Skin Cancer of Feet Often Overlooked: NY Podiatrist
 
Oftentimes, skin on the feet, especially on the bottom, is overlooked during routine medical exams. According to Dr. Robert Russo, NYSPMA’s President and a practicing podiatrist, skin cancers of the feet have several features in common. “Many times there’s a history of recurrent cracking, itching, bleeding or ulceration,” he cautions. “Because they can also be painless, these cancers may go undiagnosed until another issue presents itself near the affected site,” added Russo. Checking for warning signs is something anyone can do, paying particular attention to changes such as non-healing sores, bumps that crack and bleed, nodules with rolled edges or scaly areas. Examining the bottom of the foot is critical.

Skin cancers on the lower extremity may have a different appearance than those arising on the rest of the body. So to test, the podiatrist will perform a skin biopsy. This is a simple procedure in which a small sample of the skin is obtained and sent to a lab where a skin pathologist examines the tissue in greater detail. If it turns out the lesion is skin cancer, the podiatrist will recommend the best course of treatment.

Dr. Bryan B. Kagan has been appointed to the Science and Practice Subcommittee on Forensic Podiatry of the International Association for Identification. Kagan is a forensic podiatrist who has written and lectured nationally on forensic podiatry and is conducting research and analysis on footprints and gait at crime scenes.

Kagan maintains his private podiatric practice in White Plains, NY. He is on the executive board of the American Society of Forensic Podiatry and was elected its Secretary-Treasurer in 2013. He is also a member of the International Association for Identification.

Woman Wins $775G Judgment Against NY Podiatrist
 
A teacher from Nyack who can't stand up without pain in her foot due to what she said was botched surgery that she had as a teenager won a $775,000 judgment against the podiatrist who performed the operation. Inayah Davis was 16 when she consulted Bardonia podiatrist Dr. Ira Bernstein in 2008 about a bunion, according to court documents. Bernstein recommended surgery to remove the bunion. Davis and her mother agreed to the treatment. But Davis alleged the podiatrist also performed unneeded hammertoe surgery on four toes on her left foot. The podiatrist charged $27,000 for one surgery and $13,000 for a second procedure soon after.

Query: Topical Treatment of Squamous Cell Carcinoma in an 87 Year Old
 
I have an 87 year old homebound patient who has a squamous cell carcinoma patch (confirmed by punch biopsy) on the dorsum of her foot. It measures 2.5 cm x 1.5 cm. 
 
Having her get out of her house for a surgical incision would be extremely difficult. I am looking for advice on topical treatments that could be done in the home.

"the Privacy Rule allows covered healthcare providers to communicate electronically, such as through e-mail, with their patients, provided they apply reasonable safeguards when doing so. See 45 C.F.R. § 164.530(c). For example, certain precautions may need to be taken when using e-mail to avoid unintentional disclosures, such as checking the e-mail address for accuracy before sending, or sending an e-mail alert to the patient for address confirmation prior to sending the message.
 
Further, while the Privacy Rule does not prohibit the use of unencrypted e-mail for treatment-related communications between healthcare providers and patients, other safeguards should be applied to reasonably protect privacy, such as limiting the amount or type of information disclosed through the unencrypted e-mail. In addition, covered entities will want to ensure that any transmission of electronic protected health information is in compliance with the HIPAA Security Rule requirements at 45 C.F.R. Part 164, Subpart C."

I am considering switching from using phenol to NaOH on my chemical matrixectomies. The reason for switching would be simply because of the shorter amount of time required to use the NaOH protocol. My question is to my colleagues: Have any of you who switched from phenol to NaOH regretted it due to a higher incidence of regrowth?

Query: CPT 64450 and Medicare Denials
 
Is anyone billing Medicare procedure code, CPT 64450 (peripheral nerve block, therapeutic/diagnostic) and being rejected as not medically necessary or for too many units?

Response: I just received a response on an appeal for this very thing. 
 
I found out that we were actually billing incorrectly when I looked at the LCD. There are specific diagnosis codes you have to use to have it covered. We were billing CPT 64450 for a Morton's neuroma nerve block when we should have been billing CPT 64455. The LCD for CPT 64450 is here.

My recommendation to contact med mal isn't to make yourself bulletproof, because that's not possible. Rather, your carrier can give advice in the way to craft and advise on what are appropriate documents regarding the case. When I was in private practice, I was insured by PICA. PICA gave me incredible support, advice, and help with knowing how to address thorny patient situations. Please remember the old adage: "If it isn't documented, it didn't happen."
