Foot Surgery by PA Podiatrist Restores Woman's Independence
After dealing with debilitating foot pain for years, Robin Freeman visited podiatrist John Dresser, DPM. MRI scans revealed Freeman had significant arthritic changes throughout her foot related to a longstanding flat foot deformity. Although she could manage the pain with a walking boot and cortisone shots, surgery was another option. She decided to move forward with the surgery.

The surgery required five incisions, multiple rods to fuse the damage, and more than two dozen stitches. After surgery, she had a 10-week recovery in which she couldn't initially put any pressure on her foot. Almost two years after her surgery, Freeman is pain-free. She gets stiff sometimes if she sits for too long, and she has balance problems if she walks barefoot. But she is feeling much better now, and she's back to driving, walking, shopping, and other activities.

According to Nicole Freels, DPM, a podiatrist based in Lexington, KY, there are many different factors that affect how quickly a person wears out a pair of shoes—including a runner’s weight, gait, and the shoe itself. “There’s no system to calculate all these variables and spit out a number.”

Dr. Freels recommends the twist test, a more useful way to gauge if sneakers are ready for retirement. “Take the heel with one hand, then take the toe in the other and twist it,” she said. If there’s any bend in the midfoot or arch area, very few runners could use the shoes long-term without pain, she explained. You want that area to feel stiff and firm.

I agree with Dr. Udell's advice that any of our colleagues thinking about transitioning to "direct pay" discuss this with their accountants and family members before taking on the risk. I also suggest that if the main reason for transitioning to direct pay is eliminating the burden of dealing with insurance claims hassles, give careful consideration to outsourcing all coding, documentation, billing, and practice marketing functions to one of the many excellent, full-service revenue management companies that are expert in podiatric business management. This can eliminate the stress and usually results in maximal return on investment.

I just read Dr. Udell’s post and would like to add some context. From my perspective, only about 5 to 10 percent of podiatrists are cut out to be direct care practitioners. It requires tremendous effort and will to succeed. One must think long-term to be successful. When I started, I took an 80% income cut and it took years to catch up to the old insurance days, but day by day things progressed until I had a very successful direct care practice. I say that this type of practice is not for the faint at heart but in the end, it is well worth the investment if you have the will to succeed.

The recent thread in this forum on the future of podiatry has been extremely interesting and thought-provoking, with the most recent emphasis on the pros and cons of direct pay versus the more typical insurance dominated practice model. Let's not lose our historical perspective on this. Back in the 1960s, with the advent of Medicare, everything in healthcare changed. Prior to that, virtually all practices were direct pay, and the only insurance coverage was Major Medical, sometimes referred to simply as "hospitalization." When Medicare came around, our profession clamored to be included.
There are colorful stories of the behind closed doors antics that went on inside politicians' offices to assure that podiatric services would be covered. Since then, any time there has been a change, such as the emergence of managed care, we, along with every other medical group have done everything possible to keep ourselves included. And by being included in this payment model, we have been able to expand our status within...
