APMA and 42 Other National and State Podiatry Associations Urge Congress to Maintain Access to Podiatrists under Medicare and Medicaid
APMA and 42 other state and national podiatric medical societies have submitted a co-signed letter to the House Committee on Energy and Commerce and the Senate Committee on Finance, urging these committees to protect and preserve access to care by podiatrists for Medicare and Medicaid beneficiaries in the forthcoming budget reconciliation process. This APMA-led letter highlighted the opportunity for addressing the decline in Medicare physician reimbursement by passing the Medicare Patient Access and Practice Stabilization Act” (H.R. 879).
The letter also stressed the important role that podiatrists play in caring for Medicaid patients and how potential Medicaid cuts would negatively and disproportionately impact patients’ access to necessary foot and ankle care provided by podiatrists.

Martin Foot and Ankle Joins WellSpan
Expanding access to podiatric care for patients in Lancaster and York counties, WellSpan Health has announced plans to integrate Martin Foot and Ankle into its health system later this summer. The four Martin practices now will operate under the WellSpan name in Hanover, Lancaster, Lititz and York, PA, making WellSpan’s podiatry services the largest and most comprehensive employed care team of foot and ankle specialists in the region.

Once the affiliation is complete, WellSpan podiatry, foot and ankle will offer: Three orthopedic foot and ankle surgeons, 24 podiatrists, 375 licensed rehabilitation professionals, and 43 outpatient locations.

Healing Benefitted with Prolotherapy: MI Podiatrist
According to Conway McLean, DPM, "Our current regenerative techniques utilize a wholly different approach than traditional medicine. There are a variety of therapies and treatments falling under the auspices of regenerative medicine, all harnessing this concept of enhanced healing. One of the longest used is prolotherapy, an injection technique, in which some naturally-occurring substance is injected into an area of pain or injury. Prolotherapy doesn’t use pharmaceuticals, aka drugs, true for all regenerative techniques, so the adverse effects associated with drugs aren’t seen."

"Prolotherapy would seem to initiate acute inflammation which, it turns out, is an essential component of tissue repair. The term “inflammation” has gotten a bad rap in the media and that is appropriate when it’s the chronic type, which has many negative consequences and connotations. Very different is acute inflammation, which clearly is an essential part of the healing cascade. We don’t see problems after prolotherapy, other than those of being stuck with a needle," says Dr. McLean.

When choosing shoes for walking long distances across different surfaces like concrete, sand, or trails, Bruce Pinker, DPM says it's important to focus on features that ensure comfort, support, and durability. "Look for a durable outsole with adequate traction, a firm heel counter for stability, cushioned and shock absorbing midsole, a cushioned insole with added arch support, durable uppers, and a toe box that is not cramped and allows for an air exchange," he explains.

If you're searching for a shoe for walking in sand or on the beach, Pinker recommends choosing one with a rubber outsole and a flexible, grippy tread pattern, as it's generally the most effective for preventing slippage on those types of surfaces. 7 Ways to Turn Every Visit into a Warm Welcome
These strategies draw on real-world lessons and can be phased in without derailing workflow or budgets.
3. Empower the front desk as a hospitality hub.
The first voice a patient hears often sets the emotional temperature for the entire visit. Friendly greetings, clear explanations of co-pays and genuine eye contact not only build trust but also cut billing friction. Post a “welcome script” near each workstation and practice role-play scenarios—especially for new hires—so warmth becomes muscle memory.
Pro tip: Add small talk prompts to your EHR (upcoming vacation, favorite sports team) so staff can personalize each interaction without slowing check-in.

Although I agree with Dr. Udell's position about being satisfied with the DPM degree we received in the 1970s, I think he is missing my point. The PM News survey of current DPMs concerning their satisfaction with a DPM degree revealed that only 14% of the over 600 responses would settle for a DPM degree and a 3-year residency today if they had the option of earning a DO degree instead of a DPM degree. With the number of applicants to podiatry school dwindling at a rapid rate, it won't be long before podiatry schools will not be able to afford keeping their doors open. At the same time, DO schools have increased to over 35 institutions and offer more options to students when it comes to residency choices. I hope that a residency program offering a 1-year general internship and a 2- or 3-year foot and ankle program will evolve to keep the spirit and efficacy of podiatry alive.

I think Dr. Tonczak's letter perfectly defines the conflict between academics and practical podiatrists. I was doing some training at the Pittsburgh Podiatry Hospital when I asked the most prolific surgeon why he didn't write books and articles. He answered, "because I don't have time."
Dr. Tomczak said not knowing about a screw extractor should disqualify board certification. I was the director of a surgical residency and didn't know about them. No hospital I ever worked in would have such an instrument in stock. Maybe breaking a screw during surgery and not being able to deal with it should disqualify you from certification?

If our podiatry days are numbered and our demise is approaching according to Dr. Tomczak, can someone explain to this old retired podiatrist why more podiatry schools have been recently established? When someone asks me what my profession is, I say that I’m a retired podiatrist. If the general public doesn’t know the scope of our field, who is responsible for disseminating that information?
New Mexico is seriously short of primary care MDs and DOs. As a result, nurse practitioners are often the first primary providers seen by new patients. One of my neighbors who typically accompanies his wife on our monthly hikes was absent from the last hike. When I asked Barbara where Jim was, she said, “Jim is having foot trouble and his primary care provider (a nurse) told him, there’s nothing else that can be done for your foot.” That naive statement just dismissed our entire profession! I asked Barbara a few basic questions about Jim’s complaint and then referred him to one of the several excellent podiatrists in the area whom I know personally.
Jim was fitted with custom orthotics and is doing well. I cringe every time I see TV ads from the “Good Feet Store.” After prescribing custom orthotics for over 40 years, I don’t ever recall a patient telling me that their back pain or foot pain disappeared after one day of wearing these prescription devices, let alone those (paid persons) who claim immediate relief by wearing the OTC arch supports dispensed by the Good Feet store. So, who’s responsible for educating the general public about our beloved profession? Why are there new podiatry schools being created if we are on our “last legs?”

So sorry to hear about the passing of Bill Van Pelt. I just wanted to share some of Bill's highlights with his time spent with the American Academy of Podiatric Sports Medicine (AAPSM). Bill became involved with the academy at its peak in popularity which was the decade of the '80s. It was during this time that he "rode the wave" and immersed himself into sports, which changed his life, personally and professionally. With his strong commitment to pursue sport medicine and sports, he literally became the "running foot doctor of Houston" and he himself completed 30 marathons during his career.
With the academy, he progressed from member to fellow, then board member and ultimately to the 12th president of the AAPSM in '89-'90. Bill also originated the annual AAPSM Dive Seminar which spanned a period of 30+ years. These seminars allowed Bill to pursue his passion for diving but also provided an opportunity for him to share his sports medicine expertise. He will be missed by many!
