PODIATRISTS IN THE COMMUNITY
11 Podiatrists Chosen as Top Docs by Los Angeles Magazine

Los Angeles County is a region loaded with cutting-edge medical institutions and highly skilled doctors. But finding those doctors in the sea of options can involve a lot of trial and error. This list of L.A.'s top doctors should help. This is the first time Los Angeles magazine has undertaken such a project, but it’s a staple for city magazines around the country.

Professional Research Services of Troy, Michigan conducted a peer-to-peer survey, asking practicing physicians throughout Los Angeles County to identify the doctors they consider to be at the top of their game. Here are the podiatrists chosen: Drs. Aziz Berjis, Bob Baravarian, Charles Ananian, Jason Khadavi, Keith Gurnick, Mark Weissman, Matthew Safapour, Michael Avakian, Orlando Zorrilla, Stephen Schwartz, and Vladimir Zeetser.

IN THE COURTS
CA Podiatrist Who Murdered Partner to be Paroled

At his seventh parole hearing, a podiatrist serving a life sentence for the 1976 murder of his business partner was found suitable for parole, according to the San Mateo County District Attorney’s Office. A state board found William Moalem, 78, posed a low risk for re-offending if he is released from prison, having complied with the rules of the California Men’s Colony State Prison in San Luis Obispo, according to prosecutors. Convicted of first-degree murder in 1999, Moalem is believed to have killed his business partner, Dr. Benjamin Hurwitz, also a podiatrist, to collect $60,000 in life insurance when he ran into financial trouble nearly two decades earlier, according to prosecutors.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
IL Podiatrist Uses CBVT Imaging

The pedCAT™, TRUE 3D weight-bearing imaging device uses technology, known as cone beam volumetric tomography (CBVT), and aids doctors in diagnosing and treating conditions including but not limited to fractures, subluxations (misalignment) and dislocations, midfoot injuries, bunions, flat feet, sprains, arthritis, and diabetic-related complications.

Dr. John Sigle

According to Dr. John Sigle, founder of the Foot & Ankle Center of Illinois, “Having an in-office, state-of-the-art imaging system has allowed us to re-invent our approach to surgical planning. Scans are taken in a matter of minutes. Dr. Sigle also uses this computer-guided surgical planning system on all total ankle replacements, bunions, and hallux rigidus procedures.

TRANSITIONING TO PRACTICE

Navigating the Waters from Residency to Practice - Part 3

Supergroups offer an array of benefits and may include traditional podiatric services, physical therapy, MRI, pathology, diabetic shoe program/store, and surgery centers. Some of the key advantages to working in a supergroup are decreased expenses, increased revenues, “best practices”, and pooling of professional resources.1 You will need to seek additional legal counsel to explore the options in your area of practice.

Do your homework in advance and guide your decisions on sound judgment and proven methods. Ask questions and always think about what is next before moving on to the next step. Remember, measure twice and cut once as your career and future are counting on it!

PRACTICE MANAGEMENT TIP OF THE DAY
Common Mistakes of Physician Group Mergers - Part 1

Acquiring another practice can be a powerful tool for your group to achieve growth and build long-term value. There are, however, a few key mistakes that are often made which could be avoided with more thoughtful reflection.

1. Failing to define a vision—before the integration occurs. Crafting a vision that clearly spells out the opportunity inherent in the transformation ahead should, but often doesn’t, start long before a deal is pursued. Drawing from an "expanded due diligence" process that explores quantitative metrics, like physician group operations and financials, as well as the people and culture of the merged entity, is key.

Leaders must build and communicate a vision of success that is understandable, tangible, and compelling to employees throughout the ranks of both practices. Factoring in the value of the people, assets, and cultural elements of each practice will empower leadership to look beyond the basic additive advantages of the deal and construct a more holistic vision—inspired by both head and heart—for the opportunity ahead.

QUERIES (NON-CLINICAL)

Query: Collection Agency for Medicare Deductibles

I am looking for a reputable collection agency for a very small number of patients. The common denominator in my practice is the very few who refuse to pay their Medicare deductible. Why isn't there any recourse doctors have that would allow them to report these patients to Medicare. I scratch my head every year wondering why our government doesn’t just take the yearly deductible out of every beneficiary's Social Security check at the beginning of the year. It would save a ton of aggravation, time, and money from our already financially squeezed practices, not to mention saving the government a large sum of money. It is very frustrating to think there is a very simple fix to a yearly problem many Medicare participating practitioners have.

RESPONSES/COMMENTS (CLINICAL)
From: Michael M Cohen, DPM

Regarding Dr. Callarman's query about Hyalgan in the ankle... I co-authored an article we published in Foot and Ankle International regarding the efficacy of Hylgan in the ankle joint. The article was published with Dr. Roy Altman, our former Chief of Arthritis and University of Miami faculty member. Drs.  Altman and Moskowitz published one of the original multi-center trials of Hyalgan for osteoarthritis in the knee and is a gold standard article.

Please also note that there is another fine article published in JBJS regarding the same by Dr. Tom Chang. I hope this helps answer some of the questions raised.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: NJ to Monitor Gabapentin Prescriptions
From: Allen Jacobs, DPM

I know a lot of docs who decreased scripts for narcotics and began using gabapentin. Now this:

Dear Prescriber,

Effective May 7, 2018, the New Jersey Division of Consumer Affairs adopted amendments to the New Jersey Prescription Monitoring Program (NJPMP) rules at N.J.A.C. 13:45A to require New Jersey licensed pharmacies and registered out-of-State pharmacies to electronically transmit information to the Division about prescriptions dispensed for gabapentin. The recognition of gabapentin as a “drug of concern” stems from national prescription and overdose data. New Jersey is joining a growing list of states who have already begun to monitor gabapentin use, including those that have scheduled the medication at the state level.

Studies have shown that gabapentin prescribing in the United States has increased...

RESPONSES/COMMENTS (NON-CLINICAL)
From: Tom Silver, DPM, Lloyd Nesbitt, DPM

It had been years since I've seen MBT shoes until a new patient came in the other day wearing them. I can't speak for other MBT styles, but the pair he was wearing were ridiculously heavy (as much or more than a steel-toe boot). It's a rocker bottom with a ton of cushion in the soles, especially in the heels; kind of like wide Sketcher Shape-Ups with cement inlays!

Tom Silver, DPM, Minneapolis, MN

These may work well for patients with a painful hallux limitus, dorsal first MPJ exostosis, or hallux rigidus where they don't have adequate hallux dorsiflexion at the first MP joint during the propulsive phase of the gait cycle. The rocker bottom sole allows for facilitated propulsion when the hallux doesn't dorsiflex.
