Hair Loss of Toes Could Indicate Arterial Disease: NY Podiatrist

If you have bald toes, it could be arterial disease. If the fuzz on your toes suddenly disappears, it could signal poor blood circulation caused by peripheral arterial disease (PAD). “Signs of PAD can include decreased hair growth on the feet and ankles, purplish toes, and thin or shiny skin,” says Suzanne Fuchs, DPM, a podiatric surgeon at North Shore University Hospital in New York. PAD affects about 8 million Americans. Symptoms are subtle, but doctors can check for a healthy pulse in the foot or spot PAD on an x-ray.

Off-loading of Diabetic Ulcers Can Prevent Amputations: FL Podiatrist
 
A diabetic patient at Good Samaritan Clinic quit smoking and made changes in his diet, but healing the diabetic ulcer on his foot remained a challenge. "If you healed it, it would open up two weeks later because of the pressure on his foot," said Dr. Jeffrey Karr, a Lakeland podiatrist.

The solution proved to be special inserts, called pressure off-loading inserts, for the man's sneakers. They took pressure off the sore, helping it to heal. If the foot ulcer had become more infected, the man might have been looking at surgery, perhaps amputation.

Marathon Runners Should Follow the 10 Percent Rule: FL Podiatrist
 
According to podiatrist Dr. Brian Fullem, training for a marathon should be a gradual progression. One of the biggest risks runners take, he said, is increasing their mileage too quickly. It is important to build up the total numbers of miles run each week, he said, but increasing mileage by more than 10 percent per week can lead to injury.

But injuries still occur, even if runners follow the 10 percent rule to the decimal place. The most important thing for physicians, Fullem said, is to treat the cause of injury, not just the symptoms. When they get injured, Fullem said, “people tend to just reach for the ibuprofen.” “There’s never one magic bullet for an injury,” he said.

A 2014 survey from the American Podiatric Medical Association—composed of the nation’s top foot and lower-leg docs—found heels were far and away the most common cause of foot pain among women.

Calluses, blisters, bunions, and ingrown nails are all common among high heel wearers, adds Dr. Rodney Stuck, division director of podiatry at Loyola University Chicago. The higher the heel, the more trouble you’re likely to run (or walk) into, Stuck says.

NY Podiatrist Cuts Ribbon for Larger Office
 
On April 28, Dr. Mindy Lam used a pair of oversized golden scissors to snip the red ribbon at Foot Care of Clifton Park’s new location. Her office had been in the Emma Lane Professional Park since June 2009. After some time, her practice outgrew those confines. Lam felt that having her business in the same area that she resides in means she’s a stronger part of the community.

Query: Affordable ICD-10 Crosswalks
 
There are podiatrists independently trying to re-invent the wheel by coming up with their own ICD-10 crosswalks. It would be great if we could have one comprehensive podiatry ICD-10 list which many of us would be willing to pay a reasonable fee to obtain, from someone who has put forth the work to do it all. Is there anyone out there who has put together a comprehensive ICD-10 list that pertains to most all of the podiatry codes that we use, and could they share it for a reasonable fee for their hard work?

I am considering purchasing a nerve testing unit, SmartSkan CPT 95923. I was wondering and hoping that I can get some feedback on whether or not we, as podiatrists, are eligible to use this (involves placing both hands and feet on plates for measurements).  Any other info regarding reimbursement and usage would be appreciated.

Response: First check with your state regs whether this testing is covered under your scope of practice. Then, I would check with your major insurance companies about coverage issues, and specifically the appropriate use of CPT 95923 or other coding for the testing. Many plans do not cover this type of testing or this unit, or they do so in very limited scenarios. Be very clear when asking about coverage issues to make sure that you are being given accurate information. Many carriers deem this testing/unit investigational. Do not rely on what the company reps tell you about coverage and potential income revenue.

Please confirm that the wound center is billing for the professional component of the wound care. Wound centers bill for the facility component and supplies that are used. If the wound center is billing for the "doctor" professional component, then you would need to have a discussion with the facility about proper billing protocol.
 
Please note that although you are in a wound center facility, if you do not have an ABN and use the proper modifier, you cannot bill the patient for the service when you receive the denial. Have a talk with the billing office for the wound center to work this out.

The DPM who bills separately for his/her services regards himself as a "private contractor." This DPM needs to contact a healthcare attorney with knowledge of healthcare and Federal billing practices immediately. The healthcare attorney will then advise the client on how to handle this.
 
This is classified as a so-called "legal emergency" and must be dealt with immediately. Failure to deal with this issue could result in one or more charges of healthcare/Medicare fraud against both the clinic and the DPM. In the meantime, don't throw ANYTHING out. If you have any copies of the clinic charging, keep them.
 
You will likely soon be separating yourself from the wound clinic. They will throw you under the bus and will not protect you. Don't disclose to anyone there that you are seeing an attorney. Say nothing to them without competent legal advice. Chances are that your attorney will be contacting them. Be sure you pay for your attorney yourself, preferably by giving them a retainer, or pay the office charge by check before you leave. Your healthcare attorney must only represent YOU. By paying him, you are cementing that representation.

I want to be clear that I have never stated that EPAT was not effective for anything. EPAT shouldn’t be in the ESWT conversation. There is no relationship between EPAT and ESWT. EPAT is a class one therapeutic massage device that is indicated for pain. EPAT has never undergone FDA approval. There are no double-blind, randomized, placebo, controlled studies to attempt to show if it’s safe and/or effective. EPAT isn't an ESWT shockwave device and shouldn't ever be presented as an ESWT device to the patient or in marketing. I am saying that we need to use evidence-based medicine, especially at the level of FDA class three approval. Cheaper, yes. Effective? PROVE it.
