PODIATRISTS IN THE COMMUNITY
CT Podiatrists Volunteer at Foot Screening for the Homeless

More than 260 homeless children and adults in Hartford are now enjoying a rare comfort - high-quality, well-fitting shoes and new socks - after attending a foot screening clinic at Holy Trinity Church. The event was part of a new program, "Footwear with Care", that is providing urgently needed shoes and foot care to Hartford's homeless community.

(L-R) Drs.Gabriel Gambardella, Meredith Ward, Eric Kosofsky, Lynn LeBlanc, Kurt Rode, and Rafael Gonzalez

The initiative is a partnership of the Connecticut Podiatric Medical Association, Fleet Feet of West Hartford, the non-profit Community Partners in Action, and the Hartford Police Department. At a recent clinic, Dr. Lynn LeBlanc and other podiatrists affilated with St. Francis Medical Center examined the feet of Hartford's most vulnerable population for hours, checking for critical foot issues and making sure that new shoes fit well.

PODIATRIC PRODUCTS IN THE NEWS
FL Podiatrist Teams with Teen Inventor to Form 3D Printed Orthotics Company

When Marco Fernandez realized that commercial insoles weren't helping his flat-feet problems, he decided to work on a more customized insole product -- using 3D scanning and printing techniques. Marco, who's 16 and a junior at Miami-Dade County's School for Advanced Studies at Miami Dade College Wolfson Campus, has suffered from a flat-foot problem all his life.

(L-R) Dr. Richard Santo Mallia and Marco Fernandez

He connected with Dr. Richard Santo Mallia, a podiatrist with a master's in public health from FIU. The two shared their ideas and decided to join forces to create Digifeet, a company that connects podiatrists, designers, and patients to create custom insoles. Marco, with Mallia's mentorship, then compiled the idea into a business plan and submitted it to the Miami Herald Business Plan Challenge High School Track, which attracted 104 entries this year -- and he won.

MEDICAL EDUCATION
U.S. Medical School Enrollment Up 25% Since 2002

For the 2015-2016 academic year, medical school enrollment in the United States is up 25% over 2002-2003 levels, representing 4,143 new students, with greater growth forecast, according to the latest figures from the Association of American Medical Colleges (AAMC).

The results of the 2015 Medical School Enrollment Survey were released May 5 at the AAMC's Health Workforce Research Conference in Chicago, Illinois. The survey of deans at U.S. medical schools examines first-year medical school enrollment over the past decade and projects first-year enrollment through 2025.

RESPONSES/COMMENTS (CLINICAL)
RE: An Interesting Case

This patient presented four weeks after resolution of an acute psoriatic flair which was treated with a tapered steroid for nine days. This was a bilateral presentation currently involving the feet with no hand involvement.

Post-psoriasis skin reaction

This patient is not on any disease modifying meds and only takes hydrochlorothiazide for controlled HTN. Basically, the majority of the more superficial layers of the plantar epidermis is peeling in a large single sheet.

RESPONSES/COMMENTS (CLINICAL)
From: Daniel Chaskin, DPM

It is important to understand when not to biopsy. As a part of the history, ask patients if there are any color changes or any temperature sensitivity. If not, then document "no allodynia" and proceed with the punch biopsy.

It is more difficult in cases where there are color changes or temperature sensitivity. In these cases, my suggestion is to document that the patient understands the risks of RSDS or CRPS and then to only proceed with the punch biopsy after a full dermatologic history regarding skin cancer or family history of skin cancer, and after the use of a dermatoscope and a clinical exam.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: CMS and the Downfall of Western Civilization
From: Neil H Hecht, DPM

CMS has estimated that 85% of small medical practices will be penalized over $350 million dollars with the inception of MIPS/MACRA/whatever. We doctors are already sustaining the “2% sequestration” and now must face 4-9% reductions in our reimbursements for professional medical care. This is not news to any of you reading this.

To put this in perspective, ONE F-22 Raptor fighter jet costs approximately $137 million dollars (we have only 6 so far, but maybe more to be built - there is some controversy about this) and ONE F-35 Joint Strike Fighter costs about $104 million (we have 171 so far). So, if my math is correct:...
