If you have athlete's foot, expect dry, cracked skin that peels off the bottom of your foot and in between your toes, says Brian Fullem, DPM, a sports podiatrist and author of The Runner’s Guide to Healthy Feet and Ankles. These areas become extremely itchy and, in serious cases, you may develop blisters filled with clear fluid.

The cause: A fungus called tinea pedis, says Meredith Ward, DPM, a podiatrist at St. Francis Hospital and Medical Center in Hartford, CT. Your sweaty socks are often to blame, since fungus requires darkness, warmth, and moisture to thrive, says Dr. Fullem.

May's Bandstand Flag Flying in Honor of IA Podiatrist
 
An American flag will fly throughout the month of May in honor of Dr. C.B. Dunshee (November 4, 1919 - January 8, 2013) , Chief Yeoman, United States Navy. C.B. enlisted in the United States Navy, September 17, 1941 in Chariton, Iowa. He was assigned to the USS Lesuth, an auxiliary cargo ship, where he acted as a senior yeoman. The ship was in the South Pacific for the greater part of two years and participated in various assignments.

Upon his discharge, he enrolled at the Illinois College of Podiatry. He graduated with a Doctor of Podiatry degree in 1949. Following his graduation, C.B. established the first podiatric office in the Oskaloosa. He remained in practice until 1991.

Podiatrists Averaged $100K From Medicare in 2014
 
In the third round of what's now an annual event, the CMS posted a huge data set that breaks down the $91 billion Medicare paid individual physicians, diagnostic labs, ambulance services, and other companies. And for the first time, the agency included a set of numbers that exclude adjustments for regional factors like wages. The addition of standardized figures is intended to make the variation in payments reflect “physicians' practice patterns and beneficiaries' ability and willingness to obtain care,” the CMS said in a news release. The latest round reflects 2014 claims. 
 
According to CMS, podiatrists received an average of $99,708 in Medicare payments compared to $89,366 paid to internists and $55,536 received by family practitioners. Orthopedic surgeons received $128,820.

3-D Printed Bone with Just the Right Mix of Ingredients
 
To make a good framework for filling in missing bone, mix at least 30 percent pulverized natural bone with some special man-made plastic and create the needed shape with a 3-D printer. That’s the recipe for success reported by researchers at The Johns Hopkins University in a paper published April 18 online in ACS Biomaterials Science & Engineering.
 
That has led investigators to 3-D printing, or so-called additive manufacturing, which creates three-dimensional objects from a digital computer file by piling on successive, ultrathin layers of materials. The process excels at making extremely precise structures — including anatomically accurate ones — from plastic, but “cells placed on plastic scaffolds need some instructional cues to become bone cells,” says Wayne Grayson, PhD, associate professor of biomedical engineering at the Johns Hopkins University School of Medicine and the report’s senior author. “The ideal scaffold is another piece of bone, but natural bones can’t usually be reshaped very precisely.”

2,000 Doctors Back Single Payer Healthcare Plan
 
More than 2,000 physicians announced their support Thursday for a single-payer national healthcare system, unveiling a proposal drafted by doctors that appears to resonate with Bernie Sanders' call for "Medicare for All." In an editorial and paper published in the American Journal of Public Health on Thursday, the doctors call out the "persistent shortcomings of the current healthcare system." They warn about the risks of continuing along the path laid out by the Affordable Care Act: "down this road, millions of Americans remain uninsured, under-insurance grows, costs rise, and inefficiency and the search for profits are abetted."
 
The future of health reform has been widely discussed in the presidential campaign, and for years, health reform has sparked a raging and divisive political debate among politicians. The proposal, however, is endorsed by hundreds of physicians who have an inside view of the effects of the law on patients and medical care. It grew out of discussions in late 2014, when a small group of physicians began to assess the effects of health reform and found it coming up short.

A recent report pointed out that people lie about their smoking habits, but I think we all knew that already. If the patient is saying 1/2 pk/d, it's probably 1.5 - 2 pk/d. If this were my patient, I would take the opportunity (and put my foot down, looking her straight into her eyes) and say, that for any possible treatment to work, the patient must quit smoking 100% immediately. If she won't help herself, it's a waste of time to throw Rxs at the problem.

Consider reflex sympathetic dystrophy syndrome (RSDS) or CRPS as a differential diagnosis. These patients often present with burning-type recalcitrant nerve pain, hyperhydrosis, bluish or red color changes to the feet, and most importantly, sensitivity to cold water or temperature (allodynia). This may be attributed to prior surgery, infection, or radiation therapy, but sometimes the cause is not clear. This patient warrants a quick referral to pain management for evaluation and physical therapy as a preferred treatment course.

Preparing for our changing reimbursement model ideally means you are among the first podiatrists in your locale who are ready to step into providing value-based care. Nobody is absolutely certain about all the definitions of "value" that will emerge, but it is possible to prepare for any definition and requirement once you know what it is. If you Google search articles on value-based medical care using keywords, you will find a lot out about it. 
 
Whether we like it or not, the emerging foot healthcare marketplace will be dominated by those groups of podiatrists already transitioning into it. I bet some podiatrists reading this are already making appointments with the medical directors of ACOs to get their...

That may be true in NY, NY but in communities away from the major metropolitan areas, it certainly isn't. Here in upstate, you're more likely to find people who have a hard time coming up with co-pays than those able and willing to pay for concierge podiatry.

Dr. Shavelson is correct when he implies that in every community there are people who will afford a BMW, pay for Botox injections and tummy tucks, but will bark and even walk out the door when we ask them for their annual deductible, or to fill a limb-saving antibiotic prescription. It's not just in podiatry. Last year, the New England Journal of Medicine published a study on patients who survived myocardial infarctions and were sent home from the hospital on drugs designed to prevent another episode. Less than fifty percent of the patients surveyed filled their prescriptions. Part of the reasoning for their non-adherence was the cost of the drugs.
 
We need to understand that human beings have basic emotional needs. One is the desire to look good. Hence the plastic surgeons laugh all the way to the bank. Another need is to avoid pain, and this is why patients will run to us when their feet hurt, but after they feel better, many will not "spring" for a pair of orthotics to prevent recurrence.
