OH Podiatrist Discusses Tailor's Bunions
Tailor's unions rarely cause worrisome complications unless they go without being treated. If bunions continue without treatment, Mark Mendeszoon, DPM says there are a few possible complications, including inflammation of the joint that connects the small toe to the foot and inflamed bursal tissue which protects the fifth metatarsal bone from irritation from your shoe. If the deformity is significant, your pinky toe might evolve into a hammertoe, with a callus or painful bursa.

If you need surgery, you'll undergo weight-bearing radiographs, which will help your doctor determine the extent of the deformity. "The surgical correction may be a simple procedure that removes the bump on the side of the fifth metatarsal head or a procedure that requires a bone cut to correct the angular deformity," says Dr. Mendeszoon.

End of an Era: MA Podiatrist Retires
The era of doctors making house calls has long been a thing of the past. It appears that the era of the neighborhood doctor may also be coming to an end as well. After 35 years, Belmont podiatrist Dave Alper is retiring. Dr. Alper is well known in Belmont for more than just his foot care. Alper’s education was comprehensive and geographically broad. He studied in New York, San Francisco, and Ohio. He did an internship in Maryland, a residency in Kansas City, and a fellowship in Jerusalem.

Alper began his practice in nearby Somerville. No stranger to hard work, he simultaneously tended bar in the evenings, so he could save up money to open his own office. Alper will continue as a trustee of the APMA Board, also the board of the American Diabetes Association, as a Belmont Town Meeting member, and as an adjunct professor at Northeastern University.

This past month, the Monmouth County MRC was recognized both nationally and locally. Monmouth County MRC received honorable mention for COVID-19 Vaccination Support at the 2021 MRC Program Recognition Awards on April 21, 2021.

During National Volunteer Week, April 18-24, Monmouth County Commissioner, Deputy Director Sue Kiley recognized Hal Ornstein, DPM as one of the top medical volunteers. Since January, Dr. Ornstein has volunteered over 200 hours.

ON THE INTERNATIONAL LECTURE CURCUIT |
MO Podiatrist Co-Moderates Virtual International Academic Session
Podiatric residents and their director H. John Visser, DPM from SSM DePaul Hospital in St. Louis, MO took part in a virtual international academic session with orthopedic residents, a fellow, and orthopedist Ralph Springfeld, MD from Hamburg, Germany.

During the session, residents presented surgical cases and shared their approach to common and complex foot pathology. Topics ranged from Charcot foot reconstruction to diabetic foot infections to revision foot and ankle surgery. The session was moderated by both Drs. Visser and Springfeld.

Technologies Practices are Leveraging to Deliver Better Patient Care - Part 5
Here are technologies and their importance for practices in today's care delivery system.
5. Kiosks
Practices are turning to a variety of self-service kiosks for a multitude of patient care and engagement-related activities. Patient check-in kiosks allow patients or their caregivers to check in and complete required documentation while informing staff that patients have arrived for appointments. Thermal kiosks check the temperature of staff, patients, and visitors while non-thermal kiosks ask staff, patients, and visitors a series of health screening questions. Some kiosks include video capability that allows staff to visually interact with kiosk users. Others include a credit card slot so patients can make payments. Expect to see the usage of kiosks—by patients, staff, and visitors, including vendors—to become more commonplace in practices and many other areas of healthcare.

RESPONSES/COMMENTS (APMA NEWS) - PART 1A |

Congratulations and kudos to APMA, ACFAS, AAOS, and AOFAS and their Joint Task Force for the recent announcement of a white paper and AMA resolution whose goal is to outline a pathway by which podiatric surgeons can be defined as physicians within their scope of practice. The passage of their resolution by AMA and the subsequent ability of graduates of podiatric medical school to take and pass the USMLE will enable state podiatric associations to begin the process of having DPMs granted an unrestricted, plenary license.
For more than a decade, the California Podiatric Medical Association has been working tirelessly for true parity between DPMs and our MD and DO colleagues. Our own Task Force established a similar pathway for a plenary license with our orthopedic and medical colleagues in California. This announcement is another step towards the goal of changing our DPM license to a plenary license while allowing us to keep our unique perspective and identity as podiatrists. We are now one step closer to making DPM=MD=DO a reality. CPMA calls on all APMA members and our schools of podiatric medicine to support this effort.

I just read an email from APMA containing a copy of a white paper agreement between the MD world and the DPM world. In essence, it says that if a DPM takes and passes the United States Medical Licensure Examinations (USMLE) and all of our “training programs” meet the MD/DO Standards, they will recognize us at physicians with limited licenses. WOW - what incredible negotiations that our leadership engaged in. It's crystal clear that if we jump through all the requirements and rules of any foreign medical school graduate, we would still be a DPM with limited license with additional recognition from the big boys in medicine, while foreign medical students who pass the same criteria would be licensed as MDs. What did the orthopedic surgeons give up during this negotiation? We validated that we are NOT physicians at this time.
After reading this white paper, why are we still trying the keep the DPM degree? If this proposal goes through at the AMA meeting, and we clearly MATCH the training of MDs, we will still be subservient to the MDs.

The appointment of John Gouin and other VA podiatrists to roles as Chiefs of Surgery, Assistant Chiefs of Surgery, Associate Chief of Staff for Education and other leadership positions, highlights the progress the profession has made in the last several years. We can all take pride in Dr. Gouin and the others as we have continuously fought for improvements on two fronts - first our educational system, the colleges of podiatric medicine, and the residency and fellowship programs that we have improved over the years. During my career, the profession has constantly leveled its criticism of the education system, which is a necessary part of continuous quality improvement.

Any time that a surgeon makes a comment like “it allows for a faster healing time” typically means that up against a more traditional approach to the “same procedure”, the procedure being touted is the better of the two for the reasons listed. Unfortunately, in too many cases, this is mis-information being touted as either for monetary gain (either paid by the manufacturer, OR direct marketing to consumer) or it is simply because the surgeon has “drunk the Kool-Aid”.
The Lapiplasty procedure, of which I have done several, is a place looking for a home for well-designed surgical instrumentation. I am not speaking against the concept, nor the instrumentation in terms of its function. It works well. It does, however, add substantial cost to the procedure. Does one NEED to use it - well I see it similar to riding a bike with training wheels, or skiing with poles. It may be helpful during the initial phase to learn the Lapidus technique, but in NO WAY do patients heals faster and return to shoes and activities faster with this technique vs. the more traditional Lapidus procedure.
