 2023; Roland Ramdass, DPM, who spoke about residents’ pathway to board certification; Allen Jacobs, DPM, who spoke about what a resident needs to know about vendors and suppliers; and Matthew Feldman, JD, who spoke about HIPAA and regulatory and ethical issues for residents. Other topics discussed included burnout, financial planning, disability insurance, and hospital credentialing.

GA Podiatrist Discusses Diabetic Foot Swelling
 
According to Richard Kaufman, DPM, "The most common cause of diabetic foot swelling is poor blood sugar control. When the body does not have enough insulin to break down sugars, it triggers fluid retention and edema. High blood pressure can also cause diabetes-related foot swelling by preventing the kidneys from properly filtering out excess fluids from the body."

"Treating diabetes-related foot swelling should begin with improving one’s diabetes control. Tight glycemic control and regular exercise are often advised for people with diabetes, as these can help to reduce fluid retention in the ankles and feet. In addition to lifestyle changes, certain medications may be prescribed by a doctor to help manage foot swelling. This can include diuretics or ACE inhibitors, which reduce fluid buildup and improve circulation in the ankles and feet," says Dr. Kaufman.

UK Podiatrist Discusses Prevention of Metatarsal Stress Injuries
 
Because they take much of the impact on foot strike, the metatarsals can easily become strained, inflamed, or fractured. "The symptoms of a metatarsal stress injury can vary depending on the severity of the injury," says Siobhan McCutcheon, but a stress injury is usually evident by increasing pain on movement, "particularly during walking, running, or jumping." Other symptoms include: swelling of the metatarsals or tenderness to the touch, tingling, or numbness (also felt in the toes) and red or bruised skin in the affected area.

Strengthening the glutes, quadriceps, calves, as well as improving your core stability, will aid prevention. You should also focus on strengthening the toe flexors, plantar fasica, and peroneus muscles in the lower leg (the fibularis longus and fibularis brevis), which will help to protect the metatarsals.

TX Podiatrist Describes What Sandals Those with Flat Feet Should Wear
 
You may be itching to shop for new sandals and flip-flops, but if you have flat feet, finding a sandal that’s actually supportive and stylish can be challenging. Chanel J. Perkins, DPM, a board-certified podiatrist based in Texas, told HuffPost that without the proper foot support, flat feet can become painful due to muscle and ligament fatigue, stress, and strain, and may eventually cause conditions like plantar fasciitis and muscle tendinitis.

“The best sandal for people with flat feet is one that has an inherent arch support,” she said. “It’s best to avoid flimsy sandals that can easily fold in half. Flat feet need sandals with rigidity and firmness throughout the insole.” These include wedge sandals and sandals with adjustable straps around the ankle and across the top that provide support for flat feet.

Top Six Ways to Increase Patient Visits
 
Patient visits have a direct impact on the success of your independent practice. Here are six tips from the experts to put into action.
 
1. Focus on Your Schedule - All too often, practices create barriers in their appointment schedule, with slots reserved for certain purposes or patients being cancelled and rescheduled willy-nilly, according to author and consultant Elizabeth Woodcock. Your scheduler is your director of new business development; develop a schedule with slots for "short" and "long" appointments, easing the ability to appoint patients. If "pre-designated" slots are not filled with the intended patients with 72 hours to go, open the slot up to any patient.
 
Remember, physician time is your practice's most precious asset. Accept same-day appointments; overbook the 10 a.m. and 2 p.m. slots to account for no-shows. Most importantly, don't ever bump patients, or they will bump you.

My documentation for qualified, routine foot care is similar for most patients as little changes in 3 months. But, yes, I do look for changes and make the note reflect such. And I try to make sure notes are not exact copies of previous notes. My notes have plenty of bullet points for an E/M code on each visit. I am primarily charging procedure codes only (CPT 11056, CPT 11721, etc).
 
Can I still bill at least one E/M code maybe per year even when there are no substantive changes as I continually monitor vascular, neurologic and dermal changes each visit? Must I have a new or different diagnosis to bill an E/M code?

Response: Medicare does not pay for “screening services” unless specifically stated in the CMS regulations. Doing such an yearly exam just because they are diabetic, would constitute a screening examination. And feeling for pulses or even a quick filament check would fall under that “E/M” that is included in every procedure’s allowance.
 
Now if the patient complained of increasing burning, spreading of the numbness, feet feeling colder, calf or foot pain that you might think is ischemic in origin, then that could warrant a closer workup, plan and therefore an E/M service. This I  think would be a more infrequent event. If a patient has “changes” every two months or on a yearly basis every January, that could be difficult to defend.
