ACFAS Announces New JFAS Editor
 
The American College of Foot and Ankle Surgeons (ACFAS) has announced that Naohiro Shibuya, DPM, MS, FACFAS is the new Editor-in-Chief of the Journal of Foot & Ankle Surgery (JFAS). Dr. Shibuya stepped into the editor role officially May 1 and will oversee the editorial content and management of the Journal taking over for the retiring D. Scot Malay, DPM, MSCE, FACFAS.

As Dr. Shibuya steps into the role, he hopes to encourage high-quality submissions by expanding resources for authors, editors, and reviewers to improve submission efficiency and to re-establish policies and guides for authors.

NY Podiatrist Discussses Stress Fractures
 
Older adults are particularly vulnerable to stress fractures, or tiny cracks in the bone, because the trabeculae (the spongy tissue that connects your body’s bones) thin with age. The most common symptom is pain with activity that subsides with rest. “We saw a lot of stress fractures throughout the pandemic. People were barefoot more because they worked from home, and they gained weight, which together is not a good combination,” says Jacqueline Sutera, DPM, a podiatrist in New York City.

It’s important to realize that the symptoms of a stress fracture are more subtle than those you’d experience with an actual broken bone. In fact, a lot of people don’t even realize they have one, Sutera says. “There’s usually not much swelling or bruising, they don’t remember hurting themselves, and they usually can still walk, albeit with pain,” she explains. If pain lasts for more than a few days and doesn’t seem to get better with ice and rest, see your doctor.

VA Podiatrist and Former Soccer Goalkeeper Create Fall Prevention Videos
 
Ben Pearl, DPM and former DC United goalkeeper Andrew Dykstra have teamed up to create a series of fall prevention videos. Dykstra retired after a series of concussions. He now has a fitness consulting company and coaches soccer. Balance exercises are important in concussion protocols too. Dykstra and Pearl used a soccer field as a backdrop to demonstrate exercises that can help with stability and proprioception for both athletes and non-athletes. Dr. Pearl put barefoot science in perspective. The fast transmission signals are helpful but it is essential to develop the trunk and gluteal stability with the contact and edging that occurs in sports. Dykstra demonstrated pelvic stability exercises for upper level athletes and non-athletes. Pearl emphasized the need for consistency and to try to work exercises into your daily life.

The presentation was titled “Economic Solutions for the Upcoming Healthcare Changes”.“You cannot state that the current practice of medicine is good or bad by consensus; you must apply economic facts and then see the reality of medical practice today and the future,” stated Hrywnak.

None of us like EMRs, but it is what we have. I have had great luck with the Athena platform. It has amazing metric measuring tools to help with negotiations with payers, has easy-to-use and adjustable macros, and the billing platform is stellar. No, it is not pure podiatric, but the payers are recognizing it as the premiere program for E/M record-keeping and tracking of practice metrics. It is the wave of the future for value added care and where the DPM sits in that world.

I would like to congratulate Dr. Wolf for his courage and perseverance regarding taking his case to the ALJ. Every case (and I was selective) that I took as a practitioner to the ALJ, I won. My first battle was over 30 years ago, when my Medicare contractor, Empire BC/BS, refused to pay me for the insertion of a tissue expander. This device, typically used for breast reconstruction, was used on a diabetic patient who after reconstructive surgery, had a wound which could not be closed by typical means.
 
At that time, we had no cellular tissue products and other conventional means failed to close the wound. After several weeks with the tissue expander, the wound was closed and remained so for the duration of the patient's life several decades later. All I needed was several photos to...

Thanks, Dr. Jacobs, for those stats. This is so classic in that therapies that could have some benefits in very specific circumstances are destroyed and become not payable for over- utilization purely based on the mighty dollar. This will happen to graft codes soon enough, putting thousands of dollars of quality products on ulcers that are not properly off-loaded, compressed, debrided, etc., and would never heal until a met head is excised or vascular status maximized, or compression therapy initiated, to allow for healing.
 
Wonder why they want to limit applications now? Keep using them 8 times on your patients who walk in the door in their normal shoes, carrying their knee scooter, or holding their crutches/walker, swearing they ‘hardly’ walk on it. Why is PRP or amniotic injections considered experimental and...

While we were busy becoming better surgeons so we could feel more like “real” doctors, medicine collapsed and burned in a series of increasingly restrictive payment guidelines from Medicare and insurance companies.
