MD Podiatrist Warns About Dangerous Ways to Remove Calluses
 
While calluses typically don’t create medical issues, they can be annoyingly stubborn, which is why dermatologists and podiatrists often see patients complaining about them. When looking to remove a callus quickly, many people reach for a physical exfoliator like a foot file or pumice stone. These work but it pays to be choosy and proceed with caution, our experts recommended. “The only physical exfoliation that I recommend is a gentle pumice stone in the shower — nothing like a cheese grater,” says Priya Parthasarathy, DPM, a podiatrist based in Silver Spring, Maryland and a spokesperson for the American Podiatric Medical Association.

“Some people get blades and try to remove the dead skin themselves. Those are all very dangerous.” Harsh files damage the skin, creating the risk of injury and infection and the potential to make the callus worse, according to Dr. Parthasarathy. “Causing that area to be traumatized, one can cause the calluses to come back worse because your body’s trying to keep protecting itself,” she says.

 NY Podiatrist  Discusses How Toenails Should be Cut 
 
If you’re prone to getting ingrown toenails or have fragile toenails that constantly break, simply trimming them to a specific shape can help. According to Miguel Cunha, DPM, a board-certified podiatric surgeon and founder of Gotham Footcare, the best shape for toenails is flat across. “It’s important to avoid cutting your toenails round or angled to prevent the formation of ingrown nails,” says Cunha. “Once the nails are cut flat and straight across, the edges on either side can be filed down so that the corners are less pointy and slightly beveled to follow the shape of the toe.”

Length is also important. Though you don’t want to cut your toenails too short, it’s important to cut them regularly. “When your nails are too long, they can become damaged due to micro trauma of the nail, which is caused by the repetitive stress of your nails hitting your shoes with every step,” says Cunha. “It’s one of the main reasons why runners develop bruised toenails,” he adds. “Toenails should all have roughly the same shape, color, and thickness. If something is abnormal in the appearance of the nails it is important to determine the reason why by going to a foot specialist,” says Dr. Cunha.

10 Suggestions to be an On-time Physician
 
Many of these ideas can easily be implemented in any office, with minimal or no expenses and without adding additional staff. 
 
10. Estimate how long each visit will take. Follow-up and post-op patients require less time than a new patient. It may be challenging to determine how long a visit is needed for a newly diagnosed cancer patient. Consider seeing this patient at the end of the day to be thorough and answer all their questions.

In my first year of practice, I got bogged down one day and was significantly late in seeing a new patient who was waiting longer than he should have. It turns out he was a lawyer and he taught me a lesson I carried through my entire practice career of 35 years. Upon entering the treatment room, he said something like the following:
 
Dr. Lang, I respect that you are a professional and that your time is valuable. That is why I showed up on time or a few minutes early. However, I am also a professional and my time is also valuable. I am saddened to learn of the passing of Dr. Richard Buratti. As a teenager, I worked as a nurse’s aide in the operating room at Van Nuys Community Hospital where Dr. Buratti performed and taught surgery. I have fond memories of Dr. Buratti who treated all with kindness and respect. My thoughts and prayers go out to Dawn and the entire Buratti family.

I was saddened to hear of the passing of Richard Buratti. I worked for Dr. Buratti in his Northridge office in the nineties. He was a wonderful teacher and mentor. He was passionate about podiatry and loved to teach and was incredibly methodical in his approach to surgery. I was fortunate enough to earn a ride on his Cessna where he flew us to Catalina Island for a burger. My condolences to his wife Ilona and his daughter, Dawn. He was an asset to the profession.

RE: The Passing of Richard A. Buratti, DPM

I was one of the many 1st and 2nd year podiatric residents at the Southern California Podiatric Medical Center, which was an independent clinic and teaching institution where outside attending podiatrists and surgeons, including Richard Buratti, DPM, volunteered their time without pay to advance our profession by lecturing and teaching clinical skills and forefoot and rear-foot surgery, both in Los Angeles and at our two Mexico clinics in Tijuana and Mexicali that ran under the name of the Baja Crippled Children's Program. 

 
In addition, Dr. Buratti would allow the residents to come to his office and shadow him with his patients, and also scrub in on his private patient cases as well. We considered him to be a very progressive surgeon during that period of time in the early 1980s. He looked at the foot from a biomechanical perspective and was an excellent teacher of mid-foot and rearfoot anatomical surgical dissection for procedures that were considered advanced at that time, including calcaneal osteotomies, peroneal tendon balancing, and cuboid osteotomies.  
 
I am certain that I am not alone in saying that I enjoyed his teachings and I learned some of my surgical skills and clinical skills from him. He was a pleasure to have known, and to have worked with. He always had a calm and reassuring demeanor. To his wife Ilona who I have met many times, your husband was a fine man and a great teacher. To Dawn, and Ilona I send my condolences and best wishes.

I find it interesting how a post can be interpreted from a different lens and diametrically opposing conclusions are reached. In reading Dr. Tomczak's response to Dr. Roth, I read Dr. Roth's post differently.
 
While we should believe we are providing the best possible care, we have to face the facts; in private practice, our treatment protocols are at least partially driven by the insurance company policies. I do not read any impugning of the profession. Dr. Roth should believe he is providing the best care, as should you and I. Cash frees the practitioner from the shackles of the insurance company policies. Think of the man hours used on "meaningless use/MIPS" and the like. Dr. Roth reminds me that medicine made a deal with the devil years ago and...

It is unfortunate that local providers turfed their patients to Dr. Tomczak. However, in our area, this kind of turfing is rampant among those who accept insurance. Most of our local Latino population does not have insurance and many providers will not see them at all. 
 
Ironically, when I accepted insurance, I was taken to task by the administration of our local hospital because I gave a significant discount to all religious leaders (rabbis, priests, ministers, nuns, etc.). I was advised that I was in violation of the contracts with my insurance carriers and had to “cease and desist”. Now that I have no contacts with insurance companies, I have restored these discounts. I also see large numbers of patients who have no insurance and am able to provide care for them at a cost they can afford. 
 
Some of these patients are very sad. For example, Wednesday I saw a gentleman who had had an ingrown nail for months. He had been unable to find anyone to treat him until the local” free” clinic sent him to me. He is an uncontrolled diabetic and had osteomyelitis of the entire distal and part of the proximal phalanx. I had him admitted to the hospital which will end up eating the cost, and I will probably need to amputate at least his hallux. We have failed this patient at a huge cost to him and to our healthcare system. I do not think this is an issue of self-pay vs. insurance providers. It is a loss of the patient care mentality which should guide all of us.
