Managing Diabetes Begins with Education and Screenings: NC Podiatrist
 
In observance of National Diabetes Awareness Month, CarolinaEast Health System offered a free foot screening last November and was pleased with the interest and feedback. “It is possible to successfully manage a diagnosis of diabetes, and it all begins with education and screenings,” says James G. Underhill, DPM with CarolinaEast Physicians. “Individuals living with diabetes must learn about their condition and know how to take care of themselves, as well as know when to see a professional. I am pleased to offer this service again for those who may not know the importance of foot care when it comes to managing diabetes,” he added.

Dr. Underhill is a graduate of the Temple University School of Podiatric Medicine, where he earned his podiatry degree. He completed his residency at the Eastern Virginia Medical School, in Norfolk, VA. He is is board eligible in podiatric surgery.

FDA Clears Lapiplasty® 3D Bunion Correction System for Use in Pediatric Patients

When to Consider Succession Planning - Part 4
 
You might feel a little uneasy about starting the discussion about selling your medical practice. Your practice is your baby, and the thought of letting go can be overwhelming. However, the truth is that failing to plan is a plan to fail when it comes to your practice exit strategy. Everyone thinks of their future, but they don’t always take active steps in the present to prepare for what they want tomorrow. Here are several reasons why you should start the succession planning conversation today.
 
4. You’re feeling burnt out - Running a practice takes a lot of tenacity. Burnout can creep in slowly and take hold in what feels like an instant. It’s important to balance your work life and home life, and that takes commitment and constant fine tuning. You need to set boundaries to make it effective. If you don’t have a good handle on taking time for yourself, and you are just barely keeping things afloat, exiting partially (such as by bringing on a physician partner, engaging private equity, or joining a management services organization) might be a good option to help you reclaim time to yourself while still maintaining, and hopefully growing, your practice.

Query: Ingrown Toenail on a 6 Month Old Boy
 
A 6 month old baby boy presented with a chronic paronychia and onychocryptosis to both big toes at the medial corners. The R big toe has a lot of granuloma and is enlarged at the tibial corner. PMH: He was a month early in birth. Otherwise, he is healthy and has no developmental issue.  
 
Ingrown Toenail on a 6 Month Old Boy
 
I would like to do a matrixectomy to remove the offending borders. The procedure would be under general anesthesia. I am leaning toward the cold steel nail procedure vs. the phenol and alcohol procedure. I would love to hear the pros and cons to the treatment plan you have in mind for this particular baby boy.

It is with such sadness that I read the PM News note about the passing of Dr. Birkholm. I had the privilege of knowing him for nearly my entire life. He and my late father Leo were close colleagues-- and would cover for each other's solo practices for some four decades. He and his wife Petey occupy some of the earliest and most fond memories from my childhood, as we accompanied them on travels around the world. This was a man who was innately good. It is my hope that this goodness will live on in the memories of the thousands of lives he touched as a doctor and still more as a dad and a husband.

Medicare does not approve devices and any use of that term to describe a device regarding the coding validation it receives from the PDAC DME Medicare agency is more than misleading. There is sufficient language provided on the PDAC website to inform the consumer, supplier, and applicant that the PDAC and Medicare does not approve devices. The PDAC agency has the task of inspecting a device (and any accompanying literature) and validates that the device being inspected meets the criteria for a specific HCPCS code. Therefore, the correct terminology is the device received product coding validation from the PDAC. 
 
As for coverage, let there be no mistake, those issues are not within the realm of jurisdiction of the PDAC, but of the DME MAC. The DME MAC Orthopedic Footwear policy is quite clear on foot orthotic coverage for L3000-L3060. Specifically, those codes are not reimbursed by Medicare unless the device is placed into a shoe which has a...

Thank you, Dr. Woodly, for your comments on pedicures and for your suggestions for how consumers can make sure that the place they are going to is good; such as their use of a new and unopened package of instruments, and that the techs are licensed and are staying within their scope of practice. These are great suggestions. I would like to add some. 
 
First, that the new, unopened package of instruments should be from an autoclave; second, that the place where the pedicure is offered must be immaculate; and third, that the technician has a certification as an advanced nail technician (ANT) or, if in a podiatry office, a certificate as a medical nail technician (MNT). These technicians are trained to only perform aesthetic pedicures and they are additionally trained to "Recognize and Refer" to a podiatrist if they...

I have seen successful clearing of mycotic toenails with laser treatment most of the time. However, it almost never works as mono-therapy. It must be combined with consistent use of effective topical medications, frequent thorough debridement, consistent shoe disinfection, and treatment of the skin with a topical antifungal cream. If you are missing any of these components, you are simply “shoveling in a fungal snowstorm.” It’s like giving antibiotics but not climbing out of the septic tank with your open wound. 
 
But wait, there’s more. The condition will almost always recur without long-term prevention, including shoe disinfection, topical medication a few times a week, treating any tinea pedis as soon as it appears, treating any underlying conditions (i.e., hyperhidrosis), and returning to...
