Inspecting Feet for "Toe Jam" has Clinical Benefits: NY Podiatrist
 
It may seem a little strange to regularly inspect your feet for toe jam, but doing so could help you in a surprisingly vital way. "Being aware of toe jam has an additional clinical benefit," podiatrist Rock Positano, DPM says. "One of the deadliest forms of cancer, malignant melanoma, quite often will appear in the foot first, mainly on the heel, but also in between the toes, which is a common hiding place for this aggressive cancer." So while you're doing your daily between-the-toes cleaning session, keep an eye out for any marks and moles you haven't seen before.

Ultimately, if efforts to keep toe jam at bay means you're paying more attention to your feet, that's a very good thing. "People should be more aware of the importance of foot health," Dr. Positano says. "The feet are the wheels of the body and are often the best indicator of systemic disease. Many vital systems of the body [like the skin, nervous, muscular, circulatory, and endocrine] are found in the foot."

UK Podiatrist Discusses Care of Toenails 
 
According to Alan Ward, podiatrist, “There are three big risks to healthy nails – poor blood supply, often seen in smokers or diabetics, fungal infections and psoriasis, which will often cause pitted, poor quality, “crumbly” nails. In general, regular saltwater bathing and a simple aqueous cream on the skin – but not between toes – should be sufficient.”

Often cutting nails too short or down the side of the nail is a bigger problem than nails that are left too long. In general, nails should be left long enough for a small shoulder, or free edge at the distal end of the nail. Nails should be cut to follow the nail shape and filed so as not to have any sharp edges that can cause impaction areas to adjoining toes.

TX Podiatrist Receives Georgetown DLS Award
 
Over 650 healthcare practitioners gathered in April in Washington, DC for the Diabetic Limb Salvage 2018 conference (DLS). Hosted by MedStar Georgetown University Hospital, DLS attracted participants from 24 countries and 44 states for the three-day conference that examined the multi-disciplinary approach to the management and treatment of the diabetic foot.

The Georgetown University Hospital Distinguished Achievement Award was presented to Lawrence Lavery, DPM, MPH for his groundbreaking research in diabetic foot care. Dr. Lavery is an internationally recognized expert in diabetic foot care and has written and spoken frequently on such topics as diabetic foot ulcers, soft tissue and bone infections, Charcot arthropathy, and prevention strategies. By combining research with scientific treatment options, he has forged the way for novel and evidenced-based treatment options.

RELEVANT RESEARCH
Study Compares Ankle Fracture Fixation by Orthopaedic Surgeons Vs Podiatrists
 
This was a retrospective cohort study of data extracted from the Humana Claims database for 15,067 patients who underwent ankle fracture fixation between 2007 and 2015. 14,222 patients with ankle fractures were treated by orthopaedic surgeons and 845 patients were treated by podiatrists. Operative treatment by orthopaedists was associated with lower DVT (p<0.02) and malunion (p<0.02) rates among all types of ankle fractures. There were no differences in complications for patients with single malleolar fractures, although higher rates of implant removal (p<0.01) were noted in patients treated by orthopaedic surgeons.
 
For bimalleolar or trimalleolar fractures, treatment by an orthopaedic surgeon was associated with lower DVT (p<0.03) and malunion (p<0.04) rates. No differences were observed in rates of infection or need for irrigation and debridement based on surgeon type. Patients treated by orthopaedic surgeons versus podiatrists were also noted to have a similar median CCI at 2.0 and 2.5, respectively.

Common Mistakes of Physician Group Mergers - Part 2

Acquiring another practice can be a powerful tool for your group to achieve growth and build long-term value. There are, however, a few key mistakes that are often made which could be avoided with more thoughtful reflection.

2. Forgetting to ask, “What more could we become together?” - The single biggest pitfall that derails successful transactions occurs during the actual integration process. Those involved often focus too heavily on ensuring the tactical aspects of the deal are covered, including technology integration, financial reporting, operations, and merging organizational structures.
 
The real power of any physician group merger comes from both practices challenging each other to ask, “What more could we become together?” How can each practice learn from the best of each respective practice and let go of old biases? This will build the two parts into a better whole. Viewing the integration process through this lens will help build collective urgency and alignment around shared goals and generate excitement among employees for the new entity’s future.

Query: Diagnosis Codes
 
A diabetic presents with cellulitis and a newly formed blood blister, plantar aspect of the right foot. (12mm in diameter) The office documented E/M code 99213-25 and CPT code 10140-RT for the encounter and procedure; ICD10 code L03.115 for E/M code and S90.821A for CPT code. Is a secondary diagnosis code needed for the CPT code or is this correct for submission to Medicare?

Response: This coding looks good to me. The only other thing to consider would be to use (S90.31XA) – Contusion of right foot for the “blood blister” diagnosis code if this was more of a hematoma and depending on its etiology. Otherwise, with a -25 modifier on the E/M be sure the E/M was separately identifiable and that this is supported by the documentation and medical necessity. The E/M service for the cellulitis should be able to stand on its own as a service separate from the I&D.

A patient came in with a swollen erythematous distal 2nd toe. Last night, her cell phone fell vertically onto her toe. There is small avulsion/chip that was sheared off the lateral condyle of the base of the distal phalanx of the 2nd toe.

Newsday, our local paper carried a two-page article this weekend on this topic. It said that gabapentin is the eighth most prescribed medication in the country and for good reason. It is safe, non-addictive and is used either alone or in combination with certain antidepressants to manage millions of people suffering with neuropathic pain.
 
I have never had a case of a patient abusing gabapentin nor becoming addictive to this medication. To the contrary, this medication has been a "lifesaver" for so many of my patients. By no means does it have the potential for abuse associated with opioids.
 
If gabapentin is now in the crosshairs of the government, will Lyrica and Cymbalta be next? What will millions of patients suffering with neuropathic pain do if gabapentin and similar medications are restricted?
