OTC Insoles May Help Even Those Without Arch Issues: NY Podiatrist    
 
Even if you don’t have arch issues, insoles can provide more comfort as you walk. “They provide cushioning and support as it off-loads pressure points on the foot,” says Bruce Pinker, DPM, a board-certified podiatrist and foot surgeon in New York. “They can also improve the biomechanics of one’s feet and reduce pain in the feet.”

Because of the extra cushioning, they can also be good for those dealing with plantar fasciitis, a condition where your plantar fascia gets inflamed. This can happen from working out in shoes with no support, tight calves, carrying excess weight, and more. While custom insoles (also called orthotics) can provide personalized support, over-the-counter insoles can also help. “They can provide cushioning and shock absorption and possibly reduce pain in the heel or arch,” says Dr. Pinker. “They aren’t harmful, though they may not be helpful to those with serious foot conditions.”

NC Podiatrist Discusses What to Look For in Running Shoes for Flat Feet
 
According to Elizabeth Daughtry, DPM shopping for a running shoe with "proper alignment and balance" is key to limiting the potential for injuries and ensuring the most comfortable fit for runners with flat feet. "The biggest concern for a runner with flat feet is overpronation, or the fact that their feet roll excessively inward during stride. Overpronation reduces the foot's ability to absorb shock and alters the function of muscles and tendons across the foot and ankle, which can lead to a multitude of painful conditions and injuries," Dr. Daughtry tells PopSugar.

She also notes that because not all flat feet are the same, what works for one runner might not work for another. Some foot structures are "more rigid" while others are more flexible. "In general, a flat foot typically is wide on stance from the flattening of the arch and overpronation, so the availability of a wide width is important. Some people prefer a more rigid stable shoe, and support is important to avoid injury or foot pain, but others have a flatter foot that is irritated by excessive arch support."

IL Elks Free Crippled Children's Clinic Once Again a Success
 
For over 17 years, David Yeager, DPM has volunteered his time for the Elks Crippled Children’s Clinic, seeing children from as far downstate as Peoria, Illinois, and are far east as Chicago. The turnout for the clinic, held quarterly on Thursday mornings, included over 10 children who were provided free foot care, and received custom-made orthotics thanks to the generosity of the Elks organization.

UK Podiatry Students Run Free Clinic for Hope Centre
 
Students and staff at University of Northampton's Clinic opened the doors to people who are/have been experiencing homelessness for free treatment, conversation, and food. There was a new addition at the University’s Podiatry Clinic when the team opened the doors for a special treatment morning.

People who are homeless, rough sleeping, or otherwise facing longer than expected issues with permanent and safe housing, have the same foot health issues as the general population. As more of them are exposed to the elements more often and could have other health issues, they are especially vulnerable, making it vitally important they receive professional foot and lower limb treatment as soon and often as possible.

PODIATRISTS AND HEALTHCARE LEGISLATION
OH Podiatrist Co-Sponsors Bipartisan "Hospital At Home" Extender Legislation
 
Representatives Brad Wenstrup, DPM (R-OH) and Earl Blumenauer (D-OR) introduced the bipartisan Hospital Inpatient Services Modernization Act. This important legislation includes a 5-year extension of the Acute Hospital Care at Home program to allow participating providers to continue furnishing services that enhance access to care.  This bill will allow for the expanded delivery of hospital-level care for Medicare beneficiaries in the comfort of their own home.

This bill incorporates feedback received from providers and health systems participating in the Acute Hospital Care at Home (AHCAH) program or considering doing so under an extension of the waiver. In addition to a 5-year extension for Hospital Care at Home waivers, this legislation also includes improvements to data collection required under the program to better inform Congress on how to make this program permanent in the future. “As a physician and Co-Chair of the GOP Doctors Caucus, I have seen firsthand how important it is for patients to have access to the method of care that best fits their needs," said Dr. Wenstrup.

MEDICARE IN THE NEWS
Medicare Hospital Trust Fund Extended by 5 Years to 2036
 
The Hospital Insurance (HI) Trust Fund will be fully solvent for longer than previously projected, according to an annual Medicare Trustee report. Though the report details “significant financing issues,” the program will be able to pay all scheduled benefits until 2036, seven years later than reported in 2023 and five years later than the most recent report indicated.
 
The trustees attributed the HI Trust Fund improvement to a policy changing how medical education expenses are calculated for Medicare Advantage rates in 2024, more payroll tax income in an economy performing better than experts expected, and lower-than-expected expenses in 2023 on inpatient hospital and home health agency services.

My position in this discussion is that there is nothing unethical about charging patients directly for medical services. My position is that telling practitioners all over the country to convert their practices into pure direct pay practices is problematic.
 
Yesterday, I saw a patient who did not have any medical insurance. The patient paid us directly. We did not have hassles with insurance companies, did not have to wait months for payment, etc. I reflected on this current discussion/debate that we are having and said to myself, "I only wish that all of my patients paid me directly." 
 
The rest of the patients I saw that day were insurance company patients and one even called to make sure that we accept her insurance before confirming the appointment. Would converting my practice to a pure direct pay entity work for me? I doubt it and would advise any of my colleagues thinking about such a transition to discuss it with their accountants and family members before taking on this risk.
