Canadian Podiatrist Provides Tips on Preventing Black Toenails
Run for long enough, and sooner or later, you’ll find yourself with a black toenail, a.k.a. runner’s toe. “If you want to start becoming a runner, be prepared to have your nails injured,” says Dr. Roy Mathews, a podiatrist in Vancouver. Caused by toes rubbing up against the inside of your shoes, black toenails are often the result of long runs and running downhill. Here’s how to avoid them — and what to do if you have one.

Give your toes a bit of breathing room by buying the right shoes. “You want to get your shoes fit at a professional running shoe store, later in the day, when your feet are a little bit swollen,” says Mathews. The second most-common cause of black toes are hammertoes, says Dr. Mathews, which bend too far down. They can be straightened by a podiatrist.

GA Podiatrist Presents Kaplan-Kanat Memorial Lecture at Kern Reunion
In June 1956, Civic Hospital, the pioneer institution specifically limited to foot surgery was opened in Detroit under the guidance and direction of its founder and Chief of Staff, Earl Kaplan, DPM. Civic Hospital (later Kern Hospital) offered the first podiatric resident training program. Each year, former residents of the Civic-Kern program gather for their annual reunion and CME weekend.

This year, Dr. Bradley Bakotic presented the “Earl Kaplan - Irvin Kanat Memorial Lecture” to a crowd of more than 70 attendees who convened this past weekend for the reunion in Palm Coast, FL. This annual meeting is recognized for its outstanding CME program and is now open to all podiatric physicians and podiatric resident training programs throughout the country.

New Foot Patch Detects Diabetic Neuropathy
Diabetes patients could soon be able to check for nerve damage using a home device which works by detecting sweat. The new stick-on pad changes color from blue to pink when all is well, but does not change if there are problems with the nerves. The new test - which is designed specifically for the feet - is much simpler and can be done at home. It works on the basis that damage of the nerves leads not only to a loss of sensation, but also a malfunctioning sweat system and, in turn, unusually dry skin on the feet.
The adhesive pad contains the blue salt anhydrous cobalt II chloride, which reacts and changes to pink when exposed to water. Patients just have to remove their socks to allow the foot to adjust to room temperature before applying the pad, called Neuropad, which is kept in place for around ten minutes. If there is no sweat, there will be minimal or no color change - this suggests early diabetic neuropathy.

New ICD-10 Transition Date Set for 2015
The Centers for Medicare and Medicaid Services (CMS) announced yesterday that it would require the use of the International Classification of Diseases, 10th Revision (ICD-10), set of diagnostic codes starting October 1, 2015. That is one year later than the ICD-10 transition date that was in effect until recently.
The CMS announcement, eagerly anticipated in the healthcare industry, was a response to Congress' passage in late March of a bill that prohibited the agency from setting a deadline for ICD-10 any earlier than October 1, 2015. That provision was part of legislation that also prevented a 24% reduction in physicians' Medicare payments.

The case of the unidentified soft tissue mass hinges on the patient’s refusal for a core needle biopsy. First of all, this is the best way to initially approach this lesion, if done properly in what would be the planned bed of resection of this lesion if it were malignant. The open incisional biopsy (also needing to be in the planned bed of resection if the lesion was malignant) is second best. If unsure about what the planned bed of resection would be, then do not do the biopsy and refer the patient to a tumor specialist, usually an orthopedic oncologist.
If the doctor’s presentation of this, which should certainly include the possibility of this being potentially dangerous, is not enough to get full consent by the patient, it is time to refer the patient out with full documentation in the notes as well as informing the primary care physician of the patient’s non-compliance. I cannot disagree with another poster’s recommendation of notifying your carrier, but the assumption of being bulletproof, no matter what we do, only applies if you don’t eventually get a summons and complaint.

This case appears to be a disaster in the making, with an invasive growing lesion of unknown origin/composition, and a non-compliant, resistant patient. Dr. Albright's response was on the money. We do biopsies BECAUSE we want know the content of a lesion. The patient's wish to know what the lesion is before she consents to further treatment is foolish, self-deluding, and dangerous. If she continues to decline standard of care treatment, document it thoroughly and send her back to her PCP. No matter how good your intentions are, some people will not be helped.

Gentian violet has a value but a small drop on the floor or clothes can make a big problem. I stopped using it after my final incident. Betadine full strength is a good alternative choice and easier to clean up.

Castellani's paint is not gentian violet, but a carbo-fuchsin, phenol, and rescorcinol mixture invented in 1928. It works as well as gentian violet for similar podiatric and dermatologic conditions.

I appreciate Dr. Sherman giving his kudos to a fellow DPM for giving a commencement address. However, I do find fault with his contention that it takes more than just sitting in one's office to be a great podiatrist. I find fellow DPMs who sell their services to other DPMs when they haven't seen a private practice patient in years to be self-serving. Every day, I walk the walk, talk the talk, and deal with the daily struggles of all the foot soldiers who do what we were trained to do. If you want a business degree, get yourself an MBA. If you want to be a great DPM, go out and treat the world.
